Regular exercise plays an important role in promoting health and fitness, and the effects of various types of exercise have been consistently reported. Although the effects of weight training and aerobic-weight combination training on body composition, muscle function, and hormonal factors have been reported, the results are still inconsistent, and comprehensive studies comparing the effects of different exercise methods have not been reported. Therefore, this study aimed to systematically analyze and meta-analyze the effects of weight training and aerobic-weight combination training on body composition, muscle hypertrophy, strength, and hormonal factors, as well as to determine the effects according to training methods. To achieve the objectives of the study, randomized controlled trials (RCT) were searched from the Pubmed, Cochrane Library, CNKI, Web of Science, and RISS databases. A total of 26 studies were selected based on the literature selection criteria. To comprehensively analyze the selected studies, effect sizes were calculated using the mean difference (MD) or standardized mean difference (SMD) values and the random effects model. The quality of the studies was evaluated using the Cochrane Risk of Bias tool, and publication bias was assessed using a funnel plot. Review Manager 5.4 software was used to integrate effect size analysis and subgroup analysis. The study results revealed the following: First, weight training showed a large effect size in body composition factors, including weight reduction, body fat percentage reduction, and waist circumference reduction. It also showed a moderate effect size in lean body mass increase. In muscle function factors, weight training demonstrated a large effect size in muscle hypertrophy and lower limb maximal strength increase. Additionally, weight training showed a moderate effect size in testosterone increase and a large effect size in growth hormone increase. Second, aerobic-weight combination training showed a large effect size in body composition factors, including weight reduction and body fat percentage reduction. It also showed a moderate effect size in lean body mass increase. In muscle function factors, it demonstrated a moderate effect size in muscle hypertrophy and a large effect size in lower limb maximal strength increase. Furthermore, aerobic-weight combination training showed a moderate effect size in testosterone and cortisol increase, and a large effect size in growth hormone increase. Third, when comparing the two training methods, weight training was more effective in reducing waist circumference, while aerobic-weight combination training was more effective in increasing lower limb maximal strength. In conclusion, both exercise programs can effectively improve body composition, muscle hypertrophy, strength, and hormonal factors. However, weight training was more effective in reducing waist circumference, and aerobic-weight combination training was more effective in increasing lower limb maximal strength. Therefore, it is suggested that designing exercise programs according to individual requirements and preferences can lead to more appropriate effects. These results can serve as foundational information for developing individualized and scientifically-based training program designs that cater to specific needs and preferences. Furthermore, they are expected to contribute to planning programs for health promotion.
