Methods : We used official Ten 'Selected Korean Medical Classics(SKMC)'. For input of formulas from SKMC, we had to select proper editions of them. After consultations with experts of 'The Korean Medicine Society for the Herbal Formula Study' and 'Korean Society of Medical History', we chose the proper editions of SKMC, and according to those books, we built formulas database using microsoft office excel sheets, respectively. Results : After whole input procedures, we requested several experts to inspect the results and also, to correct errors in them. Input categories were name of formula, name of classic, classification, original source, composition herb, weight, unit of weight, weight conversion index, converted weight, basic formula, indications, reason for modification, preparing method, taking method and contraindications. Final number of whole formulas in DB was 32,882. Conclusions : We used SKMC and built database using excel program. Through this procedure, we can provide a developmental ways to make a foundation for herbal formulas from the medical classics, so that we can make herbal formulas be standardized or industrialized more efficiently.

본 연구의 목적은 10종 기존한약서를 엑셀(Excel) 서식에 입력하고 각 처방(處方)의 구성 약재와 그 용량을 입력한 후「한약(생약)제제 등의 품목허가·신고에 관한 규정」의 단위 환산 기준5)에 따라 일괄적으로 용량을 현대화하여 현행 한약제제(韓藥製劑) 허가 기준에서 기존한약서의 기성처방을 활용 시 참고하고자 함이다. 그러나 처방(處方)을 엑셀(Excel) 서식에 입력하는 과정에서 몇 가지 문제가 발생했으며, 대부분은 처방(處方)의 구성에 관한 내용이었다.
제안 방법
원문(原文)은 우선순위대로「동의보감(東醫寶鑑)」과 「방약합편(方藥合編)」순으로 입력을 시작하고 사전에 논의되지 않은 상세한 입력 방법을 실제 입력을 통하여 구체화하여 확정한 후, 나머지 8종의 기존한약서 입력을 시작하였다. 또한 단방(單方) 위주인「향약집성방(鄕藥集成方)」과「본초강목(本草綱目)」을 가장 나중에 입력하였다.
이를 위해서 한약제제(韓藥製劑)의 안전성 및 유효성 심사 면제 근거가 되는 한약서 수재 처방에 대한 표준화 작업이 반드시 선행되어야 하며, 이에 본 저자는 한약서 수재 처방에 대한 처방량·적응증·복용법·제조 방법 등에 대한 내용을 Database(DB)화하여 전통적으로 사용되던 10종 기존한약서의 내용 중 처방에 대한 부분을 전산화하였다.
또한 동의보감(東醫寶鑑), 경악전서(景岳全書), 의학입문(醫學入門)은 해당 서적을 출판한 법인문화사(法仁文化社)의 지원으로 전산화 및 교감이 완료된 문서를 받아서 사용하였다. 입력되는 항목은 ‘순번, 처방명, 수재문헌명, 위치, 최초출전, 조성, 포제법, 용량, 용량 단위, 단위 환산 기준, 가감 기본방, 효능·효과, 가감근거, 제형, 제조방법, 용법, 금기 사항’을 입력하였다. 처방명과 조성, 효능·효과, 제조 방법 및 용법은 사용자의 편의를 위해 한글과 한문을 병기(倂記)하였으며, 그 이외의 것은 한글로만 입력하였다.
지금까지 현행 규정 상 한약제제(韓藥製劑)의 한방원리로 활용되며 유효성 및 안전성 심사의 면제 근거가 되는 10종 기존한약서의 판본(板本)과 구체적인 내용 및 해당 한약서 기성처방의 DB화한 방법에 대하여 포괄적으로 기술하였다.
선정된 판본(板本)의 전산화된 원문(原文)을 입수하여 이를 마이크로소프트 오피스 엑셀(Microsoft office excel) 서식에 항목별로 입력하였다. 처방(處方)의 누락(漏落)·와전(訛傳) 및 오·탈자(誤·脫字)를 방지하기 위하여 자료를 서식에 입력한 이후, 다시 한 번 전산화된 원문(原文)과 입력된 자료를 비교하였다. 또한 방제학(方劑學)과 의사학(醫史學) 전공 전문가에게 10종 기존한약서 입력파일의 수정 및 검토를 의뢰하였다.
