Because of emergency department crowding, urgent patients are being delayed treatment, further the quality of emergency medical care is deteriorating, causing serious harm to public health. The purpose of this study is to find the optimal algorithm and characteristics of patients who stay for a long time that affects the overcrowding of emergency department. Methods: The data from this study are MIMIC-IV medical data(n=448,972) that visited Beth Israel Medical Center in Boston, USA from 2011 to 2019. Preprocessing was conducted to analyze data accurately and derive better prediction results of machine learning, then this study used 382,391 emergency department visits as data. 29%(n=111,037) of the total data were from patients who stayed over 6hours. The data used for machine learning training is basic information such as patient age and gender, Vital Check, acuity score, and patient's chief complaints. The results were compared by learning data with tree based ensemble algorithms(XGBoost, Light GBM, Decision Tree, Random Forest) which are the latest machine learning algorithms. Lastly Shapley Value was used to find characteristics that affect machine learning's prediction results. Results: The model trained with the XGBoost algorithm showed the best predictive performance, and elderly, high risk patients(Acuity Level 2), high temperature patients, and alcoholism patients(ETOH) influence the prediction results of long length of stays. Conclusion: Basic information and major discomfort symptoms of patients visiting the emergency department and the acuity index of medical staff can be used to predict the patient's long length of stay. This contributes to the efficiency of the hospital's operational process and increases patient service
