Methods: We conducted a retrospective review of 959 patients who visited the emergency department of the Dong-Eui University Korean Medical Hospital from January 2016 to December 2016. The review was conducted using electronic medical records created during the emergency department visit. Results & Conclusions: In distribution of sex, the rate of males was 47.9%, and that of females was 52.1%. In distribution of age, the greatest number of patients were in their 50s (27.5%), followed by those in their 60s (19.9%) and 40s (14.8%). In distribution of residence, most patients were from Busan (84.9%). In distribution of week, more patients visited the emergency room on Sundays and holidays (44.3%). September was the busiest month (12.5%). Visits usually occurred during daytime and nighttime, and there were few visits at dawn. In the analysis of the time interval between onset of symptoms and the emergency room visit, most patients visited within 24 hours (46.5%). In the distribution of diseases, facial palsy was the most common (34.6%). In the systematic distribution of diseases, circulatory diseases were the most common (56.7%), followed by diseases of the musculoskeletal system (28.6%). The rate of first emergency room visit was 52.9%, and the rate of revisit was 47.1%. The rate of hospitalization after emergency room treatment was 27.5%, and the rate of discharge was 72.4%. 이에 저자는 2016년 1월 1일부터 12월 31일까지 1년 동안 동의대학교 부속 한방병원 한방응급실에 내원한 환자를 대상으로 한방응급실 이용 현황을 분석한 결과를 보고하고자 한다.

제안 방법
발병 후 응급실에 내원하기 전 경유한 병원 및 처치 분포를 산출하였다.
성별을 분석하고 연령은 주민등록상 기록을 근거로 10세 미만, 10대, 20대, 30대, 40대, 50대, 60대, 70대, 80대 이상으로 분류하여 각 연령대별 내원환자를 조사하였다.
진단명이 복수인 경우 응급실 내원하게 된 주요 증상을 고려하여 하나의 질환을 선택하였고, 확진이 아닌 임상적 추정인 경우 의무기록을 참고하여 분류하였다. 순환기계 질환과 소화기계 질환, 호흡기계 질환, 근골격계 질환, 안이비계 질환, 외상성 질환, 정신신경계질환 및 기타 질환으로 분류하였다.
응급실 내원 당시 변증된 진단명 혹은 주로 호소하는 증상을 바탕으로 하였다. 진단명이 복수인 경우 응급실 내원하게 된 주요 증상을 고려하여 하나의 질환을 선택하였고, 확진이 아닌 임상적 추정인 경우 의무기록을 참고하여 분류하였다.
응급실 내원 시 작성된 전자의무 기록을 바탕으로 후향적 조사를 시행하였으며, 성별 및 연령분포, 거주지별 분포, 요일별 환자분포, 월별 환자분포,내원 시간별 분포, 발병 후 내원까지 걸린 시간 분포, 내원시 질병 분포, 초진과 재진 분포, 입⋅퇴원,전원 분포와 재진율 분포, 내원 전 처치 분포, 응급실에서의 치료 및 처방 분포 등을 비교 분석하였다.
응급실 내원 후에 최종적으로 입원, 퇴원(외래 통원치료 권유 후 귀가), 타병원 전원으로 구분하여 산출하였고, 이들 중 같은 질환으로 본원에 다시 내원한 환자의 수를 산출하였다.
응급실 내원을 기준으로 본원에 처음 방문한 환자와 이전 기록부가 있는 재진 환자를 분류하였다.
응급실 도착시간을 기준으로 각 내원 시간별로 내원 환자 수를 산출하였다. 내원시간은 하루 24시간을 1시간 간격으로 구분하였다.

요통으로 내원하는 환자에서 발병 후 24시간 이내에 내원하는 환자가 49.2%이고, 1주 이상경과 후 내원하는 환자가 26.6%로 급성기에도 많이 내원하지만 만성요통 환자 중에서도 증상 악화에 따라 응급실에 내원하는 환자가 있음을 알 수 있다.

후속연구
본 연구가 한 개별 병원단위의 내원 환자를 대상으로 하였기 때문에 전국에 위치한 한방병원 응급실을 대표하는 것에 한계가 있지만, 한 한방응급실 내에서 변화 동향을 파악함에 있어 한방응급실의 운영을 활성화할 수 있는 방법을 모색할 수 있을 것으로 생각된다.
