In these dogs, the presence of congenital and acquried portosystemic shunts and histopathologic findings were considered to represent a combination of multiple extrahepatic portosystemic shunts and noncirrhotic portal hypertension or portal vein hypoplasia.

문제 정의
본 증례에서는 20개월령의 암컷 코카 스파니엘에서 단순선천적 간문맥전 신단락증과 다발성 후천적 간문맥전 신단락증이 동시에 발생한 경우와 3년 령의 암컷 진도견에서 발생한 다발성 간외성 후천성 간문맥전신단락증에 대한 임상증상, 실험실적 검사, 영상진단학적 소견 및 핵의학적인 진단에 대해 언급하고자 한다.
제안 방법

대상 데이터
20개월령, 체중 8.5 kg 의 암컷 코카 스파니엘이 구토, 설사-, 식욕부진, 기면, 복수를 주증상으로 충청북도 소재 인근 동물병원에 내원하여 검사를 실시하던 중 aspartate transaminase(AST), alanine transaminase(ALT) 및 gamma-glutamyltrans- ferase (r-GTP) 수치의 증가와 총단백질 (total. protein) 과 알부민(albumin) 수치의 저하로 간질환이 의심되어 충북대학교 동물의 료센터로 의뢰되었다.
3년령, 체중 13.4 kg 의 암컷 진도견이 식욕부진과 기면을주증으로 하여 충북대학교 동물의료센터에 내원하였다.
또한 전반적인 문맥의 감소는 간으로의 관류가 적기 때문에 발생한다는 보고와도 일치했다(6). 증례 1의 경우 간문맥과 비슷한 직경을 가진 선천적인 혈관단락과 동시에 특발성 비경화성 문맥고혈압, 선천적 간문맥 저형성(primary portal vein hypoplasia), 특발성 간섬유화(idiopathic hepatic fibrosis), 정맥협착질환(venoocclusive disease) 등의 질환을 배제할 수 없으며, 이로 인해 점차적인 문맥흐름의 폐쇄에 의해 문맥 고혈압 발생함으로써, 다수의 작고 구불거리는 혈관단락이 발생된 것으로 추정해 볼 수 있다. 따라서 증례 1의 경우 선천적인간문맥전신단락증과 후천적 인 간문맥전신단락증이 동시 에 발생한 것으로 생각되었고, 술전 혈관양성조영사진과 Ameroid ring 장착 후 혈관양성조영방사선사진을 비교했을 때, 다수의작고 구불거리는 혈관이 확장된 것으로 미루어 수술적인 혈관폐쇄가 문맥 혈관 저형성 상태에서 문맥 고혈압을 증가시킨 것으로 생각할 수 있었다.
또한 초음파로 간문맥혈류속도 측정으로 문맥 고혈압을 확인할 수 있었으며, 충혈지수 측정을 통해 간경화는 발생하지 않은 것을 알 수 있었다(Fig 2C & D). 이 증례의 경우 신체검사, 혈청화학수치 및 영상진단학적 검사에서 후천적 간문맥 전 신단락증으로 진단되었다 . 후천적 간문 맥전 신 단락 증의 원인으로는 중증의 간질환, 선천적인 간혈관의 이상 질환 등에 의해 발생한다(6).

성능/효과
Tc-99m을 직장 내 주입하여 혈류 내 방사선 동위원소의 흐름을 관찰했을 때, 방사선동위원소가 간을 거치지 않고 심장을 통해 전신순환으로 유입되는 것을 관찰할 수 있었다(Fig 4).
혈청화학 검사에서 암모니아, AST, ALT, ALP, r-GTP 및글루코오스 수치는 상승되었으며, BUN, Total cholesterol 및 글로블린 수치는 감소하였다(Table 2).

그러나 동정맥루, 간문맥 폐쇄증은 초음파와 양성조영을 통해 배제할 수 있었다. 따라서 특발성 비경화성 문맥고혈압, 선천적 간문맥 저형성(pHmaiy portal vein hypoplasia), 특발성 간섬유화 (idiopathic hepatic fibrosis), 정맥협착질환(venoocclusive disease)이 증례 2의 경우에서 후천적 간문맥전신단락증의 원인으로 강하게 의심되었다.
