Objectives: The purpose of this study is to investigate cognitive function, performance of activities of daily living, and recognition on oral health with the cognitive function testto dementia or dementia-suspected patients in the outpatients. Methods: The subjects were 94 dementia or dementia-suspected patients visiting C University hospital for the dementia test. Study instruments included Korea Mini-Mental State Examination KMMS, The Bayer-Activities of Daily Living Scale; B-ADL, Seoul-Instrumental Activities of Daily Living; S-IADL, Global Deterioration Scale; GDS, Korean Dementia Screening Questionnaire; KDSQ, and underlying diseases. Results: Dementia or dementia-suspected patients were 42 by KMMSE test, 25 patients had impaired functioning of daily living by B-ADL test, 27 patients showed the presence of depression by GDS test, and 45 patients showed impaired functioning of daily living. There was a statistically significant difference in the subjective recognition on oral health conditions. There was a statistically significant difference in the subjective recognition on oral health conditions by ADL. There was a positive correlation between the cognitive function and ADL performance. Higher cognitive function is proportional to ADL performance. Conclusions: The cognitive function was closely associated with ADL and subjective oral health conditions.

본 연구대상은 2010년 11월부터 2011년 12월까지 인지기능 장애를 주 증상으로 치매검사를 받기 위해 C대학 부속병원 검사실을 방문한 100명의 환자를 대상으로 치매검사와 함께 구강건강실천행위 관련 설문을 시행하였으며 실제 분석에 사용된 응답자는 94명이었다. 연구도구는 한국판 치매검사 내용 중 인지기능검사(Korea Mini–Mental State Examination KMMSE)점수, 일상생활활동지표(The Bayer-Activities of Daily Living Scale; B-ADL, Seoul-Instrumental Activities of Daily Living; S-IADL)을 통한 일상생활수행능력검사를 측정하였다.

연구분석은 SPSS 18.0을 사용하여 검증하였으며 대상자의 일반적 특성은 서술적 통계를 산출하였고 인지기능검사, 우울 정도, 일상생활수행능력(환자응답), 일상생활수행능력(가족응답)등과 구강관련특성의 차이는 t-검정 및 F-검정을 하였으며 인지기능과 관련한 각 변수들 간의(KMMSE, B-ADL, KDSQ, S-IADL, GDS 등) 상관관계 분석을 하였다.

. 본 연구결과에서는 인지기능에 따라 구강건강인지에 차이가 있는 것으로 나타났으며 일상생활 기능수행정도에 따라서도 구강건강인지정도에 차이가 있는 것으로 나타났다. 노인에게 있어 일상생활 기능수행 중 개인위생 중 구강관리는 중요하며 신체적 노화 뿐 아니라 인지기능의 변화까지 오는 치매환자는 다양한 측면의 기능장애로 타인에게 의존된 삶을 살아야 하지만 서서히 악화되는 진행과정에서 손상되지 않고 보존된 기능중 개인위생기능인 구강관리을 적절히 유지하는 인지를 최대로 활용한다면 독립된 삶을 연장할 수 있다¹⁴⁾.

본 연구는 일 개 대학병원에 치매진단을 받기 위해 내원한 환자를 무작위로 조사하였으므로 치매환자 전체로 확대해석하기에는 제한성을 가질 수 있음을 제시한다.

	치매에 걸리면 어떤 순서로 장애가 나타나는가	치매는 진행의 단계에 따라 비교적 기억력 장애가 빨리 나타나는 개인위생, 옷입기 등의 일상생활 활동부터 기억력 장애가 가장 늦게 나타나는 식사하기의 기능적 행위가 점진적으로 퇴행된다고 한다. 즉 임상적 경과과정으로 볼 때 2가지 이상의 인지기능 장애가 제일 먼저 나타나고 우울이나 불안 등 성격의 변화와 이상 행동을 동반하는 장애가 나타나며 결국엔 일상생활이나 사회생활의 장애 순서로 장애가 나타난다는 것이다6, 7).
	노인성 치매환자가 가지는 문제에는 어떤 것들이 있는가	노인성 치매환자는 여러 가지 문제들을 같이 갖고 있는 경우가 많다. 소위 치매, 언어장애, 섬망, 요실금, 청각 및 시각장애, 영양불량, 골다공증 및 골연화증, 낙상 및 보행장애, 욕창, 수면장애, 각종 통증들이다3,4). 65세 이상 치매노인은 40만명으로 9%이상을 나타내고 있으며 2010년에는 46만 1천명, 2020년이 되면 69만 3천명으로 추정되고 있다5).
