Objectives: The purpose of this study is to examine the correlation between the critical thinking disposition of dental hygienists and the variables associated with clinical decision making and job performance to analyze factors affecting job performance. Methods: A questionnaire survey was administered to 166 dental hygienists from June 24 to August 8, 2019. We analyzed results with t-test, Mann-Whitney U test, ANOVA, Pearson's correlation analysis and a multiple regression analysis using the SPSS Windows version 23.0(SPSS Inc. IL, USA). Results: Critical thinking disposition was the highest for open-mindedness and clinical decision making ranked highest for canvassing of objectives and values. Job performance ranked highest in terms of assist with dental treatment. Factors affecting dental hygienists' job performance were clinical decision making and critical thinking disposition(p<0.001, adj. R2 =31.4). Conclusions: It is expected that this study will provide basic data for the development of programs to enhance the job performance of dental hygienists. Furthermore, it is necessary to actively support plans such as conducting seminars at dental institutions and providing educational participation opportunities.

대상 데이터

본 연구는 00대학교의 생명윤리심의위원회에서 승인을 받은 후, 광주 · 전남 지역에 소재한 15개의 치과 병·의원에 근무하고 있는 임상 경력 1년 이상의 치과위생사를 대상으로 자료를 수집하였다(1041485-201905-HR-001-51). 대상자의 표본크기는 G*power 3.
95, 예측변인 2개를 투입하였고, 다중회귀분석을 위한 표본 크기를 산출한 결과 최소표본의 크기는 최소 107명으로 산출되었다. 치과의료기관에 근무하는 치과위생사의 과중한 업무가 회수율에 부정적 요소로 작용할 것을 고려하여 최종 표본의 크기는 약 0.58배의 탈락률을 적용한 184명으로 하였고, 회수된 설문지 중 불성실한 응답이 포함된 18부(9.8%)를 제외하여 166부를 최종 분석 자료로 활용하였다.

데이터처리

수집된 자료의 분석방법은 SPSS for Window version 22.0(IBM Co, Armonk, NY, USA) 통계 프로그램을 이용하였고, 빈도분석, 기술통계, t-test, ANOVA, Pearson's correlation analysis과 다중회귀분석을 사용하여 분석하였다. 정규성을 만족하지 않는 경우에는 Mann-Whitney U검정을 실시하였으며, 제1종 오류의 수준(α)은 0.

이론/모형

비판적 사고성향은 권 등[17]이 개발한 측정도구를 사용하였고 이 도구는 8개의 하위영역으로 개방성(3문항), 창의성(4문항), 신중성(4문항), 객관성(4문항), 탐구성(5문항), 도전성(6문항), 진실추구(3문항), 지적통합 (6문항)의 총 35문항으로 구성되었다. 모든 문항은 Likert 5점 척도를 사용하여 측정된 점수가 높을수록 높은 비판적 사고성향을 소유함을 의미한다.
임상적 의사결정능력은 기존 임상적 의사결정능력 척도(CDMNS)[18]를 백[19]이 번역한 도구로 수정하여 사용하였다. 하위요인들로는 가치와 목표 검토, 결론의 평가와 재평가, 대안과 선택 조사, 정보에 대한 조사와 새로운 정보의 일치화로 총 4개 요인으로 나뉘어 각 10문항씩 영역별로 구성되었다.

성능/효과

1. 대상자의 비판적 사고성향은 개방성이 가장 높았고, 임상적 의사결정능력에서는 가치와 목표 검토 영역이 가장 높았으며, 직무수행능력은 치과진료협조가 가장 높았다.
3. 대상자의 직무수행능력에 미치는 영향을 분석한 결과, 비판적 사고성향(β=0.273, p<0.001)과 임상적 의사결정능력(β=0.376, p<0.001)이 직무수행능력에 통계적으로 유의한 영향을 미치는 것으로 나타났다(adj. R² =31.4).

후속연구

그러나 광주 · 전남 일부 지역의 치과위생사를 편의추출하여 일반화하기에는 무리가 있고, 직무수행능력에 대한 보다 다양한 요인들을 투입하지 않은 부분에 한계점이 있다. 따라서 후속연구에서는 치과위생사의 직무수행능력에 영향을 미치는 다양한 예측변수를 파악하고 이를 근거로 한 인과관계를 밝혀낼 수 있는 연구가 진행되어야 할 것이다. 또한 치과위생사의 전문성에 대한 요구도가 변화되고 있으므로 치과위생사의 직무수행능력의 시계열적 추이를 살펴볼 것을 제언한다.
이상의 결과, 치과위생사의 비판적 사고성향과 임상적 의사결정능력이 직무수행능력에 영향을 미치는 주된 요인임을 확인할 수 있었으며, 비판적 사고성향과 임상적 의사결정능력을 고려한 직무수행능력을 향상시킬 수 있는 효과적인 교육과정 개발 및 관련 프로그램의 적용이 필요할 것으로 사료된다.
