본 연구의 목적은 Tedeschi & Calhoun(2004)의 외상 후 성장 모델에 근거하여, 발달장애 자녀 부모의 양육 부담과 외상 후 성장과의 관계를 규명하는 것에 있다. 이에 선행연구 고찰을 통한 이론적 고찰과정을 통해 발달장애 자녀 부모의 양육 부담과 외상 후 성장과의 관계에서 인지적 정서조절전략(적응적 조절전략, 부적응적 조절전략)과 장애수용의 매개효과 검증과 그 관계에서 사회적 지지(공식적 지지, 비공식적 지지)의 조절효과 및 조절된 매개효과를 검증하고자 연구모형을 구성하였다. 또한 연구모형 검증에 앞서, 기존 외상 후 성장 측정 도구(PTGI)의 한계성을 발견하고, 이에 “질적 연구가 선행된 순차적 탐색적 설계” 방법을 사용하여 발달장애 자녀 부모 외상 후 성장 측정 도구를 개발하여 연구를 수행하였다.
연구수행 절차 및 방법은 다음과 같이 진행하였다. 먼저, 연구 진행에 대한 윤리적 검토를 위해 IRB 승인을 획득하였다. 연구의 첫 번째 단계인 발달장애 자녀 부모 외상 후 성장 측정 도구 개발을 위해 8명의 부모를 대상으로 심층인터뷰를 진행하였다. 인터뷰 결과를 바탕으로 예비문항을 구성한 뒤, 요인분석, 신뢰도 및 타당도 분석을 통해 최종 문항을 확정하였다. 다음으로 연구의 두 번째 단계인 연구모형 검증을 위해 구조화된 설문지를 사용한 설문조사를 실시하였으며, 연구참여자는 발달장애 자녀를 가정에서 양육하는 부모 총 200명이다. 본 연구의 주요 분석 결과는 다음과 같다.
첫째, 발달장애 자녀 부모 외상 후 성장 측정 도구를 개발하였다. 심층인터뷰 결과 및 전문가들의 내용타당도 검증을 거쳐 총 22문항의 예비문항이 개발되었으며, 문항 분석과 탐색적 요인분석, 신뢰도 및 타당도 검증을 통해 3개의 문항이 삭제되어 최종적으로 19문항이 확정되었다. 요인분석 결과, 본 측정도구의 하위요인은 총 3개로 “나의 삶 전반의 긍정적 인식변화”, “영적 성숙과 종교에 대한 관심증가”, “장애자녀 양육의 긍정적 영향 인식” 등으로 구성되는 것으로 나타났다.
둘째, 발달장애 자녀 부모의 양육 부담과 외상 후 성장과의 관계에서 인지적 정서조절전략(적응적 조절전략, 부적응적 조절전략)과 장애수용의 매개효과를 검증하였다. 그 결과, 양육 부담은 적응적 조절전략을 매개로 외상 후 성장에 직접적인 영향을 미치고 있었으며, 또한 적응적 조절전략과 장애수용을 순차적으로 매개하여 외상 후 성장에 영향을 미치고 있었다. 다음으로, 양육 부담이 부적응적 조절전략을 매개로 외상 후 성장에 미치는 영향을 검증한 결과 양육 부담이 부적응적 조절전략을 매개로 외상 후 성장에 직접적으로 미치는 영향은 발견되지 않았으나, 양육 부담이 부적응적 조절전략과 장애수용을 순차적으로 매개하여 외상 후 성장에 영향을 미치고 있는 것으로 나타났다. 또한 양육 부담은 장애수용을 매개로 외상 후 성장에 직접적으로 영향을 미치는 것으로 나타났다.
셋째, 앞서 경로가 검증된 발달장애 자녀 부모의 양육 부담이 적응적 조절전략과 장애수용을 매개로 외상 후 성장에 미치는 영향에서 사회적 지지의 영향력을 비공식적 지지와 공식적 지지로 구분하여 조절효과 및 조절된 매개효과를 검증하였다. 먼저, 비공식적 지지는 적응적 조절전략과 장애수용에 유의한 영향력은 검증되었으나, 조절효과 및 조절된 매개효과는 검증되지 않았다. 다음으로, 공식적 지지는 양육 부담과 적응적 조절전략과의 관계를 조절하지는 못했지만, 양육 부담과 장애수용의 관계를 조절하였다. 마지막으로 조절된 매개효과 검증 결과, 양육 부담과 외상 후 성장과의 관계에서 장애수용의 매개효과는 공식적 지지에 의해 조절되는 것으로 나타났다.
