Purpose: The purpose of this study was to examine the mediating effect of marital intimacy on the impact of uncertainty on the quality of life (QoL) of young breast cancer patients. Methods: This study used a pathway analysis with 154 young breast cancer cases in their early diagnosis stage at a medical center in Korea. Data were collected from November 2016 to February 2017 and analyzed using correlation analysis and pathway analysis. Results: Uncertainty, marital intimacy, and 4 sub-scales of QoL showed a significant correlation. Marital intimacy was directly affected by uncertainty (${\beta}=-.39$, p=.013) and 4 sub-scales of QoL were also affected by uncertainty. Among the 4 sub-scales of QoL, physical well-being (PWB) (${\beta}=.17$, p=.026), social well-being (SWB) (${\beta}=.49$, p=.010), and functional well-being (FWB) (${\beta}=.38$, p=.009) were affected by marital intimacy but emotional well-being (EWB) was not affected by it. The mediating effect of marital intimacy on the impact of uncertainty on QoL was confirmed. Marital intimacy showed a significant indirect effect on PWB (${\beta}=-.07$, p=.024), SWB (${\beta}=-.19$, p=.008), and FWB (${\beta}=-.15$, p=.005), and it means that marital intimacy has a partial mediating effect on the impact of uncertainty on PWB, SWB, and FWB. Conclusion: Effects of uncertainty on QoL was mediated by marital intimacy of young breast cancer patients in their early diagnosis stage. It suggests that marital intimacy needs to be considered in providing nursing intervention for young breast cancer patients.

이러한 연구 결과들을 볼 때, 서구와 비교하여 젊은 여성의 유방암 비율이 높은 우리나라에서[21] 진단 초기의 환자들을 대상으로 삶의 질의 정도와 그 영향요인과의 관계를 파악하는 것이 필요하다. 따라서 본 연구에서는 진단 초기 젊은 유방암 환자가 지각하는 사회적지지의 주요 원천인 배우자와의 관계에 주목하고 환자와 배우자 간의 상호작용에 기반을 둔 부부친밀도가 불확실성이 삶의 질에 영향을 주는 데 있어 매개효과가 있는지 확인하고자 한다. 이러한 인과적 구조를 확인하는 것은 임상실무에서 유방암 진단 후 수술을 앞두고 불확실성 속에서 투병을 시작하는 젊은 유방암 환자들의 삶의 질 증진을 위해 가족, 특히 배우자의 지지와 참여를 유도하는 간호 중재의 근거를 제공할 것으로 본다.
제안 방법
본 연구는 진단초기 젊은 유방암 환자의 불확실성과 부부친밀도가 삶의 질에 미치는 직·간접적인 효과를 확인하기 위해 경로모형을 구축하고 모형의 적합성을 확인한 후 경로분석을 수행하고 매개효과를 검정하는 서술적 상관관계 조사연구이다(Figure 1).
대상 데이터
본 연구는 서울 소재 일개 상급종합병원에서 유방암으로 진단을 받은 후 수술을 위해 내원한 암 병기 0~4기의 환자 중 대상자의 선정기준에 부합하고 본 연구의 목적을 이해하고 참여에 동의한 환자 154명을 대상으로 하였다. 구체적인 선정기준은 다음과 같다.

넷째, 연구모형 내 경로의 직접효과, 간접효과 및 총효과를 분해하고 매개효과의 유의성을 검증하기 위해 부트스트래핑(Bootstraping)검정을 실시하였으며 추가로 Sobel Test를 이용하여 매개효과를 검정하였다.
첫째, 대상자의 일반적 특성과 불확실성, 부부친밀도 및 삶의 질 정도는 백분율, 평균과 표준편차로 제시하였고, 자료의 정규성 판단을 위해 관측변수들의 왜도(Skewness), 첨도(Kurtosis) 및 다변량 정규성을 점검하였다. 대상자의 일반적 특성에 따른 차이 비교는 독립표본 t검정(Independent sample t-test)과 일원배치분산분석(One-way analysis of variance)을 사용하였고 Scheffe’s test로 사후검정을 실시하였다.
둘째, 대상자의 불확실성, 부부친밀도 및 삶의 질과의 관계는 Pearson’s correlation coefficient로 분석하였다.
이론/모형
부부친밀도는 Waring과 Reddon [24]이 개발한 Marital Intimacy Questionnaire를 Kim [25]이 번안한 도구로 측정하였다. 이 도구는 총 8문항의 4점 척도(1~4점)로 총점은 최저 8점부터 최고 32점의 범위를 가지며 점수가 높을수록 부부친밀도가 높음을 의미한다.
불확실성은 Mishel [9]이 개발한 The Mishel Uncertainty in Illness Scale를 Chung 등[23]이 한국어로 번안한 도구로 측정하였다. 이 도구는 불일치성, 애매모호성, 불예측성, 복합성의 4개 하부영역으로 구성되었으며, 총 33문항의 5점 척도로서 최저 33점부터 최고 165점의 범위를 가지며 점수가 높을수록 불확실성 정도가 높음을 의미한다.
성능/효과
본 연구에서 부부친밀도가 삶의 질 하위영역에 미치는 효과에 대한 경로분석 결과, PWB, SWB 및 FWB로의 경로계수는 유의하였으나 EWB에 대한 경로계수는 유의하지 않은 것으로 나타났다. 이는 배우자가 있는 170명의 유방암 환자들을 대상으로 한 연구에서 부부친밀도가 높을수록 삶의 질이 높다고 보고한 결과[18]와 일차 치료가 종료된 185명의 유방암 환자들의 부부친밀도와 삶의 질이 유의한 상관관계를 보인 결과[19]와 유사하다.
본 연구에서는 수술을 앞두고 병동에 입원 중인 진단 초기 젊은 유방암 환자들의 불확실성과 삶의 질의 관계에서 부부친밀도의 매개효과를 경로분석으로 확인하고자 하였다. 분석 결과, 불확실성이 삶의 질 하위영역 중에서 신체적 안녕, 사회적 안녕, 기능적 안녕에 미치는 영향에 있어 부부친밀도의 부분매개효과가 있음을 확인하였다. 따라서 간호 실무현장에서 불확실성을 경험하는 진단 초기 젊은 유방암 환자의 삶의 질 향상을 위해서는 배우자의 지지를 강화하기 위하여 배우자 참여 상담교육프로그램을 개발하고 활성화할 필요가 있다.
후속연구
분석 결과, 불확실성이 삶의 질 하위영역 중에서 신체적 안녕, 사회적 안녕, 기능적 안녕에 미치는 영향에 있어 부부친밀도의 부분매개효과가 있음을 확인하였다. 따라서 간호 실무현장에서 불확실성을 경험하는 진단 초기 젊은 유방암 환자의 삶의 질 향상을 위해서는 배우자의 지지를 강화하기 위하여 배우자 참여 상담교육프로그램을 개발하고 활성화할 필요가 있다. 향후 젊은 유방암 환자 뿐 아니라 배우자가 지각한 불확실성, 부부친밀도 및 삶의 질과 환자와의 상호작용적 영향을 동시에 확인하는 커플자료 분석연구를 제언하며, 배우자참여 상담교육프로그램을 개발하고, 그 효과를 검증하는 종단적 연구를 제언한다.
