IRLS, STAI-X1,X2, BDI-2, BAI, STAXI-S,T were compared before and after 4 and 8 weeks of treatment to determine whether the symptoms and mood of patients were improved. Results: After 4 weeks treatment (n=13), STAI-X1,X2, BDI-2, BAI, STAXI-S scores increased significantly; whereas, IRLS and STAXI-T scores decreased. After 8 weeks treatment (n=8), IRLS, STAI-X1,X2, BDI-2, BAI scores increased significantly; whereas, STAXI-S scores decreased.Conclusions The treatment of RLS with Korean medicine was clinically efficient and resulted in improvement of the symptoms as well as psychological problems.

이에 저자는 OO 한방신경정신과 한의원에 내원한 하지불안증후군 환자를 대상으로 하여 한약치료, 침 치료 및 한방정신요법의 한의학적 치료를 시행하였으며, 치료 전후의 증상 및 심리척도의 측정을 통해 치료 효과를 연구하고자 하였다.

제안 방법
0.25×30 mm의 1회용 호침(豪針)을 사용하였으며, 양측 백회(百會), 사신총(四神總), 태충(太衝), 내관(內關), 합곡(合谷) 등과 비증(痺證)의 치료혈인 위중(委中), 양릉천(陽陵泉), 족삼리(足三里)24) 등의 혈을 배합하여 주 1회, 유침 시간은 20분으로 하여 시술하였고, 필요에 따라 침전기자극술을 20분 병행하였다.
불안, 우울감을 낮추기 위해 주 1회 한방정신요법을 시행하였다. 오지상승위치요법(五志相勝爲治療法), 이정변기요법(移精變氣療法), 지언고론요법(至言高論療法) 중 환자의 증상과 상황에 따라 선택하여 침치료 전 또는 후에 30분씩 실시하였다.

대상 데이터

이론/모형
BDI는 1961년에 개발된 이래 현재까지 전 세계적으로 널리 사용되고 있는 척도 중의 하나로 Beck에 의해서 우울증의 심도를 측정하기 위하여 개발되었다. 1996년 수면과 식욕에 대한 문항이 개정된 BDI-2가 새로 만들어졌고, 현재까지는 이 문항들이 사용되고 있다.
본 진단기준은 IRLSSG에서 개발한 선별검사도구로서, 다음 항목들을 포함하고 있다.
상기 네 가지 항목을 모두 만족시킬 경우 RLS라 진단하였다.
하지불안증후군의 진단은 한방신경정신과 전문의의 면담을 통하여 국제 RLS 연구모임 진단 기준(IRLSSG diagnostic criteria)을 이용하였으며, 중증도나 심리 상태에 대한 평가는 IRLS18), BDI-219), STAI20), BAI21), STAXI22)를 이용하였다.
성능/효과
하지불안증후군의 중등도를 평가하는 척도인 IRLS는 10가지 항목으로 되어 있으며, 각각의 점수를 합산하여 최소 0점에서 최고 40점까지 표시되며, 점수가 높을수록 하지불안증후군의 증상이 심한 것으로 평가한다. 매우 심함 31∼40점, 심함 21∼30점, 보통 11∼20점, 약함 1∼10점, 없음 0점을 기준으로 한다.
하지불안증후군의 유병률은 미국과 유럽에서는 7.2∼11.5%로 보고되고 있고, 한국에서는 약 7.
불안 경험을 측정하기 위하여 상태 불안(STAI-X-1) 20문항, 특성 불안(STAI-X-2) 20문항으로 구성되어 있으며, 각 문항은 4점 척도로, 1-‘전혀 그렇지 않다’, 2-‘조금 그렇다’, 3-‘상당히 그렇다’, 4-‘매우 그렇다’로 평가한다.
4주간 한방치료를 시행한 13명 중, 총 8주간 치료를 지속한 환자 6명에 대한 치료 전후 평과를 분석한 결과, 상태분노와 특성분노를 제외한 나머지 모든 수치에서 유의한 감소를 보였다. 하지불안증후군의 중등도는 4주 치료 시 유의미한 감소를 보이지 않았으나, 8주간 치료를 지속한 결과 치료 전 29.
4주간의 치료 전후 평가 결과를 분석한 결과, IRLS와 STAXI-T를 제외한 나머지 모든 수치에서 유의한 감소를 보였다. 상태불안(STAI-X-1)은 치료 전 56.
78점으로 유의미하게 감소되었다. 따라서 8주간의 지속적인 한방치료가 하지불안증후군 증상을 호전시키는 데에 더 효과적이라고 생각된다. 상태불안은 치료 전 54.
92점으로 경도 이하의 수준이었다. 따라서 본원에 내원한 환자들은 하지불안증후군 중증도도 높고, 동반된 우울 및 불안 수준도 정상인 평균에 비해 높은 것으로 나타났다.
이상의 연구에서 하지불안증후군 환자에게 한약치료, 침 치료 및 한방정신요법을 통합적으로 적용한 결과 증상 심각도와 심리상태가 효과적으로 개선되었으며, 4주보다는 8주간 지속적으로 치료 받은 경우 그 효과가 유의하게 지속이 되었음을 알 수 있었다. 또한 기존의 연구 결과에서 보여지듯이 하지불안증후군의 적절한 치료를 위해서는 우울이나 불안 등의 심리상태에 대한 접근도 중요한데33), 한의학적 치료를 통해 신체적, 심리적인 상태를 함께 개선함으로써 더 효과적으로 작용하였다는 것에 본 연구의 의의가 있다.
2. 총 8주간 치료를 지속한 6명에 대한 한의학적 치료 효과를 비교한 결과, IRLS, STAI-X-1, STAI-X-2, BDI-2, BAI에서 모두 유의한 감소를 보였으며, STAXI-S는 감소했으나 유의성은 없었고, STAXI-T는 다소 증가되었다.
하지불안증후군 환자 13명에 대한 한의학적 치료 효과를 치료 전과 4주 후를 비교해본 결과, IRLS와 STAXI-T를 제외한 나머지 모든 수치에서 유의한 감소를 보였다.

3. 한약치료, 침치료, 한방정신요법의 통합적인 치료가 하지불안증후군 환자의 증상 뿐 만이 아니라, 우울, 불안, 분노와 같은 전반적 심리상태를 개선시키는 데에 효과적임을 알 수 있다.
후속연구
추후에 다양한 인구학적 특성을 가진 대규모 집단에 대해 대조 연구를 실시하고 추적 관찰을 통해 지속적인 효과를 검증하며, 독립적 치료효과에 대해 추가 연구를 하는 등의 보완이 필요할 것으로 사료된다. 또한 동의서를 받았음에도 절차 시 IRB 심사를 규정대로 받지 않은 제한점이 있는데, 추후에 이 점을 보완하여 관련 연구를 시행할 필요가 있을 것이다.
