Comparison of Dynamic Control Ratio and Lower Extremity Muscle Activity During Eccentric Hamstring Exercises

Purpose: This study aimed to investigate the dynamic control ratio(DCR) and lower extremity muscle activities during eccentric hamstring exercises. Methods: Sample size was determined to be 24 with 80% power, 0.6 effect size, and 0.05 significance, considering a potential dropout rate of 15%. To standardize muscle activities across all participants, the participants were instructed to perform maximal voluntary isometric contraction(MVIC), and activities of the vastus medialis oblique(VMO), vastus lateralis oblique(VLO), semitendinosus(ST), and biceps femoris(BF) were measured. The DCR was calculated by dividing the sum of the %MVIC of the ST and BF by that of the %MVIC of the VMO and VLO. The participants randomly performed the Nordic hamstring exercise(NHE), Supine leg curl, Single-leg deadlift. All exercises were performed for 5 s each, and the average activities in the middle 3 s (i.e., excluding first second and last second) were used for analysis. Results: The DCR was significantly lower during Supine leg curl(effect size=11.06, p<0.01) and Single-leg deadlift(effect size=11.21, p<0.01) than during the NHE. Further, VMO activity was significantly higher during Supine leg curl than during the NHE(effect size=11.24, p<0.01) or Single-leg deadlift(effect size=2.83, p<0.01). There was significantly higher VLO activity during Supine leg curl than during the NHE(effect size=12.19, p<0.01) or Single-leg deadlift(effect size=8.55, p<0.01) and was higher during Single-leg deadlift than during the NHE(effect size=6.58, p<0.01). There was significantly higher ST activity during Supine leg curl(effect size=13.91, p<0.01) and the NHE(effect size=10.71, p<0.01) than during Single-leg deadlift. BF activity was higher during Supine leg curl than during the NHE(effect size=4.74, p<0.05) or Single-leg deadlift(effect size=12.00, p<0.01) and during the NHE than during Single-leg deadlift(effect size=8.74, p<0.01). Conclusion: Appropriate dynamic control ratios were observed during Single-leg deadlift and Supine leg curl; therefore, these exercises can be considered for injury prevention and rehabilitation of the anterior cruciate ligament and hamstrings in clinical practice.
