Purpose: The purpose of this study was to investigate factors influencing health workers' infection management behavior in long-term care facilities. Methods: A descriptive cross-sectional survey was conducted with 180 health workers who are employed in long-term care facilities. The data were collected from April, 25 until July, in 2016. Results: Infection management behavior positively correlated with the perceived importance of infection management (r=.77, p<.001), but role conflict negatively correlated with infection management behavior (r=.28, p<.001). The hierarchical regression model with general characteristics (first step) and perceived importance of infection management, work environment, and role conflict (second step) against infection management behavior was statistically significant (F=31.93, p<.001). This model could explain 62.8% of infection management behavior ($R^2=.62$, ${\Delta}R^2=.39$). Particularly, perceived importance of infection management was identified as factors influencing infection management behavior(${\beta}=.70$, p<.001). Conclusion: The findings of this study imply that systemic education about infection control and monitoring should be considered, so as to encourage proper infection management behaviors among health workers in long-term care facilities.

본 연구의 설문대상은 대전에 위치한 장기요양시설에 근무하고 있는 요양보호사를 대상으로 하였다. 본 연구의 대상자 수는 G * Power 3.

이론/모형

감염관리 수행과 중요성 인지도는 Ryu와 Ryu(2010)가 APIC(Association for Professionals in Infection Control)와 CDC(Center for Disease Control and prevention)의 가이드라인 및 요양보호사 교육교재를 참조하여 개발된 질문지를 Ko(2014)가 수정한 도구를 사용하였다. 본 도구는 손 씻기와 장갑 착용(5문항), 호흡기감염관리(5문항), 인공 도뇨관 관리(5문항), 위장관 감염관리 및 구강청결관리(4문항), 약물관리(2문항), 환자의 배설물관리(4문항), 상처감염관리(3문항), 환자 방 청소(3문항) 및 세탁물 관리(2문항)의 총 33문항으로 구성되었다.
대상자의 역할갈등은 Rizzo, House와 Lirtzman(1970)이 개발한 역할갈등과 모호성 도구(Role conflict and ambiguity scale)를 Shin(2013)이 요양보호사를 대상으로 수정 보완한 도구를 이용하여 측정한다. 본 도구는 개인적 내적 기준과 역할사이에서의 갈등 4문항, 역할수행자의 시간이나 자원 또는 능력과 관련된 역할수행 상황에서의 갈등 5문항, 둘 이상의 역할을 수행하게 될 때 생기는 갈등 2문항, 불일치한 정책으로 인한 갈등 4문항, 개인의 행동에 대한 반응이나 결과의 모순성에서 생기는 모호성 3문항, 행동조건의 명료성에서 오는 모호성 9문항으로 구성되어 있다.
업무환경의 측정도구는 Hackman과 Oldham(1980)의 Job Diagnostic Survey척도를 참고로 하여 Kwak(2013)이 수정 보완한 것을 이용하였다. 본 도구는 업무정체성 2문항과 업무 자율성 3문항 그리고 업무 피드백 2문항의 총 7문항으로 구성되어 있으며, 각 문항은 ‘전혀 그렇지 않다’ 1점에서 ‘매우 그렇다’ 5점의 Likert scale의 5점 척도로 구성되었다.

성능/효과

본 연구는 노인요양시설에 근무하는 요양보호사의 감염관리 수행도에 영향을 미치는 요인을 파악하고자 시도한 연구로서, 요양보호사의 감염관리 수행도에 대한 일반적 특성, 감염관리의 중요성 인지도, 역할갈등 및 업무환경을 투입한 회귀모형은 감염관리 수행도를 약 60.9%의 설명하였으며, 감염관리의 중요성 인식도가 주요한 영향요인으로 나타났다. 이상에서 본 연구는 요양보호사의 감염관리 수행 증진에 관련된 실증적 자료를 제공함으로서, 실무적 의의를 갖는다.

후속연구

넷째, 따라서 추후연구에서는 감염관리 수행에 영향을 미칠 수 있는 좀 더 타당한 업무환경에 대한 개념적 탐색을 통해 환경과의 관련성에 대한 후속연구가 필요하다고 사료된다.
둘째, 감염관리의 중요성 인지도를 높일 수 있는 효과적인 중재방법이 도출 될 수 있는 후속연구가 제언된다. 특히, 교육적 중재 뿐 아니라 감염관리 수행에 대한 요양보호사의 책임감을 고취하고, 행정적인 지원을 제공하면서 행위에 대한관리감독을 포함하는 등의 다각적인 중재가 고려되어야 할 것이다.
첫째, 본 연구의 연구대상자가 일부지역에서 표집되었으며, 또한 기관의 특성이 상이한 집단이 포함되었으므로, 추후연구에서는 다양한 지역의 대표성 있는 대상자가 표집되어야 할 것이다. 또한 단변량 분석에서 노인요양기관의 특성에 따라 종속변수와 주 영향요인이었던 감염관리 중요성 인지도의 차이를 보였으므로, 이와 같은 기관차이를 통제한 상황에서의 반복연구를 통해 요양보호사의 감염관리 수행도에 영향을 미지는 변수를 추가로 연구할 필요가 있다고 제언한다.
