A Convergence study was attempted to identify the effects of stress on Suicidal ideation and mediating effects of depression in the relationship. The study were 166 elderly's over 65 years at G city and period was from August 29 to September 9, 2018. Data were analyzed PASW 18.0 program. As a result, stress had a significant effect on depression in stage 1, stress had a significant effect on suicide ideation in stage 2, stress and depression of parameter were significant predictors in stage 3. The explanatory power of the two variables was 24.3% in stress, 26.8% for stress accompanied with depression, and the partial mediating effect of depression on the effect of stress on suicide ideation. Based on this study, it can be used as a basic data for reducing suicide ideation of the elderly and study is needed to develop suicide prevention programs.

제안 방법

자발적 참여 의사를 밝힌 연구 대상자에게 연구목적, 익명성 보장과 연구목적으로만 사용됨을 설명하였다. 연구 참여에 대한 서면동의서를 작성하고 자료를 수집하였으며, 소정의 답례품을 설문지 작성 후 제공하였다.

대상 데이터

G 대학의 연구윤리위원회(IRB) 승인을 받은 후 G 시에 거주하는 65세 이상 노인을 대상으로 2018년 8월 29 일부터 9월 9일까지 연구자가 직접 자료수집을 실시하였 다(IRB No:.1041485-201807-HR-001-21). 자발적 참여 의사를 밝힌 연구 대상자에게 연구목적, 익명성 보장과 연구목적으로만 사용됨을 설명하였다.
연구대상은 G 시에 거주 중인 치매 진단을 받지 않은 65세 이상의 노인으로 연구목적을 충분히 이해하고 자발적 참여하여 서면 동의서를 작성한 자를 편의표출 하였 다. 표본크기는 G*Power 3.
90을 고려하여 표본 수를 산출 결과 대상자 수는 총 147명이었다. 탈락률 20%를 고려하여 총 176부 설문지를 배포 및 수거하였고, 불충분한 답변을 한 설문지 10부를 제외하여 총 166부를 최종 분석 하였다.

데이터처리

(1) 대상자의 일반적 특성은 빈도분석과 기술통계분석을 수행하였다.
(2) 대상자의 일반적 특성에 따른 자살생각 정도의 차이는 t-test, ANOVA, 사후분석은 Scheffe test로 분석하였다.
(3) 대상자의 스트레스, 우울 및 자살생각 정도는 평균과 표준편차로 분석하였다.
(4) 대상자의 스트레스, 우울 및 자살생각의 상관관계는 Pearson correlation coefficient로 분석하였다.
(5) 대상자의 스트레스가 자살생각에 미치는 영향과의 관계에서 우울의 매개효과를 검증하기 위해 Baron과 kenny의 절차를 통해서 Stepwise Multipleregression과 Sobel test로 분석하였다.
대상자의 스트레스가 자살생각에 미치는 영향 관계에서 우울의 매개효과를 검증하기 위해 Baron과 kenny(Baron, kenny, 1986)의 3단계 회귀분석을 실시하였고 모든 조 건을 충족하였다.

이론/모형

스트레스는 강인[34]의 도구를 이영자[35]가 수정· 보완하고, 윤정선[36]이 재구성한 도구를 사용하였다.
우울은 미국정신보건연구원(NIMH)의 CES-D (the Centerfor Epidemiologic Studies Depreession Scale) 척도를 조맹제와 김계희[37]가 수정·보완하고 윤정선 [36]이 재 수정 도구를 김춘지[38]가 추가로 재수정한 도구를 사용하였다.
자살생각은 Beck[39]의 자살생각 척도(Scale for suicidal ideation: SSI)를 김창진과 황진수[40]가 수정․ 보완한 도구 사용하였다. 각 문항은 5점 Likert 척도로 ‘매우 그렇지 않다’ 1점에서 ‘매우 그렇다’ 5점까지로 가족친지부담 3문항, 무가치성과 무능력 4문항, 무료감 3 문항, 고통 4문항 총 14문항으로 구성되어 있다.

