Purpose: The hepatitis B virus is a major cause of chronic liver disease. The clinical guidelines recommend that inactive chronic hepatitis (ICH) patients also check their liver function every 6 to 12 months and manage the potential risks. This study compared the hepatitis B knowledge, self-care practice, and quality of life in patients with HBV according to the disease activity. Methods: This study was conducted in a university hospital and surveyed on 65 ICH patients and 68 progressive chronic liver disease (PCLD) patients from November in 2012 to September in 2013. Results: The knowledge of hepatitis B was lower in the group of a lately perceived HBV infection and ICH. Self-care practice was lower in the male and the patients group with a perceived HBV infection within 5 years. The "taking regular liver function test" score was lower in the ICH. Eight out of 12 Liver Disease Quality of Life instrument (LDQOL) subscales were lower in PCLD. Conclusion: The hepatitis B knowledge and self-care practice are relatively lacking in ICH and the patients group with a perceived HBV infection within 5 years. More effective education programs will be necessary to enhance the hepatitis B knowledge and self-care for patients with HBV and even for ICH.

이론/모형

B형간염 지식은 선행연구(Cho, 2008; Park, 2002)와 B형간염 진료 가이드라인(The Korean Association for the Study of the Liver, 2011)을 토대로 개발한 22문항으로 측정하였다. 이는 B형간염과 관련된 질병의 전파경로, 예방적 활동 및 건강관리에 대한 지식을 측정하는 것으로 각 문항에 대해 ‘예’, ‘아니오’, ‘잘 모름’으로 응답하도록 하였으며, 총 점수는 22점으로 점수가 높을수록 지식이 높은 것으로 해석하였다.
삶의 질은 Gralnek 등(2000)이 개발한 Liver Disease Quality of Life instrument (LDOQL) version 1.0을 Kim 등(2007)이 번안한 75문항으로 측정하였다. 이 도구는 만성 간질환-특이 도구 부분 12개 영역으로 구성되었다.
자가간호수행은 Park (2002)이 개발한 13문항의 5점 척도로 측정하였다. 이 도구는 적절한 영양섭취와 휴식, 정기적인 간기능 검사, 알코올과 약물섭취 주의, 간 질환관련 정보습득 및 위생관리에 대한 수행으로 구성되었다.

성능/효과

면역비활동기 환자는 규칙적으로 병원을 방문하고 정기적으로 검진받기, 임의로 약물을 복용하지 않기, 질환에 대한 정보를 얻으려고 노력하기에서 진행성 만성 간질환 환자보다 자가간호수행 정도가 낮았다. 또한 간질환 특이 삶의 질은 진행성 만성 간질환 환자가 면역비활동기 환자보다 12개 중 8개 영역에서 낮았다. B형간염은 심각한 만성 간질환을 일으키는 주요 원인으로 최근 치료효과가 향상되고 있지만, 아직까지 만성 간질환으로 인한 사망원인에서 높은 순위를 차지하고 있으므로 B형간염 보유자뿐 만 아니라 의료인들도 B형간염에 대한 올바른 인식을 가지고 환자에게 필요한 정보를 제공하는 것이 필요하다.
자가간호수행은 남성과 HBV 보유 사실을 인식한 기간이 짧고, 소화기내과에 등록되지 않은 환자에서 낮았다. 면역비활동기 환자는 규칙적으로 병원을 방문하고 정기적으로 검진받기, 임의로 약물을 복용하지 않기, 질환에 대한 정보를 얻으려고 노력하기에서 진행성 만성 간질환 환자보다 자가간호수행 정도가 낮았다. 또한 간질환 특이 삶의 질은 진행성 만성 간질환 환자가 면역비활동기 환자보다 12개 중 8개 영역에서 낮았다.
본 연구는 면역비활동기 환자와 B형간염 관련 진행성 만성 간질환 환자를 대상으로 B형간염 지식, 자가 간호수행 및 삶의 질 수준을 파악하여 B형간염 교육과 돌봄계획을 수립하는데 기초자료를 마련하고자 하였다. 본 연구의 결과, B형간염 지식은 B형간염을 보유한 사실을 인식한 기간이 1년 미만인 경우, 가족력이 없는 경우, 소화기내과 이외의 임상과에 등록된 환자에서 낮았으며, 면역비활동기 환자가 진행성 만성 간질환 환자보다 낮았다. 자가간호수행은 남성과 HBV 보유 사실을 인식한 기간이 짧고, 소화기내과에 등록되지 않은 환자에서 낮았다.

후속연구

∙ B형간염 보유사실을 최근에 인식한 환자와 면역비활동기 환자의 지식수준이 낮으므로 이들을 대상으로 교육프로그램을 개발하여 적용하는 것이 필요하다.
∙ B형간염 환자를 대상으로 중재프로그램을 적용할 때 만성 간질환 환자의 삶의 질을 고려한 접근이 필요하다.
∙ 진행성 만성 간질환으로 이행할 위험성이 높은 고위험군을 파악하여 정기검진을 강화하는 교육과 홍보가 필요하다.
