본 연구의 목적은 젊은 유방암 환자 배우자의 돌봄 부담감과 영향요인을 확인하기 위한 서술적 조사연구이다. 연구에서는 젊은 유방암 환자 배우자의 부부 친밀감, 희망 및 사회적 지지가 돌봄 부담감에 미치는 영향 정도를 파악하였다. 연구대상자는 B시에 소재한 3개 종합병원에서 입원 중이거나 외래를 방문한 젊은 유방암 환자 배우자 130명이며, 자료수집 기간은 2016년 1월 3일부터 3월 31일까지이다. 수집된 자료는 SPSS win 18.0을 이용하여 mean, t-test, ANOVA, Pearson's correlation coefficients, stepwise multiple regression으로 분석하였다. 분석결과 젊은 유방암 환자 배우자의 돌봄 부담감에 가장 큰 영향을 미치는 요인은 사회적 지지(β=-.41, p<.001)였고, 다음으로 부부 친밀감(β=-.26, p=.001), 나이(β=-.18, p=.009), 지각한 경제적 상태(β=-.14, p=.039)를 포함하여, 총 42%의 설명력을 나타냈다(F=24.41, p<.001). 이에 본 연구의 결과를 바탕으로 젊은 유방암 환자 배우자의 돌봄 부담감을 감소 시킬 수 있는 사회적 지지 프로그램의 개발이 필요하며, 부부 친밀감, 나이, 지각한 경제상태를 고려한 차별적인 프로그램으로 개발할 필요가 있을 것이다.

This study was undertaken to identify factors influencing care burden among spouses of young women afflicted with breast cancer. Totally, 130 spouses of young women with breast cancer, who were hospitalized or visited one of the three general hospitals in B city, were enrolled for the study. Data was collected between January 3 to March 31, 2016. The mean, t-test, ANOVA, Pearson's correlation coefficients and stepwise multiple regression were analyzed by applying the SPSS/WIN 18.0 program. Social support had the greatest impact on care burden (β=-0.41, p<0.001), followed by marital intimacy (β=-0.26, p=0.001), age (β=0.18, p=0.009) and perceived economic status (β=-0.14, p=0.039). The explanatory power was determined to be 42% (F=24.41, p<0.001). Our findings indicate the necessity to develop social support programs that help reduce the care burden of spouses of young women afflicted with breast cancer. Moreover, there is a need to develop differentiated programs by considering marital intimacy, age and perceived economic status.

3) 대상자의 일반적 특성에 따른 돌봄 부담감 정도는 t-test, ANOVA로 분석하였으며, 사후검증은 Scheffé 검증으로 분석하였다.
4) 부부 친밀감, 희망, 사회적 지지, 돌봄 부담감 정도의 상관관계는 Pearson's correlation coefficients 을 이용하여 분석하였다.
5) 대상자의 돌봄 부담감 예측 요인은 Stepwise multiple regression으로 분석하였다.

이론/모형

돌봄 부감감은 Given 등[32]이 개발한 CRA(Caregiver Reaction Assesment)를 도구를 이영선[33]이 번안수정한 CRA-K(Caregivrt Reaction Assesment- Korean Version)를 사용하였다. 하위요인은 생활양식변화, 가족 협조부족, 경제적 부담, 신체적 부담의 4개 영역으로 총 17문항을 사용한 척도를 의미한다.

성능/효과

젊은 유방암 환자 배우자의 돌봄 부담감에 영향을 주 는 변수로는 사회적 지지, 부부 친밀감, 나이, 경제적 상태였으며, 그 중 사회적 지지가 가장 큰 영향요인으로 나타났다. 이에 대상자의 돌봄 부담감을 감소하기 위해서는 사회적 지지가 중요한 요인으로 적용된 프로그램 개발이 필요하고, 부부 친밀감 뿐아니라 대상자의 일반적 특성인 나이, 경제적 특성을 고려한 중재가 이루어져야 할 것이다.

후속연구

둘째, 대상자의 특성을 고려한 차별성 있는 중재 프로그램을 개발하고 그 효과를 규명해 볼 필요가 있을 것이다. 셋째, 폐경기 이후 유방암 환자 배우자에 대한 비교연구가 필요할 것이다.
본 연구를 토대로 다음과 같이 제언한다. 첫째, 젊은 유방암 환자의 배우자 돌봄 부담감 영향요인 중 희망이 영향요인으로 되지 못하였으므로 이에 대한 추후 연구가 필요할 것이다. 둘째, 대상자의 특성을 고려한 차별성 있는 중재 프로그램을 개발하고 그 효과를 규명해 볼 필요가 있을 것이다.
첫째, 젊은 유방암 환자의 배우자 돌봄 부담감 영향요인 중 희망이 영향요인으로 되지 못하였으므로 이에 대한 추후 연구가 필요할 것이다. 둘째, 대상자의 특성을 고려한 차별성 있는 중재 프로그램을 개발하고 그 효과를 규명해 볼 필요가 있을 것이다. 셋째, 폐경기 이후 유방암 환자 배우자에 대한 비교연구가 필요할 것이다.

	부부 친밀감이란 무엇인가?	부부 친밀감이란 갈등 해결력, 정서적 친밀정도, 결혼에 대한 안정감, 성적만족, 부부간의 자신감이나 자존감, 동반자 의식, 독립적 가족관계 유지 및 개방적 의사소통을 포함하여 부부관계에서 주관적으로 느끼는 친밀한 정도를 말한다[12]. 부부관계의 정서적 안정과 원활한 기능은 가족의 안녕을 도모하지만 만족스럽지 못한 부부 관계는 전체가족의 적응능력과 기능을 악화 시킬 뿐 아니라 개인의 심리적 안녕까지도 위협하게 된다[9].
	젊은 유방암 환자의 배우자가 돌봄 부담감을 느끼게 되는 이유는 무엇인가?	젊은 유방암 환자는 공격적인 치료, 조기폐경, 높은 재발율의 심각성 뿐 아니라 생존자로 살아가야 할 기간이 길기에 부인을 돌보는 배우자는 그에 따라 부정적인 경험을 하게 된다[3,4,8]. 배우자는 유방암 아내의 치료 및회복과정 동안 심리적 변화, 사회적 변화, 스트레스 등을 겪게 되는데[9,10], 특히 40대 전후 세대는 가족발달과업중 자녀의 양육과 교육, 가정경제에 집중해야 할 시기로 가장으로서 책임감, 두려움, 불안감을 느끼게 되고, 가정 생활 및 직장생활의 변화와 부부관계의 갈등이 생기면서 돌봄 부담감을 가지게 된다[4,11]. 이에 젊은 유방암 환자 배우자만을 대상으로 돌봄 부담감에 영향을 미치는 요인들을 파악하는 것이 필요하다.
