Purpose: The purpose of this study was to identify the prevalence of irritable bowel syndrome and factors related to irritable bowel syndrome (IBS) in university students. Methods: A cross-sectional, descriptive study was conducted using a self-report structured questionnaire with 420 university students. Data collection was done during May and June, 2017. The data were analyzed using descriptive statistics, a t-test, $x^2$ test, and binary logistic regression analysis with IBM SPSS/WIN 22.0 program. Results: The prevalence for IBS was 24% among Korean university students. Family history of IBS (OR=2.98, 95% CI=1.32~6.70), history of stomach and intestinal disease (OR=2.24, 95% CI=1.36~3.77), sleep quality (OR=2.01, 95% CI=1.16~3.48), mild anxiety (OR=2.75, 95% CI=1.19~6.33) and more than moderate anxiety (OR=2.02, 95% CI=1.02~4.00) were found to be factors related to IBS. Conclusion: The prevalence of IBS was high among university students, and sleep quality and anxiety significantly influenced prevalence of IBS in these students. Therefore, interventions to decrease symptom of IBS must focus on management of sleep quality and anxiety.

생활습관 요인은 대상자의 식습관, 신체활동량, 현재 흡연 유무, 알코올 섭취정도를 측정하였다.

대상 데이터

5로 설정하였다. 수집된 450명 중 불성실한 응답 및 무응답 자료가 있는 30부를 제외한 420부가 최종분석에 사용하였다.
연구대상자는 서울, 경기도, 충청권 소재 4개의 4년제 대학교에 재학 중인 만 19세 이상 대학생 중 연구목적을 이해하고 연구참여에 자발적으로 동의한 450명을 임의표출 하였다. 대상자 제외기준으로는 위장관 관련 수술을 받은 자, 현재 치료가 필요한 소화기계 질환을 가진 자, 배변활동에 영향을 줄 수 있는 전신질환(예: 종양)을 가진 자이다[1].

데이터처리

이론/모형

IBS의 발생여부는 Drossman 등[1]에 의하여 2006년 9월에 발표된 ROME III를 Song 등[19]이 한국어로 번역한 것을 대한소화기 기능성 질환 운동학회의 허가를 받은 후 사용하였다. 총 10문항으로 1~3번 문항은 복부통증이나 불편감을 경험한 기간을 측정하고, A) 4번 문항은 배변 후 이러한 증상이 사라지는지, B) 5~6번 문항은 배변횟수가 변하는지, C) 7~8번 문항은 대변의 형태가 변하는지를 확인한다.
수면의 질은 Buysse 등[20]이 개발한 Pittsburgh Sleep Quality Index (PSQI)를 MAPI Research Trust로부터 한국어버전 PSQI의 허가를 받아 사용하였다. 수면의 질을 7영역으로 분류하여 주관적인 수면의 질 1문항, 수면 잠복기 2문항, 수면기간 1문항, 습관적인 수면 효율성 2문항, 수면방해 9문항,수면제 사용 1문항 및 낮 동안의 기능장애 2문항 등 총 18문항으로 구성되며, 이들 영역의 점수를 더한 것으로 총합은 0점에서 최고 21점까지이며 측정점수가 높을수록 수면의 질이 낮음을 의미한다.

성능/효과

본 연구는 ROME III 기준을 근거로 국내 대학생의 IBS 유병률을 확인하고 이에 영향을 미치는 요인을 규명한 조사연구이다. 그 결과 본 연구대상자의 IBS 유병률은 24.0%로 나타났는데, 이는 아시아 지역의 유병률이 7.0~9.0%[2,14]라고 보고한 연구와 비교 시 매우 높은 수준이다. 대학생의 IBS 유병률은 사회문화적 차이를 보이는데[13], 국외에서 대학생의 IBS 유병률이 18.
본 연구에서 대학생의 IBS에 영향을 미치는 요인에 대한 다변량 로지스틱 회귀분석 결과 IBS 가족력, 위장관 질환 과거력, 수면의 질과 불안으로 나타났다. IBS의 가족력을 보면 가족력이 있는 대상자가 없는 대상자보다 약 3배 IBS 유병률이 높은 것으로 나타났다.

후속연구

본 연구결과 국내 대학생의 4분의 1이 IBS 증상을 경험하고 있는 것으로 확인되었으며 이에 영향을 미치는 요인은 가족력, 위장관 질환 과거력, 수면의 질과 불안으로 나타났다. 그러므로 국내 대학생의 IBS 유병률을 감소시기키 위해서는 IBS 가족력과 과거 위장관 질환 치료경험이 있는 대학생에 대한 지속적인 관리가 필요하며 수면의 질을 증진시키고 불안정도를 감소시키는 적극적인 관리전략이 요구된다.
그러나 일부 연구에서는 이들 요인이 유의한 영향요인으로 제시되지 않고 있는데[18] 각 연구마다 서로 다른 도구들이 사용되고 있어 그 연구결과를 비교하기가 어렵다. 그러므로 이들 요인들을 측정할 수 있는 신뢰도와 타당도가 높은 도구들을 활용하여 생활습관 요인과 IBS의 연관성을 규명하는 추후연구가 필요할 것으로 사료된다.

	과민성장증후군이 대학생의 건강에 미치는 영향은?	더욱이 대학생들은 IBS 증상으로 인하여 일상생활의 제한을 받으면서도 이를 질병으로 생각하지 않으며 25% 정도만이 전문적 관리를 받는 것으로 보고된다[13]. 대학생의 건강관리 습관은 평생의 건강에 영향을 미치는데[13], 대학생 시기에 IBS에 대한 적절한 중재가 이루어지지 않을 경우 증상으로 인한 강박적 신경증, 불안과 스트레스가 나타나고[5,10] 이러한 정서적 문제는 이들의 직장생활 및 가정생활에도 악영향을 미칠 수 있다[10]. 따라서 젊은 성인기인 대학생을 대상으로 IBS가 단순한 증상이 아닌 전문적인 치료가 필요한 질병임을 인식시키고 증상관리를 도모하는 등 적극적인 중재가 요구된다.