입력되는 항목은 ‘순번, 처방명, 수재문헌명, 위치, 최초출전, 조성, 포제법, 용량, 용량 단위, 단위 환산 기준, 가감 기본방, 효능·효과, 가감근거, 제형, 제조방법, 용법, 금기 사항’을 입력하였다. 처방명과 조성, 효능·효과, 제조 방법 및 용법은 사용자의 편의를 위해 한글과 한문을 병기(倂記)하였으며, 그 이외의 것은 한글로만 입력하였다.

대상 데이터
1578년에 초고(草稿)가 완성된 이후로 수많은 판본(板本)이 인쇄되고 간행되었으나, 1593년 중국 강소성(江蘇省)의 남경(南京)에서 이시진(李時珍)의 후손인 호승룡(胡承龍)이 간행한 금릉본(金陵本)이 최초이자 현존하는 유일한 판본(板本)이며 이후 출간된 판본(板本)들의 원전(原典)에 해당하므로16,18) 본 연구에서는 금릉본(金陵本)을 기준 판본(板本)으로 선정하고 해당 판본(板本)을 사용하여 출판(出版)된 인민위생출판사(人民衛生出版社)의「본초강목(本草綱目)」을 기준 문헌으로 사용하였다15).
2007년 법인문화사(法仁文化社)에서는, 상술한 세 가지 판본(板本)을 기본으로 1989년 간행(刊行)된 북경(北京) 인민위생출판사(人民衛生出版社)의 「경악전서(景岳全書)」를 저본(底本)으로 삼아, 사고전서(四庫全書)와 비교․분석하여 교감(校勘)한 「현토주석경악전서(懸吐註釋景岳全書)」를 출간(出刊)23)하였으며, 본 연구에서는「현토주석경악전서(懸吐註釋景岳全書)」를 기준 문헌으로 사용하였다.
「광제비급(廣濟秘笈)」의 판본(板本)은 정조(正祖) 14년인 1790년에 중간(重刊)된 후쇄본(後刷本) 뿐이므로28), 본 연구에서는 이를 기준 판본(板本)으로 선정하고 사용하였다.
본서(本書)의 저술(著述)이 완료된 이후, 정조(正祖)는 이를 주자소(鑄字所)에 보내어 간행(刊行)하고 이를 널리 배포하였다30). 국립중앙도서관의 선본해제(善本解題)와 한국 민족문화 대백과사전을 참고하면30,31), 「제중신편(濟衆新編)」의 판본(板本)은 하나뿐이라고 추정되며, 따라서 본 연구에서는 정조(正祖) 23년인 1799년에 간행(刊行)된 판본(板本)을 사용하였고, 해당 판본(板本)을 사용하여 출판(出版)된 여강출판사(驪江出版社)의「제중신편(濟衆新編)」을 기준 문헌으로 선정하고 함께 사용하였다15).
따라서 본 연구에서는 현대에 출간(出刊)되는「방약합편(方藥合編)」의 원류(原流)인 고종(高宗) 24년의 「증맥방약합편(證脈方藥合編)」을 기준 판본(板本)으로 선정하였다15).

본 연구는 식품의약품안전처 용역연구개발과제인 ‘한약서 수재 처방에 대한 과학적 평가 연구(13172천연물410)’의 일환으로 수행되었고, 해당 연구에 사용한 10종 기존한약서의 기준 판본(板本)을 대한한의학방제학회(大韓韓醫學方劑學會)와 한국의사학회(韓國醫史學會)의 자문과 승인을 받아 식품의약품안전처에 고지한 뒤 DB구축에 이용하였으며, 이 10종 기존한약서의 판본(板本)은 다음과 같다.

본 연구에서는 상술한 네 가지 판본(板本)을 상호비교 및 대조의 과정을 거쳐 간행(刊行)한 북경(北京) 인민위생출판사(人民衛生出版社)의 「수세보원(壽世保元)」을 사용하였으며, 이 책에서는 앞서 말한 네 가지 판본(板本)을 모두 참고하여, 오·탈자(誤·脫字) 및 와전(訛傳)된 내용이 수정되었다22). 이 중 기본이 되는 판본(板本)인 저본(底本)은 1645년의 정보본(正保本)이기 때문에, 기준 판본(板本)은 쇼호(正保) 2년의 정보본(正保本)으로 선정하고, 해당 판본(板本)을 사용하여 출판(出版)된 인민위생출판사(人民衛生出版社)의 「수세보원(壽世保元)」을 기준 문헌으로 사용하였다15).
