Objective: The purpose of this study was to present the clinical practice guideline of Korean medicine for malignant lymphoma.Background: Malignant lymphoma is the tenth most common cancer in Korea. The two main types of lymphoma are Hodgkin’s disease and non-Hodgkin’s lymphoma. Non-Hodgkin’s lymphomas are more common, comprising nearly 95 percent of all lymphomas. In China, the traditional Chinese medicine clinical guidelines for malignant lymphoma were published in 2014. Therefore, there is growing need for a clinical practice guideline in Korea, which has not thus far existed. This clinical practice guideline was created by reviewing the Chinese clinical practice guideline and Korean clinical reports. This study will be helpful in understanding malignant lymphoma and in understanding its treatment in Korean medicine.Conclusion: Further clinical research on malignant lymphoma is needed to develop a more advanced clinical guideline.

이론/모형

비호치킨림프종은 연속적이지 않은 림프절종대와 림프절외 기관의 조기 침범을 특징으로 하고 있으며, 병기 결정은 호치킨림프종과 마찬가지로 Ann Arbor 분류법을 사용한다. 비호치킨림프종은 호치킨림프종을 제외한 다양한 질환의 복합체다.

성능/효과

조직학적으로 특징적인 Reed-Sternberg 세포를 보이는 호치킨림프종과 다양한 조직형을 보이는 비호치킨림프종으로 나뉜다. 악성 림프종 환자의 대부분을 차지하는 비호치킨 림프종은 병기뿐만 아니라 어떤 아형이냐에 따라 질병의 예후 및 치료가 달라지는데, 항암요법이나 방사선 치료에 민감하게 반응하는 특징을 가지고 있으며 중등도 및 고도의 진행성 악성 림프종의 경우 효과적인 치료를 하지 않으면 짧은 시간 내에 사망할 수도 있다. 따라서 한의 단독치료를 하기 전에 조직학적 아형, 항암 및 방사선 치료의 경과 등을 고려하여 치료 계획을 세워야 한다.
악성 림프종은 위암이나 대장암 등의 고형암에 비해 발생빈도가 높은 암은 아니지만, 혈액에 생기는 악성종양중에서는 가장 높은 발병률을 보이고 있으며, 계속 그 발생률이 증가하고 있는 질환이다.

악성 림프종 환자의 대부분을 차지하는 비호치킨 림프종은 병기뿐만 아니라 어떤 아형이냐에 따라 질병의 예후 및 치료가 달라지는데, 항암요법이나 방사선 치료에 민감하게 반응하는 특징을 가지고 있으며 중등도 및 고도의 진행성 악성 림프종의 경우 효과적인 치료를 하지 않으면 짧은 시간 내에 사망할 수도 있다. 따라서 한의 단독치료를 하기 전에 조직학적 아형, 항암 및 방사선 치료의 경과 등을 고려하여 치료 계획을 세워야 한다.
임상진료지침을 만들기 위해서는 다양한 환자군 연구가 필요하다. 하지만 악성 림프종의 경우 발병 환자수가 많지 않고, 림프종 분류도 다양하다보니 한의계에서 이중맹검 임상시험이나 환자군 연구를 시행하기 힘든 실정이다.
국내의 한의 임상연구는 증례만이 보고되었으며, 이는 발병환자수가 적고 대부분의 환자들이 항암 및 방사선 치료를 선택하기 때문이라고 생각된다. 현실적인 여건상 악성 림프종 환자들을 대상으로 질 높은 무작위배정임상연구나 환자군 연구를 수행하기는 어렵다고 생각되며, 증례보고나 연속증례보고 등의 임상연구를 통해 한의 치료의 유효성에 대한 근거를 모으는 것이 타당하며, 추후 이를 토대로 환자군 연구를 통한 임상진료지침을 보완하는 것이 필요하겠다.
