중환자실 입원 환자에게 욕창이 흔한 이유는 무엇인가? | 중환자실에서 욕창이 흔한 이유는 첫째, 환자가 응급실, 일반 병동, 수술실 등 다른 장소에 체류한 후 중환자실로 입실하는 경우가 많기 때문이다. 응급실의 경우 입원이 지연되면 응급실에서의 대기 시간이 길어져 부동으로 인한 욕창이 발생할 수 있고[5,6], 수술실에서도 수술체위나 수술시간에 따라 욕창이 발생할 위험이 증가한다[7]. 중환자실에서 욕창이 흔한 두 번째 원인은 인공호흡기, 지속적신대체요법(Continuous Renal Replacement Therapy, CRRT), 저체온요법과 같은 특수 치료와 관련이 있는데 이 중 인공호흡기의 적용 여부와 기간이 욕창의 발생의 위험요인으로 보고된 바 있다[8]. 세 번째 원인은 약물의 사용으로 중환자에게 흔히 투약하는 진정제, 진통제, 혈압 상승제가 욕창 발생의 위험을 높일 수 있다[9]. 마지막으로 중환자실 환자의 불량한 영양상태 역시 욕창발생에 영향을 줄 수 있는데 선행연구들[9,10]에 의하면 영양 상태를 나타내는 지표인 체중과 혈청 알부민이 욕창 발생의 유의한 위험요인이었다고 한다. |
