The chosen trials were analyzed in terms of their inclusion and exclusion of participants, intervention, comparators, outcome, and trial design. Then, we compared the results of the analysis according to the regulations and guidelines of the Ministry of Food and Drug Safety to suggest the issues that we will have to consider when developing the Guidelines of Clinical Trials with Herbal Medicinal Products for Gastroesophageal Reflux Disease (GERD).Results: As a result, few guidelines for GERD and clinical trials with herbal medicinal products were located in the national institution homepage. In addition, 8 articles were found using the following combination of search terms: “Gastroesophageal reflux disease”, “GERD”, “herbal medicine”, “herbal therapy”, “Korean Medicine”, “Traditional Chinese Medicine”, and “TCM”. Even though all trials had their own unique research questions, all studies were performed using a randomization method. Most trials included participants with reflux esophagitis, but two trials targeted proton pump inhibitor-refractory GERD. The type of intervention varied, such as decoction, granules, and capsules. Additionally, individualized herbal medicines were used in two studies. Comparators were diverse, such as placebo, Western medicine, and electro-acupuncture. The most frequently used outcome for efficacy was the effectiveness rate. In addition, the outcome for evaluating quality of life, esophageal mucosa and pressure, esophageal acid reflux, and recurrence rates were used. Safety was investigated by recording adverse events and carrying out laboratory tests.Conclusions: We identified some issues by reviewing the existing guidelines and comparing them with clinical trials for GERD and herbal medicinal products. These results will be utilized for developing the Guidelines of Clinical Trials with Herbal Medicinal Products for Gastroesophageal Reflux Disease (GERD).

제안 방법
두 번째로, 임상시험 논문들의 비교를 통해 도출된 내용을 기존 가이드라인에 비추어 고찰하였다. 임상시험은 모두 무작위 대조군 설계였다.
실제 임상시험계획서를 작성할 때 필수적인 요소인 P(Participants), I(Intervention), C(Comparator), O(Outcome)를 중심으로 분석하였으며, 그 외 임상 시험설계에 대해서도 비교하였다.
대상 데이터
개발팀원 중 2인이 독립적으로 식품의약품안전처 홈페이지(http://www.mfds.go.kr), 국가과학기술정보센터(NDSL) 홈페이지(www.ndsl.kr) 검색을 통해 한약제제 관련 가이드라인과 GERD관련 가이드라인을 검색하였다. 그 후 개발팀이 검색된 가이드라인을 분석하여 한약제제 임상시험에 적용 가능한지 고찰하였다.

성능/효과
본 연구는 위식도역류질환 한약제제 가이드라인 개발을 위한 사전 작업으로 진행하였으며, 기존 가이드라인 검색⋅분석과 기존 임상시험 검색⋅분석이라는 두 부분으로 크게 나눌 수 있다. 먼저 기존 가이드라인을 검색한 결과, ‘위식도역류질환 치료제의 임상시험 평가 가이드라인’, ‘생약한약제제 임상시험의 일반적 고려사항’, ‘천연물신약⋅한약제제 임상시험가이드라인 연구’, ‘한약제제의 임상시험 평가지표개발에 관한 연구’를 결과로 얻을 수 있었다.

후속연구

질환의 이환기간을 다룬 논문은 2개밖에 없었는데, 그 조건도 3개월, 6개월로 차이가 있었다. 이환기간의 범위는 양약을 대상으로 한 임상시험을 추가로 검색하여 조사하고, 전문가 합의를 거쳐 명확히 해야 할 필요가 있겠다.
한편, PPI에 반응을 하지 않는 불응성 GERD를 한약제제 임상시험의 대상으로 삼게 되면, 현재 표준치료로 인식되는 양약치료의 한계를 극복할 수 있는 좋은 시도가 되리라 생각된다. 추가로, 한약제제의 치료적 탐색⋅확증 시험 시 시험대상자 권고사항, 한약제제를 단독⋅병용요법으로 할 경우의 각각의 시험대상자 선정, 병용요법의 허용여부 및 허용 시 그 범위 등도 고려하여야 하겠다. 또한 GERD의 한의학적 진단기준과 한의학적 (병증)분류와 기준, 신뢰도와 타당도가 연구된 한의학적 진단도구 적용여부, 한의학적 중증도 평가기준 마련 등도 보강이 필요한 부분이다.
또 다른 가이드라인의15 ‘위약 대조군 시험은 해당 생약(한약)제제를 이용한 치료법 자체의 가치가 있는지 없는지의 여부를 알기 위해서라기보다는 그 치료법이 대조군으로 설정된 치료법보다 가치가 뛰어난지 아닌지의 여부를 판명하기 위해 수행한다’는 내용도 이를 지지한다. 추후 PPI 불응성 GERD를 대상으로 할 때는 대조군설정 시 참고해 볼 수 있겠다.
아쉽게도 기존 가이드라인에서 권고하는 바와 같이 WHO-ART 등의 국제적인 기준에 의한 분류 및 분석은 이루어지지 않았다. 추후 개발될 가이드라인에서는 안전성 평가에 대한 내용을 보강하여 시험약의 특징에 따라 다각적 방법으로 자료를 얻어 안전성을 평가하도록 해야 할 것이다. 또한 확인된 부작용은 치료 기간, 용량 또는 혈중 농도, 회복 시간, 연령 등과 관련하여 관찰 연구해야한다15.
분석된 논문에서는 변증유형을 결정하는 연구자의 교육정도 및 임상경력에 대한 언급이 없었다. 추후 변증을 적용할 경우 변증에 대한 교육 및 연구자 간의 진단일치도 제고가 이루어져야한다. 또한 변증에 맞춘 한약제제의 마련도 실무적인 부분에서 고려되어야 될 것이다.
항의 ‘위식도역류질환의 치료’ 부분은 일반적인 치료방법이 제시되어있고, 그 중 양약 표준치료(PPI)가 예시로 나와 있다. 추후 한약제제 가이드라인 개발 시에는, 문헌과 전문가 합의에 근거하여 GERD 치료에 사용될 수 있는 한약제제 치료를 추가하는 것이 필요하겠다.