본 연구에서는 순조(純祖) 14년인 1814년에 내의원(內醫院)에서 교정한 완영중간본(完營重刊本)을 사용하였으며, 해당 판본(板本)은 2012년 법인문화사(法仁文化社)에서「신증보대역동의보감(新增補對譯東醫寶鑑)」이라는 이름으로 출판되었기에27) 이를 기준 문헌으로 사용하였다.
세종(世宗) 15년인 1433년 간행되고 이후 성종(成宗) 9년인 1478년에 복간(復刊)되었으며13), 본 연구에서는 1478년에 복간(復刊)된 것을 기준 판본(板本)으로 선정하고 해당 판본(板本)을 사용하여 출판(出版)된 여강출판사(驪江出版社)의「향약집성방(鄕藥集成方)」을 기준 문헌으로 사용하였다15).
「의학입문(醫學入門)」은 처음 편찬된 이후 수차례 다시 간행(刊行) 되었으며, 이에 따라 현존하는 판본(板本)만도 20여 종에 이른다. 우리나라에서 간행(刊行)된 판본(板本)은 5〜6종에 이르고, 이 중 현재 가장 많이 볼 수 있는 판본(板本)은 순조(純祖) 18년인 1818년에 간행(刊行)된 무인내국중교개간본(戊寅內局重校改刊本)이며20), 이 판본(板本)은 2009년 법인문화사(法仁文化社)에서「신대역편주의학입문(新對譯編註醫學入門)」이라는 이름으로 출판되었기에 이 서적을 본 연구에 사용하였다15).
「남풍현지(南豊縣誌)」에는‘이천(李梴)은 자가 건재(健齋)이며 읍상(邑庠)에서 태어났는데 재주가 뛰어났다. 융경(隆慶, 1567-1572)과 만력(萬曆, 1572-1615)의 번성하던 때에 병이 난 것을 계기로 의학 속으로 은거하였으며 「의학입문(醫學入門)」 8권을 저술하였다(李梴字健齋, 邑庠生, 負奇才, 當隆萬盛時以病隱于醫, 輯醫學入門八卷).’라고 기록되어 있다. 「의학입문(醫學入門)」 인(引)에는 ‘내 자신이 병이 많아 백약을 두루 복용하였으나 결국 낫지 못하였고, 반드시 맛을 보았던 탕약(湯藥)이라도 그 약이 그렇게 쓰여야 하는 까닭을 통달하지 못하였다.

초간본(初刊本)이 간행(刊行)된 이후「동의수세보원(東醫壽世保元)」은 여러 차례에 걸쳐 목활자본과 연인본(鉛印本), 그리고 석인본(石印本)으로 재차 간행(刊行) 되었으며32), 본 연구에서는 1901년의 신축본(辛丑本)을 기준 판본(板本)으로 선정하였다.
이론/모형
상술한 방법대로 입력된「방약합편(方藥合編)」의 상통(上統) 40번째 처방인 육미지황환(六味地黃丸)과 그 가감방(加減方) 일부를 간략하게 묘사해 보면 이는 Table 2와 같다. Table 2에서 사용된 영문 처방명은 국어의 로마자 표기법37)을 참고하여 작성하였으며, 구성약재의 영문 명칭은「대한약전 제 11 개정」36)의 의약품각조 제 2부를 참고하여 작성하였다.
선정된 판본(板本)의 전산화된 원문(原文)을 입수하여 이를 마이크로소프트 오피스 엑셀(Microsoft office excel) 서식에 항목별로 입력하였다. 처방(處方)의 누락(漏落)·와전(訛傳) 및 오·탈자(誤·脫字)를 방지하기 위하여 자료를 서식에 입력한 이후, 다시 한 번 전산화된 원문(原文)과 입력된 자료를 비교하였다.
