독활지황탕에 上焦熱 을 끄는 生地黃과 胃熱을 끄는 石膏를 가미한 이유는? 독활지황탕의 熟地黃은 滋陰補血, 益精塡髓하고 山茱萸는 補益肝 腎, 澀精固脫하고 澤瀉는 利水滲濕, 腎中瀉火하며 熟地黃의 滋膩를 방지하고 茯苓은 補脾하고 澤瀉 를 助하여 利水하며 牧丹皮는 淸熱凉血하고 瘀血 을 제거하는 효과가 있으며 防風은 祛風, 止痛하며 獨活은 祛風, 解表止痛한다15,16. 수술 후 발생하는 수술 부위의 통증은 염증으로 볼 수 있으며, 암의 주변부에서 끊임없이 탈락한 암세포들이 주변 조 직에 침윤하는 과정에서도 염증이 발생하게 되는 데, 淸熱藥(牧丹皮, 生地黃, 石膏)은 소염효과를 통 해 창상의 치유와 종양의 증식과 전이를 일정 부 분 억제할 수 있다17. 따라서 독활지황탕에 上焦熱 을 끄는 生地黃과 胃熱을 끄는 石膏를 가미하였다.
ERAS는 무엇인가? 한편 최근 수술 환자의 병태생리에 대한 이해가 높아지면서 수술로 인한 생리적 스트레스를 줄이고 환자의 빠른 회복을 돕기 위한, 수술 후 조기 회복 (Enhanced Recovery After Surgery, ERAS)이라는 다학제적 환자 관리프로그램이 대두되고 있다4 . ERAS는 “Fast Track Surgery" 또는 “Multimodal Rehabilitation Program"으로도 불리며 통증 조절 등 수술 전반에 걸친 여러 치료적인 부분들에 있어서 수술 후 생리적 스트레스를 줄이고 환자 회복을 촉진하고 더 나아가 수술 후 합병증을 줄이는 것을 말한다4. 암 수술 후유증에서 호소하는 피로, 영양장애 및 체중감소에 근거할 때, 虛勞의 陰虛에 준한 치료로 扶正祛邪 할 수 있다. 육미지황탕은 약리연구에서 cytokine 분비 억제, 면역 증강, 항암 및 항암제 부작용 예방 효능이 in vitro 및 in vivo를 통해 확인 되었으며, 임상에서 암 치료와 항암화학요법 부작용 치료에 사용되었고, 임상시험에서도 암 치료 효능이 확인되었다5,6.
