일반적으로 신경전도검사를 통해 나타난 DPN환자군의 전기생리학적 특징 다섯 가지는 무엇인가? | ① 신경병증의 중등도와 신경전도검사상에는 상관관계가 있으며, 비골신경의 신경전도속도(nerve conduction velocity), 복합근육활동전위의 진폭(compound muscle action potential amplitude)과말단잠복기(distal latency)가 주로 침범된다[10]. ② 질환에 이환된 기간과 신경전도검사의 이상소견 간에 상관관계가 있으며, 당뇨의 시작으로부터 기간이 증가함에 따라 비골신경의 운동 신경전도 속도의 감소가 관찰된다. ③ 혈당의 조절이 잘된 환자군이 그렇지 못한 환자군에 비하여 신경전도검사상 비정상 소견이 감소한다. 비 골신경의 운동신경전도검사상 당의 조절이 잘되지 않은 군이 조절이 잘된 군에 비해 전도속도가 현저히 감소한다. ④ 당뇨의 경과에 따른 추이에서 신경전도검사상의 비 정상소견은 이미 조기에 발생하며, 신경병증의 소견은 신경섬유의 길이에 의존성을 보인다[6]. ⑤ DPN환자들을 대상으로 탈수초화의 정도를 알아보기 위해 신경전도검사를 시행하여 분절성탈수초화의 특징인 전도차단의 정도를 조사한 결과 당뇨병성 신경병증에서는 전도차단이 일어나지 않는다고 하여 당뇨병성 신경병증은 탈수초화 보다는 축삭의 변성이 주로 일어남을 시사한바 있다[11]. |
