Purpose: This study aimed to investigate the influencing factors on self care, respiratory difficulty, sleep impediment, anxiety and depression among patients with neuromuscular disease who are dependent on Home Mechanical Ventilator (HMV). Methods: 157 patients were recruited through hospital based home nursing care departments and HMV rental centers. Data were collected by questionnaires. The analytic methods were n(%)/$M{\pm}SD$, $x^2$/t-test or ANOVA and multiple linear regression. Results: Patients with Amyotrophic lateral sclerosis were more likely to utilize HMV, had shorter periods of morbidity and the most extended daily use of HMV. As for medical equipment, they had ambu-bag (87.2%) and oxygenator (15.4%). Reports of respiratory difficulty and sleep impediment were low. There were no significant difference. In contrast, reported anxiety and depression were high and showed significant difference between non invasive HMV and invasive HMV. Anxiety contributes to respiratory difficulty (t=3.62, p=.002), sleep impediment (t=2.06, p=.042), and depression (t=7.24, p<.001). However, home nursing care exerts a positive influence in reducing anxiety (t=-2.73, p=.008). Conclusion: Anxiety contributes to respiratory difficulty, sleep impediment, and depression. However, those who use home nursing care reported less anxiety. Home nursing care positively impacts patients dependent on HMV as a practical service and available resource.

이에 본 연구는 재가 HMV 적용 환자의 질병과 자가간호에 대한 제 특성을 적용방식에 따라 비교하고, 건강상태인 호흡곤란과 수면장애 및 불안과 우울의 영향요인을 규명하여, 이들의 안위증진에 필요한 가정·방문 간호의 중재개발에 도움이 되는 기초자료를 제공하고자 시도되었다.
제안 방법
자료수집은 사전에 8개 의료기관 가정간호사업소와 2곳의 호흡기임대업체를 방문하여 자료수집에 대한 협조 가능성을 파악한 후, 협조 가능한 해당기관에 근무하는 20명의 간호사에게 연구의 목적과 설문지 작성법을 교육한 후 이들을 통해 자료를 수집하였으며, 직접 작성이 어려운 경우는 가정을 방문하는 간호사가 질문하여 설문지를 대신 작성하도록 하였다. 자료수 집기간은 2012년 10월 1일부터 2013년 2월 28까지였다.
대상 데이터
연구대상자는 근위축성 측삭경화증(Amytrophic Lateral Sclerosis, ALS), 원발성 근육질환(Primary Muscular Diease, PMD), 척수근위축증(Spinal Muscular Atrophy, SMA) 등신경근육계 질환으로 인하여 가정에서 HMV를 적용한 자로서, 장애인복지제도 대상자이며 의사소통이 가능하고 연구참여에 동의한 자이다. 대상자의 수는 선형 회귀분석에 적합한 표본수를 확보하기 위해 효과크기 0.
이에 서울과 수도권 거주자를 주로 하되 기타 지역도 포함하고, 탈락률 20%를 감안하여 설문지 200부를 배포하였다. 회수된 설문지는 총 175부(회수율: 87.5%)였으나 HMV와 관련된 정보가 없는 18부를 제외한 총 157부를 분석 자료로 이용하였다.

대상자의 일반적 특성, 질병 관련 특성, 자가간호, 호흡곤란과 수면장애, 불안과 우울에 대한 HMV 적용방식간 차이분석은 범주형의 경우 x2 test / Fisher's exact test로, 연속형의 경우는 t-test / ANOVA로 분석하였다. ANOVA로분석하여 통계적으로 유의한 경우는 사후 검정으로 Scheffé test를 이용하였다. 또한 호흡곤란과 수면장애 및 불안과 우울의 영향요인은 문헌과 본 연구에서 적용방식에 대한 차이가 있었던 성별, 나이, 진단명, 신체 기능상태, 질병이환 기간, HMV 사용기간, HMV 일 적용시간, 회로변경 주기, 구강간호, 가정.

불안은 실제 또는 상상 속의 위협적인 상황에서 일어나는 불쾌감으로 시간의 경과에 따라 변하는 인간의 정서 상태를 의미하며[24], 우울은 정상적인 기분의 변화로부터 병적인 상태에 이르기까지 연속선상에 있는 근심, 침울함, 실패감, 상실감, 무력감, 수치심 등의 정서 장애를 나타낸다[25]. 본 연구에서 불안과 우울은 Zigmond와 Snaith [26]가 개발한 HADS (Hospital Anxiety and Depression Scale)를 GL Assessment로부터 사용허가를 득한 후 Oh et al.,이 번안한 도구로 측정하였다[27]. 이 도구는 총 14문항으로 구성되어 있으며, 홀수번호 7문항은 불안 관련 문항이고, 짝수번호 7문항은 우울 관련 문항이다.

후속연구
첫째, HMV 적용방식에 따라 현장에서 적용 가능한 간호중재 프로그램을 개발하고 그 효과성을 검증해 볼 것을 제언한다. 둘째, HMV 적용 환자가 퇴원 후 지속적 관리가 가능하도록 전국 지역거점 호흡기센터나 공공의료기관에 가정·방문 간호부서가 개설, 운영되도록 간호사 관련 단체는 이를 국가 해당부서에 건의할 것을 제언한다.
