Ohmae-hwan has been used to treat the symptoms of loose stools mixed with blood accompanied by abdominal pain due to heat dysentery in Korean medicine. However, its mechanism of action on inflammatory disease using macrophages was not known. The author was to find out about the influence of Ohmae-hwan water extract on expressions of NO, cytokine IL-6 and TNF-α in LPS-stimulated RAW 264.7 cell, on expressions of mRNA of iNOS, IL-6 and COX-2 and on suppression of expressions of protein of iNOS and COX-2, and was to find out about the mechanism. As a result of the study, it was found that Ohmae-hwan suppressed generation of NO activated by LPS depending on its concentrations, suppressed expressions of cytokine IL-6, and TNF-α and suppressed expressions of mRNA of iNOS, IL-6, and COX-2 and suppressed expressions of protein of iNOS, COX-2. Considering the above results, it can be known that Ohmae-hwan water extract has anti-inflammatory effect against inflammatory response mediated by macrophages by suppressing generation of NO, TNF-α, and IL-6 and expressions of iNOS, COX-2 as it acts on macrophages.

이에 우리는 장기복용에 의한 부작용이 적고, 효과적인 치료효과를 기대할 수 있는 새로운 항염증 물질을 찾는데 주력하였고, 그 중 예로부터 쓰이고 있는 한약처방에 초점을 두어 연구를 진행하였다. 한약처방을 이용하여 항염증 에 관한 연구를 진행한 여러 논문이 보고된 바 있으나5-10), 烏梅丸에 관한 연구는 없었다.

제안 방법
烏梅丸 물추출물은 烏梅丸(18g)을 distilled water (D.W.)를 용매로 하여 2시간 2회 열수 추출하였다. 추출액을 0.
COX에는 COX-1과 COX-2가 존재하는데 COX-1은 항상성을 조절하기 위한 prostaglandin 합성에 관여하고 COX-2는 염증반응 부위에서 활성화된 대식세포에 의해 염증반응을 매개하는 prostaglandin 합성에 관여한다30,31). 본 연구에서는 LPS로 처리된 RAW 264.7 세포에서 烏梅丸은 COX-2의 mRNA와 protein의 발현을 농도 의존적으로 억제하는 양상을 관찰하였다. 이는 염증관련 매개물질 합성을 억제하여 항염증효능을 가짐을 시사하고 있다.
본 연구에서는 烏梅丸 물추출물이 LPS로 자극된 설치류 대식 세포주 RAW 264.7 세포에서 염증 매개물질의 생성과 발현의 억제를 실험하였다.
설치류 대식 세포주 (murine macrophage cell line, RAW 264.7)는 한국 세포주 은행으로부터 구입하여 사용하였다 (Korea Research Institute of Bioscience and Biotechnology American Tissue Culture Collection). RAW 264.
이론/모형
NO 생성에 대한 烏梅丸의 효과를 알아보기 위해 Griess assay 방법을 이용하여 세포 배양액 중에 존재하는 NO2−의 형태로 측정하였다. 烏梅丸을 50, 100, 250, 500 μg/mL 처리하였을 때 NO생성이 농도 의존적으로 감소하는 것을 관찰하였다.
烏梅丸에 의한 염증인자 NO의 억제와 iNOS 및 COX-2의 mRNA발현과의 상관성을 알아보기 위하여 RT-PCR 방법으로 mRNA 발현을 조사하였다
烏梅丸의 iNOS의 발현에 미치는 영향을 보기 위하여 Western blotting을 수행하였다. 그 결과, LPS 처리에 의하여 증가되어진 iNOS의 발현은 烏梅丸을 50, 100, 250, 500 μg/mL 처리 시에 농도 의존적으로 억제되는 양상을 확인할 수 있었다(Fig.
烏梅丸이 세포독성에 영향을 주는지에 대하여 알아보기 위해 MTS 분석법을 이용하여 세포 생존율을 측정하였다. 烏梅丸을 50, 100, 250, 500 μg/mL의 농도로 처리하였을 때 측정한 결과 烏梅丸 50, 100, 250, 500 μg/mL 농도에서 세포독성이 나타나지 않았다(Fig.
세포 생존력은 MTS assay를 이용하였다. 烏梅丸을 처리한 세포에 20 μL의 MTS(3-(4,5-dimethylthiazol-2-yl)-5-(3-carboxymethoxyphenyl)-2-(4-sulfophenyl)-2H-tetrazolium, 5 mg/mL) 용해액을 첨가한 후 37℃에서 1시간 보존한 후 microplate reader를 이용하여 490 nm에서 흡광도를 측정하였다.
성능/효과
RAW 264.7 세포에서 烏梅丸의 NO 생성 저해정도를 관찰한 결과 LPS로 유도된 RAW 264.7 세포에서 NO 생성이 농도 의존적으로 크게 억제되었다.
烏梅丸이 IL-6의 mRNA 발현에 대한 효과를 알아보기 위해 RT-PCR을 수행한 결과, LPS 처리에 의해 증가된 IL-6의 mRNA 발현은 烏梅丸을 50, 100, 250, 500 μg/mL 처리하였을 때 약간 감소하는 양상을 보였으며, 특히 烏梅丸250, 500 μg/ml 에서 억제되는 것을 확인할 수 있었다(Fig. 6).
결론적으로 烏梅丸 물추출물은 세포내의 전염증성 인자들을 현저하게 억제하는 효과가 있어 烏梅丸의 항염증효과를 확인할 수 있었다. 이에 烏梅丸은 만성 염증질환의 치료에도 응용할 수 있는 우수한 후보 물질임을 알 수 있으며, 이에 대한 심도 깊은 연구가 필요하리라 생각된다.
그 결과 烏梅丸 추출물은 LPS로 자극된 RAW 264.7 세포에서 IL-6와 TNF-α 발현이 현저하게 줄어들었으며, 용량 의존적으로 억제되는 양상을 보였다. 이는 염증 매개물질인 IL-6와 TNF-α 발현 억제를 통하여 항염 효능을 시사한다고 볼 수 있다.
이와 같은 결과로 보아 烏梅丸 물추출물은 대식세포에 작용하여 NO, TNF-α, IL-6의 생성과 iNOS, COX-2 발현을 억제함으로써 대식세포를 매개로 한 염증반응에 항염증효과가 있음을 알 수 있다.
當歸는 정유와 sucrose, vitamin B12, Vitamin A류 물질 등을 함유하고 있으며, 자궁에 대한 작용, 물질대사 및 내분비에 대한 영향, 순환계통에 대한 영향, 항균작용 등의 약리효과가 있는 것으로 보고되고 있으며13), 실험적 연구 결과로는 當歸의 약리 작용으로는 항산화19), 항암20), 항염증21) 등이 있는 것으로 알려져 있으며, 枳殼은 정유와 플라보노이드 배당체 등을 함유하고 있으며13), 항산화 및 항염증효과에 관한 연구22), 알레르기 반응에 대한 효과23) 등이 보고되고 있다. 이와 같이 단일 한약재가 나타내는 항염증효과를 감안할 때, 혼합처방인 烏梅丸이 염증에 대해 억제활성을 가진 다는 사실을 유추할 수 있다.
후속연구
결론적으로 烏梅丸 물추출물은 세포내의 전염증성 인자들을 현저하게 억제하는 효과가 있어 烏梅丸의 항염증효과를 확인할 수 있었다. 이에 烏梅丸은 만성 염증질환의 치료에도 응용할 수 있는 우수한 후보 물질임을 알 수 있으며, 이에 대한 심도 깊은 연구가 필요하리라 생각된다.
