Objectives The purpose of this study is to investigate recent clinical studies on effect of acupuncture for strabismus in Korea and other countries. We have analyzed 10 years (from 2006 to 2015) of case studies and randomized controlled trials (RCT) on effect of acupuncture for strabismus. Methods The search database includes Oasis, KTKP (Korean Traditional Knowledge Portal), Medline, Embase, and Cochrane library. To narrow the search, the following key search terms were used: 'strabismus, acupuncture'. The search was limited to the publication date from 2006 to 2015. 7 case studies and 5 control studies were selected for analysis. Results and Conclusions 1. The acupuncture treatment is effective for paralytic strabismus 2. The studies used BL1, GB1, ST2, TE23, GB15, EX-HN5, ST1, and LI4 acupoints and also an electroacupuncture treatment, which is 15 minutes of extraocular muscle stimulation, was used. 3. Degree of strasbismus was tested by Ocular motor range test, Corneal light reflex test, Ocular abducent scale test (Scott and Kraft, Diploptic range test).

그러므로 융합기능이 완성되기 전에 일찍 사시를 발견해서 치료하면 시력도 증진되고 시기능 손상도 예방할 수 있으므로 초기의 적극적인 사시치료가 중요하다. 꾸준히 사시의 침치료에 대한 관심이 높으며 그에 따라 치료법도 다양해지고 있어 최근 10년간의 논문을 분석해보았다.
대조군이 되는 약물치료로는 주로 Vitamin B1, Vitamin B12, Methycobal, eye drug, oral ATP를 시행하였다. 치료기간은 평균 4주였다.
검색어에 ‘사시/strabismus/abducens palsy’을 입력하여 나온 결과 중에 ‘침/acupuncture’을 검색하였으며 그 중 2006년 이후부터 2015년까지 발표된 연구로 한정하였다. 이중 침치료를 이용하여 사시를 실제로 치료한 case study 및 control study를 대상으로 연구방법, 침치료혈위, 자극방법, 치료결과 등을 조사하고 분석하였다
효과를 판정하기 위한 scale은 대부분의 연구에서 안구운동검사, 각막반사검사, 안구외전장애 정도 (Scott and kraft)를 사용하였으며 일부 연구에서 복시발생거리나 사시 발생측 안구의 이동거리를 측정하였다.

대상 데이터

본 연구의 논문검색은 Oasis/KTKP (Korean Traditional Knowledge Portal) 및 Medline/Embase/Cochrane library를 이용하였다.
7편의 case study 논문은 국내 6편, 해외 1편이었으며 (Table 1)과 같다.
검색어에 ‘사시/strabismus/abducens palsy’을 입력하여 나온 결과 중에 ‘침/acupuncture’을 검색하였으며 그 중 2006년 이후부터 2015년까지 발표된 연구로 한정하였다.

효과를 판정하기 위한 scale은 안구외전장애 정도(Scott and kraft), 복시거리를 사용하였다.

성능/효과

1. 마비성 사시에 대한 침치료는 효과가 있다.
3. 사시평가의 scale은 안구운동검사, 각막반사검사, 안구외전장애 정도 (Scott and kraft), 복시발생거리가 사용되었다.
7편의 증례보고에서 모두 안구운동성의 호전이 있었다. 김⁷⁾은 마비성 사시 7세 남환에게 전침치료를 1년간 시행결과 각막반사검사상 초진시 좌안 30도, 우안 45도의 내사시각이 좌안 0도, 우안 7-15도로 호전되었다고 보고하였다.
사시의 침치료에 관한 논문은 모두 마비성 사시 환자를 대상으로 한 연구였다. case study 결과 모두 침치료를 통해서 증상의 호전이 있었으며 Vitamin, Methycobal, eye drug^12-6) 등의 양방 약물치료군을 대조군으로 둔RCT 연구에서 또한 침치료가 약물치료군보다 증상의 개선에 더 도움이 되었다. 마비성 사시의 경우 우선적으로 6개월에서 2년 동안 보존적 치료를 시행하고 경과를 지켜보는 것으로 치료방향이 제시되어 있기 때문에 이 시기에 한방치료가 적극적으로 활용될 수 있다.
사시는 육안적으로 드러나는 질환이기 때문에 사시가 계속 지속되면 자존감, 사회성 결여의 문제로 이어질 수 있기 때문에 최대한 적극적으로 치료하는 것이 좋다. 상기한 내용을 종합하면 마비성 사시의 경우에 침치료가 효과가 유의한 것으로 나타났다. 이는 침의자극이 눈 주위의 기혈 순환을 원활히 하거나 직접적으로 근육을 자극하여 마비된 안근의 운동을 돕는 것으로 생각된다.
침치료 vs 약물치료 연구에서는 모두 침치료군이 우세하게 나타났으며, 침 vs 약물 vs 침 + 약물치료 세 가지 그룹으로 나눈 연구에서는 침 + 약물 그룹이 침 또는 약물 단일 그룹보다 효과가 뚜렷하게 나타났다.

후속연구

에 따르면 수술 후 구토 증상은 사시 수술환자의 41-88%에서 나타나는데 내관, 수지침의 K-K9혈 (넷째 손가락 내측 두 번째 마디에 위치), 天柱,大杼, 陽陵泉에 자침을 한 후 antiserotonin 수치가 droperidol, metoclopramide와 같은 진토제를 복용했을 때보다 높았다. 다만 사시 수술 후 소화기 증상에 대한연구가 다양하지 않아 앞으로 이와 관련된 주제로 다양한 연구들이 이루어져야 할 것이다.
