Objectives : This study was performed to observe the effect of complex therapy, including electro-acupuncture and magnetic-acupuncture, on peripheral facial nerve palsy. Methods : Nine patients with peripheral facial nerve palsy were treated with acupuncture using electrical and magnetic stimulation. Acupoints in the face were stimulated with an electromagnetic field, as widely and as evenly as possible. To evaluate the effects before and after treatment we used Yanagihara's unweighted grading system, House-Brackmann scale, and Sunnybrook facial grading system and image once a week. Results : After treatment, the scores of Yanagihara's unweighted grading system, House-Brackmann scale and Sunnybrook facial grading system each improved (p-value < 0.05). Conclusion : Complex therapy using electro-acupuncture and magnetic-acupuncture might be an effective treatment to improve symptoms of peripheral facial nerve palsy. Further randomized-controlled trials are required to verify the efficacy and results of this study.

입원 기간 중 환자의 마비가 가장 극대화된 것으로 보이는 시점에서부터 전기와 자기장의 복합 침 자극을 이용한 치료가 시작되고 모든 치료 과정은 6년간 한의과대학 교육을 받고 한의사 면허 취득 후 1년 이상의 임상경험을 가진 한의사 한 명이 시술하였다. 침치료는 입원 기간 동안 하루 2회씩 시행되었고 그 중 일요일을 제외한 월요일부터 토요일까지 1일 1회, 매주 6회씩 전기와 자기장의 복합 침 자극 치료를 시행하였다.

대상 데이터

2016년 3월 1일부터 2016년 5월 30일까지 대전대학교 부속 천안한방병원 침구과에 말초성 안면신경마비로 내원하여 입원치료 받은 환자 중 선정/제외 기준에 부합한 대상은 총 9명이었다.

데이터처리

0 for window를 이용하여 통계처리를 하였다. Y-system과 SFGS는 Paired t-test로 검정을 실시하고 H-B Scale은 비모수 검정법인 Wilcoxon signed rank test를 시행해 분석하였다. 모든 분석은 p-value 값이 0.

이론/모형

자침 전 혈위에 피부 이상이 없는지 확인한 후 일회용78 % 알콜솜으로 소독한 후, 자화 수준을 높이기 위한 304H를 원재료로 제작된 stainless steel 침(0.30 × 40 mm, SMC, 한국)을 사용하였다.

성능/효과

1. 전기와 자기장의 복합 침 자극 치료 후 Yanagihara’s unweighted grading system의 점수가 유의하게 호전되었다(p ＜ 0.001).
2. 전기와 자기장의 복합 침 자극 치료 후House-Brackmann scale이 유의하게 호전되었다(p ＜ 0.01).
3. 전기와 자기장의 복합 침 자극 치료 후Sunnybrook facial grading system의 점수가 유의하게 호전되었다(p ＜ 0.01).
이상의 결과를 통하여 말초성 안면신경마비 환자에게 전기와 자기장 자극의 복합 침치료 후 유의한 효과가 있음을 알 수 있었다.

후속연구

향후 무작위배정 대조군 설정을 통한 임상 연구로 근거 수준을 높일 필요가 있으며, 전기와 자기장 자극 크기에 따른 효과에 대한 연구 및 지속적인 추적 관찰을 진행하여 30일이 지난 만성 환자 또는 안면마비 후유증이 있는 경우에 대한 연구 등 더 객관적이고 지속적인 후속 연구들이 진행될 필요가 있다고 생각된다.