성능/효과

즉, 자살생각은 연령에서는 85세 이상이 다른 군보다, 학력은 초졸 이하가 다른 군보다 자살생각 수준이 높았 다. 결혼상태는 기혼 외 군(미혼, 사별, 별거 등)이 다른군보다, 동거유형에서는 독거와 기타(시설, 친척 등)군이 다른 군에 비해 자살생각 수준이 높았다. 또한 건강인식은 ‘건강하지 않다’군이 다른 군보다, 월수입은 50만원 이하 군이 다른 군보다, 경제인식은 ‘어렵다’군이 다른 군에 비해 자살생각 수준이 높았다.
대상자의 스트레스 수준은 평균 39.19±12.71점, 평균평점은 2.31±0.75점이었고, 우울 수준은 평균 55.17±11.63점, 평균평점은 2.76±0.58점이었다.
대상자의 일반적 특성에 따른 자살생각 정도의 차이에서 연령은 85세 이상이 다른 군보다, 학력은 초졸 이하가 다른 군보다, 건강 상태는 ‘건강하지 않다’ 군이 다른 군에 비해 자살생각 수준이 높았다.
대상자의 자살생각은 스트레스(r=.497, p<.001)와 우울(r=.411, p<.001) 모두 유의한 양의 상관관계를 나타 냈으며, 스트레스도 우울(r=.533, p<.001)과 유의한 상관관계를 나타냈다.
또한 건강인식은 ‘건강하지 않다’군이 다른 군보다, 월수입은 50만원 이하 군이 다른 군보다, 경제인식은 ‘어렵다’군이 다른 군에 비해 자살생각 수준이 높았다.
노인 우울은 자살생각에 영향을 줄 뿐 아니라[56], 자살생각의 위험요인이기도 하다[62]. 또한 노화로 인한 신체 변화와 더불어 만성질환에 쉽게 노출이 되고 배우자 부재와 경제적 지지약화가 자살생각 수준을 높아지게 하는 것으로 확인 되었다. 따라서 노인의 스트레스 완화와 우울 감소를 통해 자살생각이 감소될 수 있다는 것을 시사한다.
본 연구 대상자의 스트레스, 우울 및 자살생각 간의 상관관계를 살펴보면, 자살생각은 스트레스와 우울 모두 유의한 양의 상관관계였고, 스트레스도 우울과 유의한 양의 상관관계를 보였다. 즉, 대상자의 스트레스와 우울이 심해질수록 자살생각이 높아지게 되며, 스트레스와 우울도 상호 간에 영향을 있는 것으로 확인되었다.
본 연구 대상자의 스트레스가 자살생각에 미치는 영향에서 우울의 매개효과 분석결과, 스트레스가 자살생각에 미치는 영향은 24.3%, 스트레스와 우울이 자살생각에 미 치는 영향은 26.8%의 설명력을 보였으며, 우울은 스트레스가 자살생각에 미치는 영향에서 부분매개효과가 확인 되었다. 노인 대상으로 스트레스가 자살생각에 미치는 영향에 대해 우울을 매개효과 연구가 많지 않아 비교하기는 어려운 실정이다[15,33].

후속연구

둘째, 노인의 스트레스와 우울 관리를 위한 프로그램 개발 및 적용을 위한 후속 연구를 제언한다.
또한 노인 대상은 아니었으나 대도시 거주하는 50 ∼59세 대상 연구에서도 연령이 많을수록 우울의 차이를 보였고[24], 또한 본 연구에서도 65세 이상의 대상자를 선정하여 연령의 차이는 있었으며, 연령이 증가에 따라 우울 수준이 높아질 수 있음을 유추할 수 있으나 거주지, 연령, 성별 및 Likert 척도의 차이로 점수 비교는 무리가 있다 하겠다.
37점[38]으로 본 연구와 유사한 결과를 보였다. 연구 대상자가 400명 이상이었으나 [36], 본 연구는 160명으로 선행연구에 비해 대상자의 특성 반영이 어려웠을 것으로 보이므로, 대상자 수를 확대하여 다양한 특성을 반영하여 비교 연구가 요구된다. 또한 요양 시설에 입소한 대상자로 이들은 건강 상태가 좋지 않은 상태의 대상자였으나[41], 본 연구는 지역사회의 노인 대상자로 병력이 있고 치료 중인 대상자도 있었 으나, 현 건강 상태가 상대적으로 좋은 편이어 낮은 스트레스 수준이었다고 보인다.
첫째, 본 연구는 일개 지역사회 노인을 대상으로 국한된 조사로 연구결과를 일반화하는데 한계가 있으므로 다 양한 지역의 대상자로 확대한 반복연구가 필요함을 제언 한다.
노인들은 자녀와의 단절에 대한 힘들어하였고, 이러한 문제를 개인적인 문제로 인식하고 해결하려고 경향이 컸으며, 자살 의도나 경험을 자연스럽게 노출되므로 가족이나 지역사회 관계망을 이용한 외부의 도움을 요청할 수 있는 사회적 환경 조성이 필요하다[54]. 최근 독거 노인의 자살률이 증가하고 있으므로, 이들을 위한 인적자원의 지지와 더불어 현재 운영되고 있는 프로그램 수정 및 보완을 해나가는 과정이 필요할 것으로 사료된다.

	노인의 자살생각은 우울 및 스트레스와 어떤 연관성이 있는가?	이외에도 평균 기대수명 연장과 의료혜택의 질 향상과 더불어 노인 인구 증가 와 더불어 우울증 환자의 증가를 예상하고 있다[19]. 노 인에게 외로움, 우울감 및 스트레스가 증가하면 자살생각도 증가하였으며, 이 중 우울이 자살생각을 증가시키는 가장 중요한 요인으로 확인되었다[20]. 또한 신체 상태가 악화되면 우울수준이 높아져 자살생각이 더욱 증가하게 되는데[14], 경제적 요인, 건강 요인 및 사회적 소외 등이 자살생각의 80%를 차지하며, 이는 우울과 관련되었다 [21]. 노인의 자살을 유발하는 강력한 요인이 우울과 스트레스로[22], 우울은 스트레스에 의해 증가된다[23].
