CHAPTER I. Immediate Effects of Static and Proprioceptive Neuromuscular Facilitation Stretching of Hamstring Muscles on Straight Leg Raise, Craniovertebral Angle, and Cervical Spine Range of Motion in Neck Pain Patients with hamstring tightness: A Prospective Randomized Controlled Trial
Abstract
The cranio-cervical flexion exercise and sub-occipital muscle inhibition technique have been used to improve a forward head posture among neck pain patients with straight leg raise (SLR) limitation. However, little is known about the cranio-vertebral angle (CVA) and cervical spine range of motion (CROM) after applying stretching methods to the hamstring muscle. To compare the immediate effects of static stretching and proprioceptive neuromuscular facilitation stretching on SLR, CVA, and CROM in neck pain patients with hamstring tightness. 64 subjects were randomly allocated to the static stretching (n1 = 32) or proprioceptive neuromuscular facilitation (n2 = 32) stretching group. The SLR test was performed to measure the hamstring muscle's flexibility and tightness between the two groups, with CROM and CVA also being measured. The paired t-test was used to compare all the variables within each group before and after the intervention. The independent t-test was used to compare the two groups before and after the stretching exercise. There were no between-group effects for any outcome variables (p > .05). However, all SLR, CVA, and CROM outcome variables were significantly improved within-group (p .05). Experient II describes that twenty in neck pain patients with hamstring tightness was included, and were randomly allocated for 10 CCFE and 10 SMI group. All patients received each intervention protocol for one-session. The present results also showed that SLR, PA, CVA (sitting and standing), and CROM (flexion, extension, lateral flexion, and right and left rotations) after CCFE and SMI were significantly higher than those before the interventions (p .05). Results in the Experiment I and Experiment II findings suggest that the SS and PNF stretching should be emphasized to reestablish normal nerve gliding and neurophysiology to the nerve and surrounding tissues, reducing pain and improving function. These findings also recommend that both SS and PNF stretching may be very effective in reducing neural tension, increasing length in the hamstring and cervical area. Furthermore, the suboccipital muscles are attached to the cervical dura mater via myodural bridge connection. Based on this results, it is also probable that the reduced tension in the myodural bridge after the SMI technique and CCFE may alleviate dural enfolding, thereby reducing the provocation of the nociceptive pain mechanism. Therefore, these findings suggest that the direct release of the posterior neck area after the SMI technique and reciprocally eccentric lengthening of the suboccipital muscles after the CCFE facilitate greater stretching and reducing of the tone of the myodural bridge and hamstring muscles.