넷째, 발달장애 자녀 부모의 양육 부담이 부적응적 조절전략과 장애수용을 매개로 외상 후 성장에 미치는 영향에서 비공식적 지지와 공식적 지지의 조절효과 및 조절된 매개효과를 검증하였다. 그 결과, 비공식적 지지의 경우 조절효과 및 조절된 매개효과는 검증되지 않았다. 공식적 지지의 경우 양육 부담과 부적응적 조절전략과의 관계를 조절하지는 못했지만, 양육 부담이 장애수용을 매개로 외상 후 성장에 미치는 영향력을 조절하는 조절된 매개효과가 나타났다. 이에 본 연구는 주요결과를 토대로 발달장애 자녀 부모의 양육 부담 감소와 외상 후 성장 증진을 위한 정책적·실천적 제언과 후속 연구를 위한 제언을 하였다.

The purpose of this study is to investigate the relationship between caregiver burden and post-traumatic growth of parents of children with developmental disabilities, based on Tedeschi and Calhoun’s model of post-traumatic growth (2004). Therefore, a research model was developed to verify the mediating effects of cognitive emotional regulation strategy (adaptive emotional regulation strategy, maladaptive emotional regulation strategy) and acceptance of disability, as well as to determine the moderating effect and moderated mediating effect of social support (formal social support, informal social support) in the relationship between caregiver burden and post-traumatic growth of parents of children with developmental disabilities, based on a theoretical literature review of previous studies. In addition, before the research model was verified, the limitations of the existing post-traumatic growth measurement tool (PTGI) were discovered. Thus, the “sequential exploratory design preceded by qualitative research” method was used in this study to develop a tool for measuring post-traumatic growth of parents of children with developmental disabilities.
The research procedure and methods used in the study are as follows. First, research ethics approval was obtained from the Institutional Review Board (IRB). In the first stage of the study, in-depth interviews were conducted with eight parents to develop a tool for measuring post-traumatic growth of parents of children with developmental disabilities. After preliminary questions were formulated based on the interview results, the final questions were confirmed through factor analysis, reliability analysis, and validity analysis. Next, a survey was conducted using a structured questionnaire to verify the research model, which was the second stage of the study. The research participants included a total of 200 parents raising children with developmental disabilities at home. The main results of this study’s analysis are as follows.
First, a tool for measuring the post-traumatic growth of parents of children with developmental disabilities was developed. A total of 22 preliminary questions were created through in-depth interview results and verification of content validity by experts, and 3 questions were deleted through item analysis, exploratory factor analysis, and verification of reliability and validity, resulting in the final confirmation of 19 questions. As a result of the factor analysis, three sub-factors of the measurement tool were identified: “positive change in perception of my life in general,” “increased interest in spiritual maturity and religion,” and “recognition of the positive impact of raising children with disabilities.”
Second, the mediating effects of cognitive emotional regulation strategy (adaptive emotional regulation strategy, maladaptive emotional regulation strategy) and acceptance of disability were verified in the relationship between caregiver burden and post-traumatic growth of parents of children with developmental disabilities. The results showed that caregiver burden had a direct effect on post-traumatic growth mediated by adaptive emotional regulation strategy and also had an effect on post-traumatic growth mediated sequentially by adaptive emotional regulation strategy and acceptance of disability. Next, as a result of verifying the effect of caregiver burden on post-traumatic growth mediated by maladaptive emotional regulation strategy, caregiver burden was found to have no direct effect on post-traumatic growth mediated by the maladaptive emotional regulation strategy. However, caregiver burden was found to have an effect on post-traumatic growth, mediated sequentially by maladaptive emotional regulation strategy and acceptance of disability. In addition, caregiver burden was found to have a direct effect on post-traumatic growth, mediated by acceptance of disability.
Third, the moderating effect and moderated mediating effect of social support, which was divided into formal social support and informal social support, was verified in the relationship between caregiver burden and post-traumatic growth of parents of children with developmental disabilities, a relationship whose path was previously verified to be mediated by adaptive emotional regulation strategy and acceptance of disability. Informal social support was first verified to have a significant effect on adaptive emotional regulation strategy and acceptance of disability, but the moderating effect and moderated mediating effect were not verified. Next, it was found that formal social support did not moderate the relationship between caregiver burden and adaptive emotional regulation strategy but did moderate the relationship between caregiver burden and acceptance of disability. Finally, as a result of the verification of the moderated mediating effect, the mediating effect of acceptance of disability was found to be moderated by formal social support in the relationship between caregiver burden and post-traumatic growth.
Fourth, the moderating effect and moderated mediating effect of informal social support and formal social support were verified in the relationship between caregiver burden and post-traumatic growth, mediated by maladaptive emotional regulation strategy and acceptance of disability. As a result, the moderating effect and moderated mediating effect were not verified in the case of informal social support. In the case of formal social support, although the relationship between caregiver burden and maladaptive emotional regulation strategy could not be moderated, a moderated mediating effect was found in the relationship between caregiver burden and post-traumatic growth, mediated by acceptance of disability. Therefore, based on the main results, this study made policy and practical suggestions for reducing caregiver burden and promoting post-traumatic growth of parents of children with developmental disabilities, as well as suggestions for follow-up studies.
