Purpose: This study investigates the social network, self-care agency, and quality of life of high-risk beneficiaries in case management of Medicaid and the correlations between these variables. It also identifies influencing factors on their quality of life. Methods: The subjects included 187 individuals chosen from the high-risk beneficiaries in case management of Medicaid in D Metropolitan City. Data was collected through direct interviews based on a structured questionnaire on home visits. Results: The perceived health status was the most influential factor in their quality of life, followed by self-care agency, mutual support network, and natural support network in order. These factors explained 40.6% of their quality of life. Conclusion: These findings raise a need to develop a nursing intervention program to increase the self-care agency of the high-risk beneficiaries in case management of Medicaid.

데이터처리

· 대상자의 사회적 관계망, 자가간호역량 및 삶의 질은 실수와 백분율, 평균평점과 표준편차를 산출하였다.
· 대상자의 일반적 특성 및 건강 관련 특성에 따른 삶의 질의 차이검정은 t-test와 ANOVA를 실시하고 Scheffe ́test로 사후 분석하였다.
· 사회적 관계망, 자가간호역량 및 삶의 질 간의 상관관계를 파악하기 위해 Pearson’s correlation coefficients을 실시하였다.

이론/모형

사회적 관계망은 Jeong [16]이 개발한 구조화된 도구를 사용하였다. 사회적 관계망은 자연적, 전문적, 상호적 원조 관계망의 3개 유형으로 구분되며, 유형별로 각 관계망에 대해 크기, 친밀대상, 접촉빈도를 조사하였다.
삶의 질은 세계보건기구에서 개발된 삶의 질 척도를 Min 등[19]이 번역 ․ 수정한 한국판 세계보건기구 삶의 질 간편형 척도(WHOQOL-BREF) 도구를 사용하여 측정하였다. 이 도구는 신체적 건강 영역 7문항, 심리적 영역 6문항, 사회적 관계 영역3문항, 환경적 영역 8문항, 주관적 느낌 2문항 총 26문항으로 구성되어 있다.
자가간호역량은 Geden과 Taylor [17]가 개발한 Self-as CarerInventory를 So [18]가 번역한 도구로 측정하였다. 인지적 측면 11문항, 신체적 기술 9문항, 의사결정 및 판단과정 5문항, 정보추구행위 4문항, 자기조절에 관한 인식 2문항, 자기관리에 대한 주의력 3문항 등 6개 하부 영역 총 34문항으로 구성되어 있다.

성능/효과

본 연구결과 의료급여 사례관리 고위험군 대상자들의 사회적 관계망은 매우 제한되어 있으며, 삶의 질과 자가간호역량도 낮은 수준이었다. 또한 지각된 건강상태와 자가간호역량이 대상자들의 삶의 질에 영향을 미치는 주요 요인으로 나타났다. 따라서 대상자들의 삶의 질을 향상시키기 위해서는 사례관리에서 건강상태 향상을 위한 중재가 이루어져야 하며, 자가간호역량을 높이기 위한 교육 프로그램이 필요하다.
본 연구결과 의료급여 사례관리 고위험군 대상자들의 사회적 관계망은 매우 제한되어 있으며, 삶의 질과 자가간호역량도 낮은 수준이었다. 또한 지각된 건강상태와 자가간호역량이 대상자들의 삶의 질에 영향을 미치는 주요 요인으로 나타났다.

후속연구

둘째, 추후 의료급여 사례관리 고위험군 대상자들의 자가간호역량 향상을 위한 교육 프로그램을 개발하고, 그 효과를 검정하는 연구를 제언한다.
본 연구에서 연구참여자의 개별사례관리 횟수를 조사하지 않았으므로 사례관리를 오래도록 받아온 대상자와 신규로 선정되어 경험이 없는 대상자를 구분하지 않았던 점은 자가간호역량이나 삶의 질의 결과 해석에 있어 제한점이다. 추후 사례관리 경험에 따라 어떻게 변화되어 가는지를 분석해 볼 필요가 있다.
