Purpose: This study aims to establish a theoretical framework for the fall prevention behavior improvement program by verifying the associations between fracture risk, fall risk perception, and fall prevention behaviors in women with osteoporosis. Methods: A survey was conducted in 122 women who were diagnosed as osteoporosis by having T-score of the femur bone density below -2.5 standard deviation from the bone density examination performed in 2 orthopedic clinics located in B metropolitan city between July 2014 and September 2014. The risk of fracture, level of fall risk perception, and fall prevention behaviors were measured. Results: Fall risk perception had a complete mediating effect on the relationship between the fracture risk and fall prevention behaviors in women with osteoporosis. Conclusion: The perception of fall risk is important to enhance fall prevention behaviors in women with osteoporosis, and the development of various education programs to improve awareness of fall risk is needed.

데이터처리

• 대상자의 골절위험도, 낙상위험지각 및 낙상예방행위와의 관계는 Pearson's correlation coefficients로 분석하였다.
• 대상자의 골절위험도, 낙상위험지각, 낙상예방행위 실천 정도를 파악하기 위하여 기술통계분석을 실시하였다.
• 대상자의 골절위험도와 낙상예방행위와의 관계에 미치는 낙상위험지각의 매개효과를 검증하기 위하여 Baron과 Kenny (1986)가 제시한 절차에 따라 3단계로 나누어 회귀분석을 실시하였으며 Sobel test를 사용하여 매개효과의 유의성 검증을 실시하였다. 매개효과검증을 위해 회귀분석의 첫 번째 단계에서 골절위험도가 낙상위험 지각에 유의한 영향을 미쳤는지, 두 번째 단계에서 골절위험도가 낙상예방행위에 유의한 영향을 미쳤는지, 세 번째 단계에서 낙상위험 지각에 유의한 영향을 미치고 낙상예방행위에 대한 골절 위험도의 영향이 두 번째 회귀분석에서보다 반드시 감소하는지를 확인하였다.
• 대상자의 일반적 특성을 파악하기 위해 실수와 백분율, 평균, 표준편차를 구하였다.

이론/모형

골절위험도를 산출하기 위해 WHO가 개발한 골절위험사정도구(Fracture Risk Assessment Tool, FRAX^®)를 이용하였다. 이 도구는 대상자의 연령, 성별, 키, 체중, 이전 골절력, 부모의 대퇴골 골절력, 흡연, 스테로이드제제 사용여부, 류마티스 관절염, 기저질환으로 인한 골다공증, 하루 3단위 이상의 음주여부, 대퇴골 경부 골밀도까지 총 12개 지표의 임상위험요인을 입력하여 10년 내 골다공증성 골절과 대퇴골 골절위험도를 계산한다.
낙상위험지각정도를 측정하기 위해 Hong (2011)이 개발한 재가노인의 낙상위험지각도구를 사용하였다. 이 도구는 신체적 요인, 감각적 요인, 인지 및 심리적 요인, 약물 및 일상생활 요인, 바닥과 관련한 환경적 요인, 일상생활도구와 관련한 환경적 요인 등 총 6개요인의 24문항 4점 척도이고 ‘전혀 그렇지 않다’ 0점에서부터 ‘매우 그렇다’ 3점으로 점수가 높을수록 낙상위험이 높다고 지각하는 것을 의미한다.
본 연구에서는 Gu 등(2002)이 개발한 낙상예방행위 도구를 Hyeon, Park, Park과 Kim (2010)이 수정한 도구를 사용하여 측정하였다. 이 도구는 14문항으로 구성되어 있으며 각 문항은 ‘예’ 1점, ‘아니오’ 0점으로 최저 0점부터 최고 14점으로 점수가 높을수록 낙상예방행위의 실천도가 높은 것을 의미한다.

성능/효과

본 연구는 골다공증 여성의 골절위험도와 낙상예방행위간의 관계에 미치는 낙상위험지각의 매개효과를 검증하여 낙상 예방행위 증진 프로그램의 이론적 기틀의 기초자료를 제공하기 위해 시도되었다. 대상자의 골절위험도와 낙상위험지각 및 낙상예방행위와의 상관관계를 분석한 결과, 모두 유의한 상관관계가 있는 것으로 나타났으며, 골다공증 여성의 골절위험도와 낙상예방행위 간의 관계에서 낙상위험지각은 완전 매개효과가 있는 것으로 나타났다. 이상의 연구결과를 토대로 다음과 같이 제언하고자 한다.

후속연구

대상자의 10년 내 골절위험도와 낙상위험지각 및 낙상예방행위와의 상관관계를 분석한 결과, 골절위험도와 낙상위험지각, 낙상예방행위 간에 통계적으로 유의한 양의 상관관계가 있는 것으로 나타났다. 골절위험도와 낙상위험지각, 골절위험도와 낙상예방행위 그리고 낙상위험지각과 낙상예방행위 간의 관련성이 크지 않는 것으로 나타나 추후 반복연구가 필요할 것으로 사료된다.
첫째, 골다공증 여성의 낙상위험에 대한 지각에 영향을 미치는 요인을 파악하는 연구가 필요하다.
