Purpose: This study aims to conduct an integrated literature review of infection control studies conducted by nursing care workers in long-term care facilities in Korea. Methods: Through the domestic search engines RISS and KISS, seven articles were selected by searching for theses and academic journals published in Korea from 2008 to January 2020. Results: In total, six research studies and one intervention study out of seven studies were analyzed. Measurement tools for examining the knowledge and performance of infection-related care workers consisted of skin infection, aspiration pneumonia, urinary tract infection, and hand hygiene. Factors affecting nursing care workers' infection management performance were infection management knowledge, education level, health status, and importance awareness. Conclusion: This study showed it is necessary to develop a tool that can accurately measure nursing care workers' infection management knowledge and performance. In addition, it is necessary to develop an intervention program for nursing care workers' infection control.

최종 분석된 7편의 문헌은 조사연구 6편, 중재연구 1편이었다. 본 연구의 목적에 적합하게 분석하기 위해 조사연구 6편은 저자(년), 대상기관 수, 연구대상자 수, 연구변수 및 연구도구 구성항목, 감염관리 수행도에 미치는 영향요인으로 구성하였으며, 중재연구 1편은 연구설계, 저자(년), 연구대상자수, 연구결과변수 및 측정시점, 측정도구, 연구결과로 구성하였다.

대상 데이터

본 연구에서는 노인장기요양보험제도가 도입된 이후 장기요양시설과 요양보호사가 증가했던 시점인 2008년 7월 1일부터 2020년 1월 31일까지 국내에 발표된 학위논문과 학술지에 게재된 논문을 바탕으로 분석하였다. 본 연구의 대상문헌의 구체적인 선정기준은 연구대상자가 요양보호사이고, 논문의 초록과 전문을 확보할 수 있으며, 감염관리를 측정한 도구가 제시되고 감염관리와 관련된 조사연구와 중재연구로 하였다.
그 중 연구기간을 장기요양보험제도가 시행된 2008년 7월 1일부터 2020년 1월 31로 제한하여 1편의 논문이 배제되어 총 12편이 검색되었다. 최종 분석문헌을 선정하기 위해 연구자 2인은 논문 초록과 본문 내용을 검토하였으며, 이를 통해 본 연구의 목적에 적합하지 않은 대상자 2편, 학위논문이 국내학술지 논문으로 출판된 2편, 논문의 초록과 본문을 확인할 수 없는 논문 1편을 제외하여 최종 7편이 선정되었다(Table 1).

이론/모형

본 연구는 현재 장기요양시설에 근무하는 요양보호사와 감염관리와 관련된 논문을 분석하기 위해 Whittemore와 Knafl (2005)가 제시한 통합적 문헌고찰 방법을 사용하였다. 이 통합적 문헌고찰방법은 문제규정, 문헌검색.
본 연구에서는 최종 선정된 7편의 문헌 질적수준을 평가하기 위해 Arbesman, Scheer와 Lieberman (2008)이 개발한 근거기반 연구수준 5단계를 바탕으로 분석한 결과, 개별실험연구 및 설문조사연구인 근거수준 IV 6편, 단일집단 비무작위 연구인 근거수준 III 1편이었다(Table 2).

성능/효과

요양보호사의 감염 관련 측정도구는 피부감염, 흡인성 폐렴, 요로감염, 손 위생, 전반적인 감염관리로 나타났다. 감염 관련 측정도구는 지식, 인지도, 태도, 수행도, 인식 중요성을 확인하기 위한 도구였으며 측정도구는 WHO, CDC, APIC, 요양보호사 양성 표준교재를 바탕으로 개발되었다.

본 연구에서 선정된 7편 문헌의 연구방법(연구설계, 측정도구, 연구결과 등)을 분석하기 위하여 연구자 2인이 논의하여 추출할 자료 목록을 선정하였고 분석틀을 구성하였다. 최종 분석된 7편의 문헌은 조사연구 6편, 중재연구 1편이었다. 본 연구의 목적에 적합하게 분석하기 위해 조사연구 6편은 저자(년), 대상기관 수, 연구대상자 수, 연구변수 및 연구도구 구성항목, 감염관리 수행도에 미치는 영향요인으로 구성하였으며, 중재연구 1편은 연구설계, 저자(년), 연구대상자수, 연구결과변수 및 측정시점, 측정도구, 연구결과로 구성하였다.

후속연구

첫째 요양보호사의 역할 및 직무분석을 통해 요양보호사와 관련된 감염관리 범위를 파악하는 것이 필요하다. 둘째 요양보호사의 감염관리 정도를 파악하기 위한 측정도구개발이 필요하다. 셋째 요양보호사의 교육요구도를 파악한 후 감염관리 교육 프로그램을 개발하여 감염예방활동을 증진시키는 것이 필요하다.

	장기요양시설에 입소한 대상자는 대부분 어떠한 상태로 입소하는가?	본 연구결과 국내에서 발표된 요양보호사의 감염관리에 대한 연구 논문은 노인장기요양보험제도가 시행된 2008년 이후 2014년에 처음 발표되었으며, 2015년 중동호흡기증후군 유행 이후 감염관리가 강화되면서 매년 발표되고 있으나, 연구 활동은 미비한 것으로 나타났다. 장기요양시설에 입소한 대상자는 대부분 기저질환을 가지고 있으면서 고령의 노인대상자로 감염에 취약한 상태이다(Hong, 2015). 중동호흡기증후군 유행 시 감염된 환자 중 40%는 환자 가족이나 간병인으로 이들은 병원감염을 전파시키는 중요한 경로이면서 환자에게 감염을 전파시키기도 한다(Kim, 2015).
