Objectives: The purpose of this study is to analyze factors associated with depression among wage earners by subjective health and stress perception. Methods: The study subjects include 1,787 wage earners(aged 19 and older) who are currently working, using data from the 2014 National Health and Nutrition Survey. Data were analyzed using frequency test, t-test, ANOVA and multiple regression. Results: Multiple regression analysis showed that perceived stress level, subjective health status, age, monthly household income, sex, marital status, average daily sleeping time, and whether type of employment was regular or non-regular were significantly related to depression. Conclusions: It is necessary to develop an appropriate mental health promotion program and to manage prevention of depression at the national level.

대상 데이터

본 연구는 질병관리본부에서 수행한 국민건강영양조사 제6기 2차 년도(2014년) 원시자료를 활용하였다. ‘국민건강영양조사’는 국민의 건강수준을 파악하고 국가의 건강정책을 수립⋅평가하는데 필요한 기초통계로써, 매년 192개 지역의 20가구를 확률표본으로 추출하여 만 1세 이상 가구원 약 1만 명을 대상으로 조사하고 있다(KNHANES, 2014).
국민건강영양조사 제6기 2차 년도(2014년)의 참여자수는 7,550명이고, 이 중 만 19세 이상 성인은 5,976명이었다. 성인 중 취업형태 문항에 임금근로자로 응답한 2,310명을 선택하였고, 주관적 건강상태,수면시간, 스트레스인지여부, 근로시간 및 형태 등의 주요변수에서 무응답 또는 결측치를 제외하고, 최종 1878명을 대상자로 선정하였다(Figure 1).

데이터처리

국민건강영양조사 자료는 층화 이단계 집락추출법을 사용한 표본조사이므로, 복합표본설계인 가중치,층, 집락을 적용하였다. 대상자의 일반적 특성, 하루평균 수면시간, 주관적 건강상태, 스트레스 인지정도, 근로시간 및 형태를 알아보기 위하여 기술통계를 이용하여 빈도와 백분율을 구하였다.
대상자의 일반적 특성, 주관적 건강인지 및 근로조건에 따른 우울의 차이를 분석하기 위하여 t-test, ANOVA 그리고 사후검정은 Scheffe’s test로 분석하였다. 대상자의 우울에 영향을 미치는 요인을 알아보기 위하여 진입(enter)방식 다중회귀분석(multiple regression analysis)을 실시하였다. 자료의 분석은 SPSS(Version 21.

이론/모형

우울은 Han et al.(2008)이 타당화한 한국버전의 우울증선별도구(Patient Questionnaire-9: PHQ-9)를 사용하여 측정하였는데, PHQ-9은 우울증을 간단하게 선별하고, 심각도를 평가하기 위한 목적으로 만들어진 자기보고식 검사이다. ‘지난 2주 동안, 아래 나열되는 증상들에 얼마나 자주 시달렸습니까?’라는 질문에 ‘전혀 아니다(0점)’, ‘여러 날 동안(1점)’, ‘일주일이상(2점)’, ‘거의 매일(3점)’의 4점 척도로 평가되었으며, 점수의 범위는 0-27점으로 구성되어 있다.

성능/효과

본 연구는PHQ-9로 우울을 측정한 국민건강영양조사(2014)를 이용하여 임금근로자의 우울에 영향을 미치는 요인을 파악하였다. 그 결과 스트레스 인지정도, 주관적 건강상태, 연령, 가구소득, 성별, 결혼상태, 수면시간, 정규직 여부 순으로 나타났다. 이는 국민건강영양조사(2009)로부터 근로자를 대상으로 분석한 Lee etal.

후속연구

그 결과 임금근로자의 우울에 영향을 미치는 예측요인은 스트레스 인지정도, 주관적 건강 상태, 연령, 가구소득, 성별, 결혼상태, 수면시간, 정규직 여부 순으로 나타났다. 이러한 연구기초자료를 근거로 하여, 우울 예방과 정신건강 증진을 위한 인식개선과 국가적인 체계적 관리가 요구된다.
이상의 결과를 토대로 임금근로자의 주관적 건강인지와 근무조건의 개선을 통해 우울을 예방하고 조기에 치료하는 것이 필요하며, 특히 배우자가 없는 비정규직 여성 근로자의 직무스트레스와 우울관리를 위한 종합적인 방안이 필요하다고 하겠다.

	주요우울장애 근로자가 기업과 사회에 심각한 피해를 줄 수 있는 이유는 무엇인가?	9% 이상이 최근 1년 동안 연속적으로 2주 이상 일상생활에 지장이 있을 정도의 슬픔이나 절망감 등을 경험한 것으로 나타났다(KNHANES, 2012). 근로자들의 우울은 자살생각을 3.05배 높이는 가장 높은 위험인자로 분석되었으며(Lee & Jung, 2014), 개인적인 건강뿐만 아니라 능률저하, 자존감 상실 등 복합적인 원인 때문에 기업과 사회에 심각한 피해를 줄 수 있다(Lee et al., 2013).
