Objectives The purpose of this study is to investigate about chief complaints of Korean pediatrics and adolescent outpatients in the ${\bigcirc}{\bigcirc}$ university hospital for their recent trend. Methods The study was composed of 4,677 new patients aged between 1 month and 20 years who had been visited pediatrics in ${\bigcirc}{\bigcirc}$ university hospital from 2012 January to 2015 December. Results 1. Among those patients, majority was early childhood as 51.4%, followed by early childhood, late childhood, infancy, and adolescence. 2. Chief complaints in this population have been recorded in the fall the most as 29.6%, but the distribution was inconsistent. 3. To categorize, digestive-related chief complaints were the highest as 30.6%, followed by respiratory, supplemental use, growth, psychiatrics. Respiratory-related chief complaints increased dramatically in the fall. 4. In infancy and early childhood, digestive/respiratory complaints were common, whereas, growth complaints were common in late childhood and adolescence. 5. Digestive/respiratory complaints have been over 50% out of all chief complaints annually. Psychiatric and growth-related complaints have been trending up, whereas, supplemental/skin complaints have decreased every year. 6. Specifically, anorexia were the most common in digestive complaints, and rhinitis/sinusitis were top among respiratory complaints. Precocious puberty and developmental disability increased every year. Conclusions The most common chief complaints were digestive and respiratory-related, and precocious puberty and growth issues have been trending up lately. Based on this result, more studies that targets precocious puberty and growth problems are needed.

환자의 초진 설문지를 근거로 하여, 내원 시 성별,연령, 방문일, 주소증을 기록하였고 이를 바탕으로 계절별 분포, 연령별 분포, 주소증 계통별 분포를 하였다. 연령별 분포는 만 1개월~1세까지 영아기, 만 2~5세까지 유아기, 만 6~10세까지 학령기, 만11~20세까지 청소년기로 구분하였다1).
대상 데이터
2012년 1월 1일부터 2015년 12월 31일까지 4년 동안 ○○대학교병원 한방소아·청소년과에 내원한 만 0세부터 20세 이하 초진 환자 5,460명 중, 임상 연구 피험자 및 프로그램 체험자 783명을 제외하고, 총 4,677명을 대상으로 하였다.

데이터처리
SPSS 19 for window program을 이용하여 자료를 분석하였다. 연령 및 성별 분포의 통계처리는 빈도 분석을 사용하였고, 연도별 성별 분포, 연도별 연령군 분포, 연도별 계절 분포, 성별 계통 분포, 계절별 계통 분포, 연령군별 계통 분포, 연도별 계통 분포, 연도별 주소증분포의 통계처리는 Chi-square를 사용하였으며, p-value0.05 미만인 경우에 통계적으로 유의성 있다고 간주하였다.
성능/효과
1. 연령군 총 분포는 유아기가 51.4%로 가장 많았으며, 연도별 연령군 분포는 4년간 연속해서 유아기, 학령기, 영아기, 청소년기 순서였다.
2. 계절별 총 분포는 가을이 29.6%로 가장 많았으나, 연도별 경향은 일정하지 않았다.
3. 계통별 총 분포는 소화기 계통이 30.6%로 가장 많았으며, 호흡기 계통, 보약 계통, 성장 계통, 신경·정신 계통 등의 순서로 나타났다.
4. 연령군별 계통 분포는 영아기, 유아기에서는 소화기 계통과 호흡기 계통이, 학령기, 청소년기에서는 성장 계통이 편중되어 나타났다.
5. 연도별 계통 분포는 소화기 계통, 호흡기 계통은 매년 50% 이상을 차지하였다. 성장 계통, 신경·정신 계통은 매년 증가하였고, 보약 계통, 피부계통은 매년 감소하였다.
6. 연도별 주소증 분포는 식욕부진이 가장 많았고, 비염·부비동염, 보약 순서로 나타났다. 성조숙증과 발달장애의 비율은 매년 증가하였다.
계절별 계통 분포는 보약 계통, 비뇨생식기 계통은 봄에, 신경·정신 계통, 기타 계통은 여름에, 소화기 계통, 호흡기 계통, 성장 계통, 비뇨생식기 계통, 근골격 계통은 가을에, 피부 계통은 겨울에 내원 비율이 가장 높았다.
발달장애 비율은10.0%, 15.4%, 55.0%, 51.5%로 증가하는 경향이고 통계적으로 유의하였다 (p<0.01).
보약 비율은 77.1%, 79.2%,86.6%, 86.6%로 증가하는 경향이고 통계적으로 유의하였다 (p<0.01).
