소아의 두부손상의 임상양상과 병변 상태를 평가하는 방법과 그 한계는? | 소아의 두부손상은 시간이 경과함에 따라 임상양상과 병변이 변화할 수 있어 이에 따른 적절한 평가와 처치가 지연될 경우 비가역적인 신경학적 장애가 남을 수 있으며 사망에 이를 수도 있다. 따라서 임상적으로 경과가 악화될 경우에는반복적인 두부 CT 검사를 통해 병변의 악화 및 신경외과적인 중재술 등의 필요성 여부를 판단할 수 있으나 소아는 성인과 달리 신경학적인 진찰과 임상양상의 관찰을 통한 상태평가 및 경과의 악화여부 등을 판단하기에 제한점이 있다.(10-12) 이러한 어려움으로 인해 실제로 소아 두부외상환자들을 대상으로 반복적인 두부 CT 검사가 흔하게 시행되고 있지만 현재 반복적인 두부 CT 검사의 정해진 가이드라인은 없다. |
