문제 정의
본 연구는 교통사고 환자 임상연구에서 활용할 수 있는 평가척도들에 대한 기초 연구로 한방의료기관을 내원한 교통사고 환자의 국내 임상연구에서 활용된 척도들을 검색하고 분석한 결과 다음과 같은 결과를 얻었다.

제안 방법
1. 교통사고 환자의 상태평가 시 환자가 호소하는 통증정도, 기능 이상, 심리 상태, 영상 검사, 한방 진단생기능 검사, 한의학적 변증 등 다각적인 접근을 제안한다.

대상 데이터
2016년 3월 31일까지 국내에서 발간된 논문을 대상으로 한국전통지식포탈(OrientalMedicineAdvanceSearching Integrated System), 과학기술정보통합서비스(NDSL), 학술연구정보서비스(RISS), 국회전자도서관(National Assembly Library), 한국의학논문데이터베이스(KMbase), 한국학술정보원(KISS) 등 6개 데이터베이스에서 ‘교통사고’, ‘자동차사고’를 포함한 논문들을 검색하였다.
31건의 임상 논문에서 사용된 평가도구는 20개였다. 이중 시각적 상사척도(Visual Analogue Scale, VAS)가 25건으로 가장 많이 사용되었고, 목 장애 지수(Neck Disability Index, NDI) 16건, 통증 장애 지수(Pain Disability Index, PDI) 4건, 경추부 단순 방사선 촬영 검사 결과를 바탕으로 측정한 Cobb’s angle 4건, McGill 통증 설문지(McGill pain questionnaire, SF-MPQ), 관절가동범위(Range of Motion, ROM), 통증 역치(Painthreshold) 3건, 통증평가척도(Pain Rating Score, PRS), 언어숫자 통증등급(Verbal Numerical Rating Scale, VNRS), 경추부 단순 방사선 촬영 검사 결과를 바탕으로 측정한 Jackson’s angle, 적외선체열검사(Digital Infrared Thermal Image, DITI) 2건 순으로 사용되었다.
검색된 논문 146편 중 중복 논문 4편, 임신 여성이나 아동을 대상으로 한 6편을 제외하였다. 이후 전문을 확인하여 한방의료기관에 내원한 환자를 대상으로 하지 않은 경우 4편, 교통사고 환자가 대상이 아닌 경우 4편, 출혈 등의 외상, 마비를 동반하는 등 특수한 질환을 대상으로 한 16편, 주제와 맞지 않은 연구 4편, 교통사고 환자에 대해 통계적 분석만 시행한 경우 23편, 기존 문헌을 대상으로 한 연구 7편을 제외하였다.
교통사고 환자에 대한 평가도구가 언급된 78편의 임상논문 중 연구대상이 되는 주소증이 편타성 손상이나 경추부 염좌 등으로 인한 경항통인 논문 31건, 요통인 논문 15건, 두통인 논문 5건, 두통 외 불면, 불안 등 신경정신과적 증상 또는 외상성 스트레스 장애(Posttraumatic Stress Disorder, PTSD)인 논문 14건, 특정 부위에 국한되지 않은 전반적인 근골격계 통증 또는 특정 증상 언급 없이 교통사고 환자를 대상으로 한 논문 14건으로 분류되었다. 각 증상에 따라 사용한 평가도구, 평가시점을 살펴보면 다음과 같다.

특정 증상에 대해 제한을 두지 않고 교통사고 후 발생한 증상을 대상으로 한 논문 14편에서 사용한 평가도구는 23개였다. VAS가 8건으로 가장 많이 사용되었으며 SFMPQ, NDI, ODI 이상 4편, PDI가 3편, 피부저항변이도(Skin Resistance Variability)가 2편에서 사용되었다.
이론/모형
VAS, Beck 우울척도(Beck Depression Inventory, BDI), 한국어판 개정판 사건충격척도(Impact or Event Scale-Revised Korean Version, IES-R-K) 각각 4건, MMPI 3건, Beck 공포척도(Beck Anxiety Inventory, BAI), 상태-특성 불안척도(Spielberger’s state-Trait Anxiety Inventory, STAI), 간이정신진단검사(Symptom CheckList-90-Revised, SCL-90-R), SMH sleep questionnaire 2건, 기질 및 성격 검사(Temperament and Character Inventory-Revised-Short Version, TCI-R-SV), HRV, QSCCⅡ 한국인 수면척도, 통증 완화 기간 등이 1건씩 사용되었다.
교통사고 환자를 대상으로 사용한 평가도구를 김 등10)의 방법을 참고하여 분류하였다. 기구를 사용하여 측정한 경우, 설문지 등 환자나 평가자가 판단하여 평가한 경우로 분류되었으며, 후자의 경우 평가 영역에 따라 통증, 기능장애, 심리상태, 한의학적 변증, 기타 영역으로 평가도구를 분류할 수 있었다.
교통사고 후 호소하는 증상 중 두통을 주 대상으로 삼은 연구는 4건이었다. 두통 정도를 평가하기 위해 VAS, 두통지표, 한국어판 두통 영향 검사(Korean Headache Impact Test-6, Korean HIT-6) 등이 사용되었으며, 사고 후 발생하는 두통이 환자의 심리에 미치는 영향을 확인하기 위해 미네소타 다면적 인성 검사(Minnesota Multiphasic Personality Inventory, MMPI)를, 두통과 목 부분과의 연관성을 확인하기 위해 NDI를 사용하였다.

성능/효과
9편의 논문에서 치료 전과 치료 후 2회 평가를 시행하였으며 두 번째 평가까지 경과 시간 및 치료 횟수는 다양하였다. 1건에서 퇴원 후 2개월 후 평가가 시행되었다.

평가시점은 치료 전과 최종 치료 후 2회 평가한 경우가 18건으로 가장 많았다. 두 번째 평가가 이루어질 때까지 경과 기간 및 치료 횟수는 2일간 1회부터 4주간 8회까지 다양했으며 경과 기간 및 치료 횟수에 대한 명확한 언급 없이 퇴원하기 전 시행한 경우도 있었다.
후속연구
2. 교통사고로 인한 환자의 충격, 불안 정도를 평가할 때, 사고 발생일로부터 1개월 이내 내원한 경우 BAI, 1개월 이상 경과하여 내원한 경우 BDI, STAI-Ⅱ, MMPI 등, 치료 경과에 따른 심리 상태 변화를 평가하는 경우 IES-R-K, STAI-I 등 상황에 따라 적합한 평가도구를 활용할 것을 제안한다.
3. 교통사고 후 구조적 손상 없이 증상을 호소하는 경우 한방 진단생기능 검사 및 한의학적 변증 평가도구가 유용하며 이에 대한 추가적인 연구 및 임상자료의 축적이 필요하다.
그러나 동일한 평가시점에 시행한 환자 상태 평가 자료 축적은 교통사고환자의 한의학적 치료를 발전하는 데 필요하다. 교통사고 환자에 대한 한의학적 치료효과에 대해 의미 있는 결과가 발생하는 시점을 분명히 할 수 있고, 다른 치료방법과 효과 비교를 통해 경쟁력을 확보하고, 환자의 현재 상태와 예후에 대해 환자와 원활하게 정보와 이해를 공유할 수 있으며 이를 바탕으로 한의학적 치료에 대한 자동차 보험 적용 근거가 더욱 공고해질 것이다. 향후 교통사고 환자 표준 진료지침을 개발할 때 고려되어야 할 것이다.
