Dementia is a syndrome characterized by the decreased cognitive function that impairs an individual's ability to carry out daily activities. Many patients around the world have Alzheimer's disease (AD), which is the most common type of dementia, accounting for about 60-80% of cases. The most effective management of AD is to delay the progression of symptoms and aim to improve cognitive function through early diagnosis. By identifying the timing and risk of AD progression in high-risk patients, cognitive decline can be minimized through appropriate treatment. Mild cognitive impairment (MCI) is a state of cognitive impairment intermediate between normal aging and dementia. The risk of progressing to AD is higher in MCI patients than cognitively healthy individuals. Therefore, early screening and treatment decisions for MCI patients at risk of converting to AD will delay the progression of AD and reduce treatment costs. In this study, we aimed to develop deep learning (DL) survival model for AD progression in MCI patients, improve the prediction performance using neuroimaging modalities and visualize individual prediction. We compared DL model with traditional survival models such as Cox proportional hazards model (CoxPH) and random survival forest (RSF) using the time-dependent concordance index (Ctd index) to evaluate their 60-month AD progression prediction performance. And we validate the model with a permutation feature importance method and visualize individual prediction with survival functions and activation map. We enrolled 506 MCI patients who underwent magnetic resonance imaging (MRI), 18F-fluorodeoxyglucose (FDG), and amyloid-β (Aβ) positron emission tomography (PET) from the Alzheimer’s Disease Neuroimaging Initiative dataset. Using selected feature set including cerebrospinal fluid chemical composition and cognitive assessments, DL model, CoxPH and RSF yielded Ctd index of 0.8671, 0.8366 and 0.8660, respectively. Furthermore, the DL model achieved an improved Ctd index of 0.8755 when Aβ-PET images were included. In the feature importance analysis, Aβ-PET image was ranked top. The deep learning model could visualize individual AD progression prediction as a survival function and highlight critical prediction regions from personalized brain images. This study demonstrates that deep learning model could improve the prediction of AD conversion using Aβ-PET image and provide personalized prediction results.
