Purpose: The purpose of this study was to develop a relapse prevention program (RPP) and examine the effects of the RPP on insight, empowerment, and treatment adherence in patients with schizophrenia. Methods: A non-equivalent control group pretest-posttest design was used. Participants were 54 inpatients who had a diagnosis of schizophrenia (experimental group: 26, control group: 28). The study was carried out from February 7, 2012 to February 6, 2013. Over a 10-day period prior to discharge each participant in the experimental group received three one-hour sessions of RPP a one-to-one patient-nurse interaction. Data were collected using Assess Unawareness of Mental Disorder (SUMD), Empowerment Scale, and Insight and Treatment Attitude Questionnaire (ITAQ) and analyzed using PASW 18.0 with chi-square test, independent t-test, Mann-Whitney U test, and ANCOVA. Results: The experimental group had a significant increase in insight and treatment adherence compared to the control group. However, there was no significant difference in empowerment between the two groups. Conclusion: Findings indicate that the RPP for patients with schizophrenia was effective in improving insight and treatment adherence. A longitudinal study is needed to confirm the persistence of these effects of RPP in patients with schizophrenia.

대상 데이터

본 연구의 대상자는 서울특별시에 소재하는 K대학병원의 정신건강의학과 폐쇄 병동에 조현병 치료를 위해 입원한 환자 중에서, 본연구의 목적을 이해하고 연구에 참여할 것을 서면으로 동의한 자이다. 대상자의 선정기준은 다음과 같다.
단기입원기간과 탈락률을 고려하여 대조군 34명, 실험군 29명을 대상으로 연구를 진행하였다. 탈락자는 대조군 6명, 실험군 3명으로 모두 연구기간 중에 퇴원을 하게 된 경우였으며, 최종적으로 대조군 28명, 실험군 26 명의 자료를 분석하였다. 대상자 탈락률은 대조군 17.

데이터처리

연구대상자의 일반적 특성 및 질병 특성은 실수와 백분율, 평균과 표준편차로 분석하였고, 실험군과 대조군의 일반적 특성의 동질성 검증은 Chi-square test, independent t-test로, 질병 특성은 inde-pendent t-test로 분석하였다. 실험 전 종속변수인 병식(실험군 D=0.15, p=.167; 대조군 D=0.17, p=.049), 치료준수(실험군 D=0.18, p=.038; 대조군 D=0.10, p=.200)는 kolmogorov-smirnov로 동질성 검증을 한 결과 정규분포하지 않아 Mann-Whitney U test로, 정규분포하는 임파워먼트는 independent t-test로 분석하였다.

조현병 재발예방 프로그램의 효과를 확인하기 위한 가설검정에서는 정규분포하지 않은 병식은 Mann-Whitney U test로 분석하였고, 임파워먼트, 치료준수는 사전 검사 결과가 동질하지 않아 사전 점수를 공변량 처리하여 공분산분석(ANCOVA)으로 분석하였다.

이론/모형

병식을 측정하기 위해 Amador 등[17]이 개발한 Scale to Assess Unawareness of Mental Disorder (SUMD)를 Song 등[18]이 번안한 도구를 사용하였다. 총 9문항의 4점 척도로서 각 항목은 0~3점으로 측정되는데, 0점은 해당 없는 경우, 1점은 정신장애를 인식할 경우, 2점은 어느 정도 인식할 경우, 3점은 전혀 인식하지 못할 경우에 해당된다.
본 연구에서는 Rogers 등[19]이 개발하였고 Lim [20]이 번역한 임파워먼트 척도를 사용하였다. ‘자기존중-자기효능’, ‘무기력-힘’, ‘지역사회 참여 및 자율성’, ‘낙관적 시각 및 미래에 대한 통제력’, ‘정당한 분노’의 5 요인으로 구성된 총 28문항 중에서, 입원상황의 조현병 환자에게 부적합한 ‘지역사회 참여 및 자율성’과 ‘정당한 분노’요인 항목을 제외한 총 19문항을 사용하였다.
본 연구에서는 치료준수를 측정하기 위해 McEvoy 등[21]이 개발한 구조화된 면담도구인 Insight and Treatment Attitude Ques tionnaire (ITAQ)를 사용하였다. 치료준수 이행 여부를 직접 측정하기에는 시간적 제약이 있으므로 개인의 행동은 행위에 대한 태도에 의해 유발된다고 주장한 합리적 행위이론(Theory of Reasoned Ac-tion)을 기반으로 치료에 대한 태도를 측정하여 치료준수 행동을 예측하였다[27].

성능/효과

본 연구에서 적용한 재발예방 프로그램은 재발예방의 가장 중요한 예측 요인인 치료준수를 향상시키는 데에 효과가 있는 것으로 나타났다. 이는 재발경고 신호를 주 내용으로 한 정신교육 프로그램이 치료에 대한 태도를 향상시켰다는 연구결과[25]와 일치한다.

조현병 재발예방 프로그램 실시 후 실험군은 대조군보다 병식이 향상되었다. 이러한 결과는 초기경고증상 인식이 조현병 환자의 병식과 높은 상관성이 있음을 나타낸 연구[13]와 재발경고징후를 주내용으로 하는 정신교육을 통해 병식 향상을 보인 연구[24,25]를 지지하는 결과이다.

후속연구

연구결과 조현병 재발예방 프로그램은 재발예방에 긍정적인 영향을 주는 간호중재임을 확인할 수 있었다. 그러므로 본 연구의 조현병 재발예방 프로그램을 단기 입원병동 에서 퇴원 시 추후관리 교육 차원에서 활용할 것을 권장하고자 한다.
또한 실험군, 대조군이 한 병동 내에 있을 경우 발생할 확산효과를 막기 위해 시간 차이를 두어 연구를 시행하였기에 실험군, 대조군을 무작위 할당하지 못하였다. 추후 연구에서는 맹검을 이용한 무작위할당 실험연구를 제안한다. 또한 본 연구에서는 두 집단의 결과변수에 대한 사전 동질성이 확보되지 않아 정규분포하지 않았음에도 불구하고 결과변수의 사전점수를 공변량으로 하는 ANCOVA 분석을 선택하였다.