성능/효과

후속연구
또한 등분(等分)이라는 표현을 사용한 처방(處方)은 1회 복용량을 설정하는 연구가 진행될 필요가 있다. 결과적으로 한약제제(韓藥製劑)의 표준화를 위해서는 일정한 기준 혹은 특정 질환에 부합하는 용량 설정이 선행되어야 한다고 생각된다.
또한 기존한약서의 처방(處方)을 구성하는 약재의 용량이 비도량형 단위인 경우 셀(Cell)에 용량을 나타내는 숫자 대신에 문헌에 수재된 대로‘얼마 안 되는 적은 분량(分量)’을 뜻하는38) 소허(少許)를 입력하였을 경우, 소허(少許)가 숫자가 아니고 한글인 관계로 엑셀(Excel) 서식에서 인식하지 못하고 오류가 발생하였다. 그러나 오류를 피하기 위하여 소허(少許)를 크지 않은 적당한 숫자로 한정하여 입력한다 하더라도 그 숫자를 무엇으로 할 것인지에 대한 추가적인 연구가 필요하다고 생각되어 도량형 단위로 기재되지 않은 경우에 대한 비도량형 계량단위에 대한 연구를 진행 중이다.
적응증이 요혈(尿血)인 처방(處方)을 검색할 경우, 같은 의미의 적응증이지만 혈뇨(血尿)나 적혈(赤血)의 적응증을 가진 처방(處方)은 전산 상에서 배제될 것이기 때문이다. 따라서 동일 적응증에 대한 유사 단어 DB 구축이 요구되며 효율적인 적응증 분류와 전산 처리 절차에 대한 추가적인 연구가 필요하다.
과거에 사용되던 구시대의 유물이 아니라 현재에도 사용되고 있는 의약품이라는 관점에서 한약제제(韓藥製劑)를 바라본다면, 한약제제(韓藥製劑)를 표준화하는 과정은 반드시 필요한 절차이다. 때문에 한의약의 발전을 위해서는 전통적으로 사용된 한의약의 전모를 파악하여 체계적으로 가시화하는 과정이 필요하며 이는 본 연구와 같이 전통 의약학(醫藥學)에 관한 내용들을 한 곳에 모으고, 이후 이를 적절한 방법으로 분류하고 선후경중을 나누어 다시 현대화하는 방법으로 가능하다. 이러한 과정을 통해서 전통적으로 사용되던 한의약이 다시 발전하는 계기가 될 수 있을 것이라 기대한다.
약재의 용량이 없는 것은 환자에 따라 의사 나름의 기준으로 그 용량을 적절히 가감(加減)하여 쓰라는 의미로 추측해 볼 수 있다. 또한 등분(等分)이라는 표현을 사용한 처방(處方)은 1회 복용량을 설정하는 연구가 진행될 필요가 있다. 결과적으로 한약제제(韓藥製劑)의 표준화를 위해서는 일정한 기준 혹은 특정 질환에 부합하는 용량 설정이 선행되어야 한다고 생각된다.
한약서에 취약군에 대한 안전정보가 부족하다는 문제 또한 존재한다. 소아(小兒)·노인(老人)·부인(婦人) 등은 대표적인 취약군이며, 평균 수명이 연장되고 난치성 질병이 늘어나는 현 추세에서 한약제제(韓藥製劑)의 효율적이고 안전한 사용을 위해서는 기존한약서에 명시되어 있지 않은, 취약군에 대한 금기(禁忌) 사항이 포함된, 복용법 등의 세부적인 내용을 추가적으로 연구·개발하여 제시해야 할 필요가 있다.
때문에 한의약의 발전을 위해서는 전통적으로 사용된 한의약의 전모를 파악하여 체계적으로 가시화하는 과정이 필요하며 이는 본 연구와 같이 전통 의약학(醫藥學)에 관한 내용들을 한 곳에 모으고, 이후 이를 적절한 방법으로 분류하고 선후경중을 나누어 다시 현대화하는 방법으로 가능하다. 이러한 과정을 통해서 전통적으로 사용되던 한의약이 다시 발전하는 계기가 될 수 있을 것이라 기대한다.
