본 연구의 목적은 관절염 노인을 대상으로 스트레스, 건강생활동기, 도구적 일상생활 수행능력에 대한 연구를 통해 관절염 노인의 활동을 연구하기 위함이다. 자료수집은 65세 이상의 관절염 노인 117명을 대상으로 구조화된 설문지로 조사하였다. 수집된 자료는 SPSS WIN 12.0을 이용하여 independent t-test, Pearson Correlation Coefficients, Stepwise multiple regression으로 분석하였다. 연구대상자의 스트레스와 도구적 일상생활 수행능력간에 유의한 역상관관계, 건강생활동기와 도구적 일상생활 수행능력 간에 유의한 순상관관계가 있었다. 대상자의 도구적 일상생활 수행능력에 영향을 미치는 요인은 스트레스 하위영역 중 신체적 요인(${\beta}=-0.354$, p<.001), 건강생활동기 하위영역 중 자기효능감(${\beta}=0.250$, p<.001)이 도구적 일상생활 수행능력에 영향요인으로 나타났으며, 총 설명력은 18.5%였다. 본 연구를 통하여, 관절염 노인의 도구적 일상생활 수행능력에 영향을 미치는 다양한 요인을 파악하고 효율적인 건강생활을 증진할 수 있는 교육프로그램개발이 필요하다.

The aim of this study was to identify stress, motivation for a the healthy lifestyle and IADL of the elderly with arthritis. This study examined 117 elderly person over the age of 65 years living in S city. The data were analyzed using an independent t-test, Pearson correlation coefficients, and stepwise multiple regression with the SPSS Win 12.0 Program. Significant negative correlations were observed between stress and IADL, significant positive correlations between motivation on healthy lifestyle and IADL. The predictors on IADL were the physical area of stress (${\beta}=-0.354$, p<.001) and self-efficacy of motivation on healthy lifestyle (${\beta}=0.250$, p<.001). The model explained 18.5% of the variance. More study will be needed to explore a range of factors influencing the IADL and develop education programs for effective healthy lifestyle of elderly people with arthritis.

데이터처리

0을 이용하여 분석하였다. 대상자의 일반적 특성은 빈도와 백분율을 가족기능, 공감능력, 공격성 정도는 평균과 표준편차를 구하였으며, 일반적 특성에 따른 제 변수 간 차이는 Independent t-test로 분석하였고, 제 변수간의 상관관계는 Pearson's Correlation Coefficient로, 도구적 일상생활 수행능력에 미친 효과는 Stepwise Multiple Regression으로 분석하였다.

이론/모형

건강행위 수행여부를 이끄는 개인의 의지로서[20]본 연구에서는 Song 등[20]이 노인의 건강생활 행동변화의 동기요소를 측정하기 위해 개발한 동기요소 측정도구의 점수를 의미한다. 이 도구는 건강생활을 수행하는 것에 대한 자기효능감, 지각된 유익성, 지각된 장애, 건강생활에 대한 호감도 4영역, 총 28문항으로 이루어졌다.
긍정적, 부정적 생활사전과 일상생활의 변화와 혼란으로[19], 본 연구에서는 Seo 등[19]이 개발한 SESS(Seo's Elderly Stress Scale)를 본 연구자가 대상자에 적합하도록 단어와 어구를 수정․보완한 도구로 측정한 점수를 의미한다. 이 도구는 가족관계 요인, 신체적 요인, 심리 · 사회 · 경제적 요인의 3영역, 총 35문항으로 구성되고, ‘전혀 없다’ 1점, ‘가끔 있다’ 2점, ‘자주 있다’ 3점, ‘항상 있다’ 4점이며, 점수가 높을수록 스트레스가 높은 것을 의미한다.
노인이 사회에서 기능할 때 독립적 생활을 영위하는데 필요한 좀 더 복잡한 기능을 말하며[18], 본 연구에서는 Lawton과 Brody가 개발한 IADL(Instrumental Activities of Daily Living)을 Son[18]이 수정․보완한 도구로 측정한 점수를 의미한다. 이 도구는 전화사용, 외출 및 여행, 식품점에 가서 장보기, 식사준비, 집안일(청소, 정리정돈), 빨래, 약물 복용, 금전 관리 등의 8문항으로, ‘혼자서 할 수 있다’는 3점, ‘약간의 도움이 필요하다’는 2점, ‘많은 도움이 필요하다’는 1점이며, 점수가 높을수록 도구적 일상생활 수행능력이 높은 것을 의미한다.

성능/효과

대상자의 건강생활동기 및 스트레스와 도구적 일상생활 수행능력의 상관관계를 분석한 결과, 도구적 일상생활 수행능력은 건강생활동기와 순상관관계(r=0.270, p<.01)를 나타내었고, 건강생활동기 하위영역 중 자기효능감(r=0.272, p <.01), 지각된 유익성(r=0.234, p <.05), 건강생활에 대한 호감도(r=0.254, p <.01)와 순상관관계를 나타내었다(Table 3).
또한 대상자의 도구적 일상생활 수행능력은 스트레스와 역상관관계(r=-0.288, p <.01)를 나타내었고, 스트레스 하위영역 중 신체적 요인(r=-0.205, p <.05), 심리 · 사회 · 경제적 요인(r=-0.370, p <.001)과 역상관관계를 나타내었다(Table 3).
이상의 결과에서 관절염 노인의 도구적 일상생활 수행능력에 신체적 스트레스와 건강생활동기 중 자기효능감이 영향을 미친 것으로 나타났고, 신체적 스트레스를 가장 높게 인식하며, 건강생활동기에서 자기효능감이 가장 높게 나타났다. 이러한 결과는 만성적인 염증과 통증으로 기능적 동작의 어려움을 호소하는 관절염 노인들에게 건강생활에 대한 자신감이 적극적인 일상생활과 사회적인 활동 참여에 중요한 요인임을 의미한다.

후속연구

본 연구는 지역적으로 편의 추출된 일개 시의 노인복지관 2곳에 대한 조사연구만을 시행하였고, 관절염을 종류나 부위를 분류하지 않았으므로 본 연구 결과를 일반화하는데 제한이 있다. 또한 본 연구의 변수들이 갖는 설명력은 18.5%로 높은 수준이 아니었으므로 관절염 노인 도구적 일상생활 수행능력을 설명하기에는 부족하였으므로 향후 종단적 연구를 통해 영향요인을 추가로 규명할 필요가 있다.

	도구적 일상생활 수행능력은 무엇인가?	도구적 일상생활 수행능력(Instrumental Activities of Daily Living : IADL)은 노인의 건강생활 척도로서 노인들의 기본적인 일상생활 활동 외에 복합적인 적응능력이나 스스로 생활을 유지해 나갈 수 있는 기능이므로[9], 관절염 노인의 IADL을 평가해 보는 것은 매우 중요하다. 최근까지 진행된 노인의 IADL에 관한 국외 연구로는 노인들의 건강상태와 IADL이 관련이 있다는 연구[10,11]가 있었고, 국내연구들은 도시지역과 농촌지역노인들의 ADL과 IADL을 비교[12,13], 시설노인과 재가노인의 ADL과 IADL을 비교[14,15,16] 및 우울과 IADL과의 연관성에 관한 연구[17]이었다.
