Purpose: This study was to examine the impact of acculturation and social support on the mental health among Korean-American registered nurses. Methods: Data were collected with a convenience sample of 203 Korean-American registered nurses living in New York state and New Jersey state from 8th May to 25th August in 2012. The structured questionnaires measured the degree of acculturation, social support, and mental health. Data analysis was conducted using t-test, ANOVA, Pearson correlation efficient, and Stepwise multiple regression. Results: Lower level of integration and higher level of marginalization were associated with the lower mental health. Social support was also positively related to mental health and had mediating effect on the relationship between acculturation and mental health. The five variables, including participants' educational background, work shift, integration, marginalization, and social support were significant to predict the level of mental health among Korean-American registered nurses, accounting for 41% of the variability. Conclusion: The results imply that higher level of social support and strong identity of both home and host culture will enable Korean-American registered nurses to maintain sound mental health.

본 연구의 자료수집은 2012년 5월 8일부터 8월 25일까지 미국 동부의 뉴욕 주와 뉴저지 주에 위치하고 있는 병원에 근무하는 한인간호사를 대상으로 하였다. 본 연구의 설문을 위해 먼저 뉴욕한인간호협회 회장에게 이메일과 전화로 본 연구의 취지와 목적을 설명한 후 협조를 요청하였고, 본 연구의 제1 저자가 직접 뉴욕한인간호협회 정기총회에 참가하여 설문을 배부하고 회수하였다.

데이터처리

대상자의 정신건강에 미치는 요인은 Stepwise Multiple Regression으로 분석하였다.

이론/모형

문화적응과 정신건강 간에 사회적 지지의 매개효과를 확인하기 위해 Baron과 Kenny (1986)가 제안한 3단계의 매개효과검정방법을 이용하여 매개효과를 검증한 후, 간접효과의 유의성 검정을 위해 Sobel test calculator를 이용하였다. Sobel test calculator는 매개효과 검증 모형에서 독립변수에서 매개변수의 비표준화된 계수값(A)과 표준오차값(SEa)를 구한 후, 매개변수에서 종속변수의 비표준화된 계수값(B)과 표준오차값(SEb)를 구한 값을 입력하면 매개변수가 독립변수에서 종속변수에 유의한 영향을 미치는지를 계산해준다(Statistics Calculators, 2013).
본 연구에서는 Barry (2001)가 문화적응모델 단계를 측정하기 위해 미국 사회 내에서의 아시아 사람들을 대상으로 개발한 동아시아인 문화적응 측정도구(The East Asian Acculturation Measure, EAAM)를 Noh (2007)가 번안한 도구를 사용하였다. 문화적응 도구는 총 29문항으로 동화(8문항), 분리(7문항), 통합(5문항), 주변화(9문항) 등 네 개의 하위요인으로 구성되어 있다.
사회적 지지는 Weinert (1988)가 개발한 PRQ-Ⅱ(Personal Resource Questionnaire-Ⅱ)를 Kim (2000)이 수정 · 보완한 21문항의 도구를 사용하였다. 각 문항은 4점 척도로 ‘전혀 그렇지 않다’ 1점에서 ‘매우 그렇다’ 4점으로 점수가 높을수록 지각된 사회적 지지가 높음을 의미한다.
정신건강을 측정하는 지표로 Goldberg (1972)가 일반인을 대상으로 개발한 ‘일반건강척도’인 GHQ (The General Health Questionnaire)를 Shin (2001)이 한국판 ‘일반정신건강척도’(The Korean General Health Questionnaire, KGHQ-20)로 표준화한 척도를 사용하였다. Shin (2001)은 번안하는 과정에서 일반건강척도라는 도구명이 도구의 목적 및 내용과 일치하지 않아 ‘일반정신건강척도’라는 용어로 표현하였다.

성능/효과

본 연구의 결과는 재미한인간호사의 최종학력과 근무형태, 문화적응 중 통합과 주변화, 사회적 지지 등의 변인이 정신건강 수준과 유의한 연관이 있음을 확인하였다. 또한, 사회적 지지는 문화적응과 정신건강과의 관계에서 매개효과로 확인되었다. 따라서 재미한인간호사의 정신건강을 향상시키기 위해 문화 적응력이나 사회적 지지와 네트워크를 사정하는데 보다 많은 관심을 가질 필요성을 제안한다.
대상자의 정신건강에 미치는 영향요인을 규명하기 위해 문화적응 중 통합과 주변화, 사회적 지지와 일반적인 특성 중 정신건강에 유의한 차이를 나타냈던 최종학력과 근무패턴을 더미변수 처리하여 단계적 다중회귀분석을 실시하였다. 그 결과 대상자의 최종학력과 근무패턴은 전체의 6%의 설명력을 갖는 것으로 나타났으며(F=3.04, p=.007), 이들과 문화적응 중 통합과 주변화를 포함시켰을 때는 전체의 32%의 설명력을 가졌으며(F=16.65, p<.001), 사회적 지지를 포함하여 이들을 모두 포함시켰을 때의 영향요인은 대상자의 정신건강을 총 41% 설명하는 것(F=16.65, p<.001)으로 나타났다(Table 4).
본 연구의 결과는 재미한인간호사의 최종학력과 근무형태, 문화적응 중 통합과 주변화, 사회적 지지 등의 변인이 정신건강 수준과 유의한 연관이 있음을 확인하였다. 또한, 사회적 지지는 문화적응과 정신건강과의 관계에서 매개효과로 확인되었다.
