After delivery, women go through many physical changes, but regular exercise can improve those changes. The purpose of this study is to confirm the effect of Pilates exercise with and without 'Pilates breathing' on rectus abdominis muscle, back pain, and pelvic floor muscle function in postpartum women. A total of 18 women between the ages of 20 and 45 who were between 8 and 48 weeks after childbirth, were divided into 9 experimental groups and 9 control groups. The exercise group accompanied by Pilates breathing conducted 50-minute classes of Pilates breathing (10 minutes) and Pilates exercise (40 minutes) three times a week for 4 weeks. The exercise group without Pilates breathing performed 40-minute Pilates exercise classes 3 times a week for 4 weeks. Body composition tests (body weight, body fat, and skeletal muscle mass) were measured with InBody120. Rectus abdominis dilatation was measured using the self-measurement method and micrometer caliper digital ruler measurement tool, and the scales of low back pain were measured using the Visual Analog Scale and the Oswestry Disability Index. Pelvic floor muscle function was measured using the Pelvic Organ Prolapse Distress Inventory 20, and vaginal contraction force was measured using the e:Kegel vaginal pressure tester. All variables were measured twice; before and after exercise. After 4 weeks of exercise for both groups, the group which conducted Pilates exercise with Pilates breathing showed meaningful improvements in skeletal muscle mass (p = 0.046), chest circumference (p = 0.001), rectus abdominis dilatation (p = 0.001), back pain (There was a significant effect on pelvic floor muscle function (p = 0.007) and pelvic floor muscle function (p = 0.035). The group that conducted Pliates exercise without Pilates breathing also showed meaningful improvement in skeletal muscle mass (p = 0.035), chest circumference (p = 0.001), rectus abdominis muscle dilatation (p = 0.001), low back pain (p = 0.009), and pelvic floor muscle function (p < 0.001). However, compared to the group which conducted Pilates exercise without Pilates breathing, the group that conducted Pilates exercise accompanied by Pilates breathing method showed meaningful improvements in rectus abdominis muscle dilatation (p = 0.001). This study confirmed that Pilates exercise accompanied by Pilates breathing has a meaningful positive effect on rectus abdominis muscle dilatation in women giving birth compared to conducing Pilates exercise without Pilates breathing. Based on the results of this study, we suggest the use of Pilates exercise accompanied by Pilates breathing to improve the physical changes experienced after delivery.
