혈관내피전구세포 수치 증가가 미치는 영향은 무엇인가요? | 혈관내피전구세포는 세포의 통합성과 항상성 유지에 주요한 역할을 하며 혈관내피전구세포 수의 감소는 높은 심혈관계 위험과 관련이 있기 때문에[11] 혈관내피전구 세포는 심혈관 사건의 독립적·비독립적 위험요인으로서 심혈관 사건을 예측하는데 유용하다[12]. 심혈관질환자를 대상으로 한 선행연구에서 혈관내피전구세포 수치 증가가 최대 산소섭취량을 증가시키고[13], 혈관재증식을 증진시킬 뿐 아니라[14], 관상동맥질환자의 스텐트 내 재협착을 감소시키며[15], 혈관내피전구세포 수치는 심혈관 사건 발생, 입원 및 사망률 감소를 예측하는 요인으로[13,14] 보고되고 있다. 즉, 혈관신생을 담당하는 혈관 내피전구세포는 허혈 및 손상 부위에 재혈관화를 촉진하여 심혈관질환자의 질병 과정과 예후에 중요한 영향을 미치게 되며, 이러한 관점에서 심혈관질환자에게 혈관내 피전구세포의 수준과 기능을 향상시키기 위한 전략이 필요하다. | |
혈관형성 과정은 어떠한 과정인가? | 위와 같이 심혈관질환자에게 있어 운동은 매우 중요하며, 심혈관질환자의 운동중재는 심혈관의 혈관내피전 구세포(Endothelial Progenitor Cells, EPCs)의 수를 증가시키고 혈관내피세포의 기능을 향상시키는 등 혈관내 피세포와 밀접한 관련이 있는 것으로 알려져 있다[7,8]. 새로운 혈관은 혈관형성과 혈관신생의 두 가지 경로로 만들어지는데, 혈관형성(vasculogenesis)은 혈관이 없는 부위에서 혈관전구세포로부터 혈관내피세포가 분화되어 혈관 망상구조를 형성하는 과정으로 주로 태생기의 혈관 신생기전이며, 혈관신생(angiogenesis)은 기존의 혈관으로부터 혈관을 구성하는 세포들이 증식하고 이주하면서 가지를 쳐 새로운 혈관을 만들어 나가는 과정으로 성인의 혈관신생의 주된 기전이다[9]. 주로 발생초기 과정에 일어나지만 성인에서도 혈관형성 과정으로 새로운 혈관이 만들어진다[10]. |
