Objectives: The purpose of this study was to investigate the effects of oral health behaviors of patients with hypertension and diabetic patients on the number of remaining teeth in Korean adults over 45 years of age, furthermore, we also aimed to develop oral health care programs and health promotion for patients with hypertension and diabetes. Methods: Altogether, 1,526 subjects with hypertension and diabetes from the 6th Korean National Health and Nutrition Survey (2013-2015) were analyzed. Results: A multiple logistic regression analysis was performed to determine the effects of oral health behavior and periodontal disease on residual teeth in patients with hypertension and diabetes after adjusting for age. We found 1.53 times the risk of having less than 20 remaining teeth in the group of brushing more than 3 times a day (95% CI: 1.00,2.35, p<0.001), 2.27 times (95% CI: 1.23-4.17, p0.05), compared to the non-smoking group 0.77 times (95% CI: 0.51-1.14, p>0.05), 1.33 times (95% CI: 0.98-1.80, p>0.05) in the drinking group compared to the non-drinking group, in the periodontal disease group, the risk of having less than 20 remaining teeth was 2.19 times higher (95% CI: 1.48-3.24, p>0.001) than in the normal group. Conclusions: As a super-aging society, develops in Korea, the prevalence of hypertension and diabetes will increase gradually. Therefore, an oral health policy should be implemented at the national level for the prevention and treatment of periodontal disease as well as the management of oral health the elderly.

이에 본 연구는 제6기(2013-2015년) 국민건강영양조사를 이용하여 45세 이상 고혈압, 당뇨환자의 구강건강 행태가 잔존치아 수에 미치는 영향요인을 분석하여 향후 고혈압, 당뇨 환자의 구강건강증진에 필요한 기초자료를 제공하고자 하였다. ;
제안 방법
통계분석을 위해 대상자 정보에 관한 자료를 모두 코드화하였고, 독립변수로 일반적 특성(성별, 연령, 교육 수준, 가구소득, 거주지, 가족형태, 경제활동, 흡연, 음주, 의치사용여부, 잔존치), 구강관리행태(하루 칫솔질 횟수, 치간칫솔 사용여부, 치실사용여부, 구강검진여부)변수를 사용하였다. 인구학적 특성의 성별은 남자, 여자로, 연령은 만 45-64세, 만 65-74세, 만 75세 이상으로 구분하였다.
대상 데이터
본 연구는 질병관리본부와 보건복지부가 우리나라에 거주하는 국민을 대상으로 실시한 제6기(2013-2015년) 국민건강영양조사 원시자료를 이용하였으며, 제6기 국민건강영양조사는 질병관리본부 연구윤리심의위원회의 승인(2013-07CON-03-4C, 2013-12EXP-03-5C, 2015-01-02-6C)을 받아 수행하였다. 원시자료의 표본은 조사에 참여한 전체 대상자수 22,948명 중 만 45세 이상 중년 1,526명을 최종 연구대상자로 선정하였고, 연구 결과의 총 빈도수 불일치는 결측치에 의한 누락분이다.

데이터처리
연구대상자의 일반적 특성은 빈도와 백분율로 분석하였고, 일반적 특성과 잔존치아 수와 구강건강행태에 따른 잔존치아 수는 종속변수를 20개 미만과 20개 이상으로 구분하여 교차분석을 실시하였으며, 만성질환자의 구강건강행태와 치주질환에 따른 잔존치아 수에 미치는 영향을 살펴보기 위해 종속변수인 잔존치아 수를 이분화하여 다중 로지스틱 회귀분석(Multiple logistic regression analysis)을 시행하였으며, 통계적 검증을 위한 유의수준은 α=0.05로 고려하였다. ;
성능/효과
1. 사회경제학적 특성 및 구강건강행태에 따른 치주질환을 분석한 결과 성별에서는 남자, 연령에서는 75세 이상, 교육수준에서는 고졸, 가구소득에서는 하, 흡연군, 음주군, 하루 평균 칫솔질 1회 이하 , 치간칫솔을 사용하지 않는 군, 치실을 사용하지 않는 군, 정기적인 치과검진을 받지 않는 군에서 치주질환이 높게 나타났다. ;
2. 사회경제학적 특성 및 구강건강행태에 따른 잔존 치아 수를 분석한 결과 성별에서 남자, 연령에서는 75세 이상, 교육수준에서는 초졸 이하, 가구소득은 하, 하루 평균 칫솔질 횟수 1회 이하, 치간치솔을 사용하지 않는 군, 치실을 사용하지 않는 군, 정기적인 치과검진을 받지 않는 군, 흡연군, 비음주군에서 20개 미만 잔존치아 수를 보유하고 있는 것으로 나타났다. ;
3. 연령을 보정하여 고혈압과 당뇨병 환자의 구강건강행태와 치주질환이 잔존치아에 미치는 영향을 파악하기 위해 다중로지스틱 회귀분석을 실시한 결과 하루 3회 이상 칫솔질 실시군에 비해 하루 1회 이하 실시군에서 20개 미만 잔존치아를 가질 위험이 1.53배(95% CI: 1.00-2.35, p<0.001)높게 나타났고, 치실사용군에 비해 사용하지 않는 군에서 2.27배(95% CI: 1.23-4.17, p<0.05), 비흡연군에 비해 흡연군에서 20개 미만 잔존치아를 가질 위험이 2.19배(95% CI: 1.48-3.24, p<0.001)높은 것으로 나타났다. ;
후속연구
본 연구결과 치아상실은 전신건강을 유지함에 있어 영양섭취와 소화에도 장애를 초래하게 되지만 생명과 직접적인 영향을 미치지 않는다는 이유로 중요하게 다루어지지 못하고 있는 실정이다. 그러나 구강질환으로 음식섭취에 제약이 따르면 식사의 양과 질이 떨어지게 되고, 영양 불균형을 초래하여 전신건강과 수명에도 영향을 미치게 되므로 만성질환 관리와 동시에 구강질환 관리도 함께 이루어져야 할 필요가 있을 것으로 사료된다.
그럼에도 불구하고 본 연구는 국민건강영양조사 자료를 이용하였다는 점에서 그 의미가 있다 할 수 있다. 향후 추후연구에서 주관적인 설문보다는 대상자의 면접조사와 영양 상태 등의 중재를 통한 폭넓은 연구와 객관적인 진단검사에 의한 분석이 함께 이루어져야 할 것으로 생각된다. ;
