Objectives : Atopic dermatitis is a chronic inflammatory disease characterized by severe pruritus, and may be accompanied by Molluscum Contagiosum, Verruca Vulgaris. This paper aims to examine the clinical implications of the treatment of 3 cases of atopic dermatitis with Molluscum Contagiosum, Verruca Vulgaris. Methods : 3 patients were treated by herbal medicine, herbal acupuncture, acupuncture, moxibustion and external preparations. Photographs of lesions, VAS were used to evaluate the changes in symptoms. Results : Atopic dermatitis is accompanied by Molluscum Contagiosum or Verruca Vulgaris because the underlying cell layer is easily exposed to Molluscum Contagiosum and Human papilloma virus because of scatch by pruritus. And all three cases have been well recovered by korean medical treatment. Conclusions : This study shows that Korean medical treatment is effective to treat atopic dermatitis with Molluscum Contagiosum, Verruca Vulgaris.

② 보통 사마귀 병변에는 직접구용 사자발뜸쑥(안진팜메디)을 직경 1㎜-3㎜의 크기로 말아 환부에 올린 후 불을 붙여 시술하였으며 보통 주 1-2회 시술하였다.
② 약침 : 임상에 사용한 黃連解毒湯 약침은 대한약침제형연구회 원외탕전실에 제조 의뢰하여 사용하였으며, 주 1회 0.3㏄를 30gauge, 1㎖ 1회용 주입기를 사용하여 피내 주입 시술하였다.
한약 처방은 각 환자의 한의학적 변증 결과에 따라 藿香正氣散(Table 1), 小兒甫兒湯(Table 2), 補中益氣湯 (Table 3) 등의 기본처방에 세포성 면역활동 증가와 항바이러스 작용이 있다고 알려진 薏苡仁을 1첩 당 12-20g 범위 내에서 가감해서 처방하였으며, 20첩 60 포(pack)를 1회 1포(pack, 60-80㏄), 1일 2회 복용하도록 하였다.

대상 데이터

증례 1의 경우 왜소하고 흰 피부의 7세 남아로, 평소 피부 건조증과 가려움 증상으로 인해 긁는 행동이 두드러져 상처가 심할 때는 피부과에서 가려움 완화를 위한 항알레르기약을 복용하였다. 하지만 최근 목, 손목, 다리에 보통 사마귀가 발생했을 뿐만 아니라 전염성 연속 종의 주변 피부로의 확산 경향이 크고 아토피 피부염 증상이 전신으로 악화되고 있는 상태였다.
증례 3의 경우 마르고 누런 피부의 9세 여아로, 얼굴에 부분적으로 백색비강진이 발견되며 아토피 피부염으로 가려움을 자주 호소하던 주와, 오금에는 보통 사마귀가 밀집되어 발생하였으며 그 외의 손톱 주변과 이마에도 번져나가고 있는 상태였다. 수면 소화 등의 컨디션은 양호한 편이였으나 컨디션에 따라 변비 경향이 나타나며, 체력이 약한 것을 고려하여 補中益氣湯加味方 (Table 3)을 처방하여 2개월 간 복용하게 하였다.

데이터처리

치료 효과는 사마귀 크기, 개수 및 환자가 자각하는 소양감 정도 VAS(Visual Analogue Scale, 초진 당시 가장 불편한 정도를 10점, 무증상을 0점으로 하여 그 사이를 10단계로 나눔)를 활용하여 불편도를 측정하고 사진 촬영을 시행하였다.

성능/효과

이번 증례에서는 正氣虛弱으로 인해 전염성 연속종과 보통 사마귀가 병발된 아토피 피부염 환자에서 각기 다른 한약 처방을 복용하였지만 扶正祛邪 치법을 이용하여 양호한 치료 효과를 거두었다. 아토피 피부염은 만성피부질환으로 전염성 연속종, 보통 사마귀와 같은 바이러스 감염이 동반되는 경우가 있다.
전염성 연속종과 보통 사마귀가 병발된 아토피 피부염을 주증상으로 호소하는 환자 3례에 대하여 한약, 약침, 침, 뜸 치료를 진행하여 재발, 반흔 등의 부작용 없이 긍정적인 변화를 확인 할 수 있었다. 대상 환자의 수가 적다는 한계점이 있어 이를 보강하여, 향후 바이러스 감염을 동반한 아토피 피부염의 한의학적 치료에 대한 보다 다각적인 연구가 필요할 것으로 사료된다.

후속연구
