말기신부전 환자들의 기능적 한계는 무엇인가? | 말기신부전 환자들은 심폐기능 저하, 쉽게 지치는 증상, 피로 등과 같은 심각한 기능적 한계들을 갖고 있다[2]. 특히, 혈액투석 환자는 주 2~3회 4~5시간을 투석실에 머무르고 주로 앉아서 보내는 시간이 많아 근육소실의 위험이 있으며[3] 빈혈, 대사 이상, 심장 자율 조절 기능 손상, 심기능부전 등의 다양한 심혈관계 요인들과 요독성 근병증과 신경병증으로 인한 근육의 산화물질 대사기능 저하, 골격근 위축으로 인해 운동 내성이 현저히 감소하는데, 이러한 상태로 인해 혈액투석 환자의 운동능력은 대체로 감소되어 같은 연령대의 건강한 성인의 최대운동능력(VO2peak)의 50% 수준이다[2]. |