본 연구에서 연령군별 계통 분포는 영아기에서 소화기 계통 31.6%, 호흡기 계통 27.3%, 신경·정신 계통22.0%, 보약 계통 14.0%, 피부 계통 3.2% 등의 순서로 나타났다.
비염·부비동염 비율은 43.1%, 51.5%, 56.7%, 55.5%로 증가하는 경향이고, 감기 비율은 11.7%, 6.6%, 7.0%, 6.1%로 감소하는 경향이다.
성별 계통 분포는 남아는 호흡기 계통, 신경·정신 계통, 기타 계통, 비뇨생식기 계통에서 여아보다 비율이 높았고, 여아는 소화기 계통, 보약 계통, 성장 계통, 피부 계통, 근골격 계통 남아보다 비율이 높았다.
신경·정신 계통 또한 매년마다 4.3%, 6.9%, 14.3%,9.2%로 점차 증가하는 비율을 보였다.
2012년은 개원한지 6년 후로 전반적인 한방소아·청소년과의 위치 및 진료에 대한 홍보가 되었으므로, 최근까지 주소증 변화 양상을 알아보기에 충분한 기간이라고 생각된다. 연구 결과, 이전의 연구들3,5,7,11)과 마찬가지로 본 연구에서도 호흡기 계통, 소화기 계통은 여전히 반수 이상을 차지하였는데, 이는 의학의 발전에도 불구하고 한방치료가 호흡기, 소화기 치료에 효과 있다고 인식하는 것은 여전하다는 것을 알 수 있다. 계통의 분포 중 성장 계통 비율이 점차 높아지는 것은 통신 매체를 통한 미적, 외적관심 증가 및 사춘기 시작 시기가 빨라지는 것도 요인 중 하나가 될 수 있으므로, 현재 시대 변화를 반영한 결과라 할 수 있다.
이러한 결과를 비추어 보았을 때, 한방소아·청소년과에 방문하는 보호자들의 생각이 한방치료를 단순히 보약위주의 치료로 인식하던 예전과는 달리 근본적인 치료적 관점으로 인식하고 있다는 것을 볼 수 있다.
최근 맞벌이부부가 증가하면서 어린이집이나 유치원 등 조기 집단생활의 시작으로 호흡기 감염 등 외부 환경에 노출되는 시간이 늘어남에 따라 유아기 환자의 비율이 높아지는 것으로 생각된다. 청소년기 환자의 비율은 14.8%, 11.7%, 9.5%, 7.5%으로 매년 감소하였다. 청소년기에는 학교 활동시간 및 방과 후 교육활동으로 진료시간 이용의 어려움과 다른 연령군에 비해 면역력이 성숙된 상태이므로 질환 치료를 위한 병원 방문이 상대적으로 감소할 것이다.
초진 환자의 계통 분포를 살펴보면, 소화기 계통이30.6%로 가장 많았으며, 호흡기 계통 26.4%, 보약 계통 15.6%, 성장 계통 11.3%, 신경·정신 계통 9.3% 등의 순서로 나타났다.
후속연구
결론적으로 최근 한방소아·청소년과의 주소증 경향을 파악하여 소아·청소년 질환의 변화양상을 알았고, 성조숙증, 발달장애 등 구체적 질환에 대한 연구 필요성을 제시한다.
현재 발달장애와 관련된 연구들은 발달장애의 주된 문제에 대해 침, 한약 등 일차적인 치료 위주의 연구가 대부분이므로23,28-30) 발달장애를 관리하기 위한 체계적인진료 자료를 구축할 필요가 있다. 그리고 발달장애를 언어장애, 인지장애, 운동장애 등으로 세분화하여 진단 및 치료, 관리에 대한 구체적인 지침 등을 제시하는 것이 필요하다.
따라서 향후 더 정확한 분석을 위해서는 특정 병원이 아닌 지역별로 소아환자들을 대상으로 하는 연구가 시행되어야 하며, 주소증 뿐만 아니라 최종 진단명에 대한 계획적이고 전향적인 연구가 필요할 것이다.
본 연구의 제한점은 전체 소아 환자를 대상으로 한 것이 아니라, 특정 병원에 내원한 환자를 대상으로 하였기 때문에 일반 소아 환자 집단에 대한 대표성이 부족하고, 병원 특성상 전문 클리닉 운영으로 Tic, 수면장애 등은 한방신경·정신과로, 여드름, 아토피 등은 한방안이비인후·피부과로, 안면마비는 침구과에서 진료를 보는 상황으로 추가 초진이 타과로 유입되어 모든 소아 환자에 대한 분석이 어려웠다는 점이다.
