This investigation was a descriptive research study that analyzed a total of 137 medical records. Differences in T-score between general and obstetric characteristics were assessed by independent t-tests and one way ANOVA followed by Scheffe's post-hoc test. 문제 정의

제안 방법
골밀도는 출산 후 4주에 이중에너지 X-선 흡수장비(DEXXUM T-오스테오시스)를 이용하여 대상자의 Lumber 1-4(요추)와 Femoral neck(대퇴 경부)의 T-score값 중 낮은 수치를 기준으로 하며. T-score가 -1보다 큰 경우는 정상, -1∼-2.
대상 데이터

성능/효과
과거 총 피임약 복용기간은 복용한적 없음이 59.9%로 가장 많았는데, 본 연구에서는 피임약 복용기간이 골밀도와 유의한 차이는 없는 것으로 나타났다. 하지만 19세 이상을 대상으로 한 선행 연구에서 여성 호르몬제를 복용하지 않는 여성이 골밀도가 더 높게 나타났고[41], 25세~ 45세 여성 154명을 대상으로 한 코호트 연구에서도 경구피임약을 복용한 군이 골밀도가 더 낮게 나타났다[42].
본 연구 결과 임신 12주에 측정된 BMI는 전체 대상자의 51.1%가 과체중으로 나타났다. 임신 초기에는 임산부의 절반이상에서 호르몬 변화로 인한 입덧을 경험하게 되며[18] 이로 인해 불규칙한 식사와 영양불균형으로 오히려 임신 전에 비해 체중감소가 나타나는 경우가 많다고 보고되고 있다[2].

본 연구 결과의 출산 후 4주에 측정된 BMI도 과체중이 전체 대상자의 59.1%로 나타냈다. 이는 출산 후 아직 회복되지 않은 산후조리 기간이라 운동량 감소 및 모유수유로 인한 수면 부족으로 지방분해효소의 분비가 감소되고[20] 수분과 노폐물의 축적으로 인한 결과라 생각된다.
본 연구 대상자의 골밀도 평균은 0.03이였고, 골밀도가 정상인 대상자는 121명(88.3%), 골감소증이 있는 대상자는 11명(11.7%), 골다공증을 진단받은 대상자는 없는 것으로 나타났다(Table.2).

본 연구결과 임신 12~14주에 측정된 비타민 D 수치는 결핍인 대상자가 전체의 82.5%를 차지했고 비타민 D 가 충분한 대상자가 부족한 대상자보다, 부족한 대상자가 결핍인 대상자보다 골밀도가 유의하게 높은 것으로 나타났다(F=4.90, p=.009). 이는 비타민 D의 부족이 골밀도 저하와 골절 위험을 증가시킨다[31]는 결과를 뒷받침해주었다.
본 연구결과 임신 35~36주와 산후 1주일 내에 측정된 헤모글로빈수치는 전체 대상자의 각각 70.8%, 81.8%가 정상보다 낮게 나타나 빈혈이 있는 산모의 비율이 높았고 빈혈에 따른 골밀도는 유의한 차이가 없는 것으로 나타났다. 산모를 대상으로 한 빈혈과 골밀도와의 관계를 비교한 논문은 거의 없어 직접 비교는 어려우나 성인 여성을 대상으로 한 선행연구에서 빈혈이 있는 군이 골밀도가 유의하게 낮은 것으로 나타나[28] 본 연구 결과와는 차이가 있었다.
본 연구에서 분만형태는 자연분만이 71명(51.8%), 제왕절개는 66명(48.2%)으로 나타났는데 자연분만을 한 대상자가 제왕절개를 한 대상자보다 골밀도가 유의하게 높은 것으로 나타났다(t=2.01, p=.046). 선행 연구에서도 제왕절개 대상자가 자연분만 대상자에 비해 산후 초기와 후기에 골밀도가 유의하게 낮아[36] 본 연구결과와 유사하게 보고되었다.
