Purpose: The purpose of this study was to investigate the improve on brain activity during action observation training for cerebral palsy of diplegia. Design: Randomized controlled trial. Methods: 18 subjects were divide into two groups: action observation training group and a control group. Action observation group practiced repeatedly the action with their motor skill and control group practiced conventional physical therapy. The subjects participated in eighteen 30-min sessions, 3 day a week, for 6week. To confirm the effects on brain activity were evaluated. Results: The results show that the Mu-rhythm was statistically significant increase on the C3 of the action observation training group (p<0.05). Conclusion: The action observation training improves brain activity of a cerebral palsy with diplegia. These results suggest that the action observation training is feasible and beneficial for improving brain activation for the cerebral palsy with diplegia. In the future, I think we need to be actively utilized to the action observation training program in the clinical with the neuromuscular development treatment. And the study on the various the action observation training program that can improve the function of the children with cerebral palsy is thought necessary.

본 연구는 양하지 뇌성마비 아동을 대상으로 동작관찰훈련이 양하지 뇌성마비 아동의 뇌활성도에 미치는 효과를 알아보고자 실시하였다. 연구 방법으로 동작관찰훈련군과 대조군은 일반적 물리치료를 공통적으로 실시하 였고 동작관찰훈련군은 동작관찰훈련을 추가적으로 실시하였다. 훈련 전ㆍ후 뇌활성도를 평가하여 다음과 같은 결과를 얻었다.

대상 데이터

실험 진행 중 대상자의 경기 유발 및 개인적인 사유로 실험에 참여할 수 없거나 참여율이 80% 미만인 대상자는 최종 연구 대상에서 제외하였다. 동작관찰훈련군에서 경기 유발과 개인사유로 실험에 참여할 수 없는 아동 2명이 탈락하였으며, 대조군에서 개인적인 사유로 실험에 참여할 수 없는 아동 2명이 탈락하여 최종적으로 각각 9명씩, 총 18명이 최종 연구 대상에 포함되어 6주 동안 실험에 참여하였다. ;
본 연구 대상자는 양하지마비 뇌성마비로 진단 받고 서울시 소재 K병원, E센터에서 물리치료를 받고 있는 아동 30명을 대상으로 다음의 선별기준을 거쳐 선발하였다. 연구 대상자의 구체적 선정 기준은 양하지마비 뇌성마비 진단을 받은 5∼11세 사이의 아동, 시각적 장애 및 시야 결손이 없는 아동, 연구자의 지시를 따를 수 있는 아동, 대동작기능분류시스템(Gross motor function classification system; GMFCS) 평가에서 level Ⅰ∼Ⅲ 사이인 아동, 도수근력검사에서 발목관절 배측굴곡근, 척측굴곡근이 가+(poor+) 이상인 아동, 대상자에게 본 연구의 목적을 설명한 후 연구에 동의한 보호자의 아동으로 하였다.

데이터처리

전체 대상자는 Shapiro-Wilks 검정법을 이용하여 변수들의 정규성 검정을 실시한 결과, 모든 변수가 정규분포하였다. 대상자의 일반적 특성은 기술통계를 사용하였고, 집단 간 차이를 알아보기 위해 독립표본 t검정을 실시하였다. 동작관찰훈련 전·후를 비교하기 위하여 대응표본 t검정을 실시하였다.

이론/모형

뇌파자료 분석은 뇌파분석 시스템인 Telescan 2.98(Laxtha INC., Deajeon, Korea, 2011)을 이용하여 정량적 분석을 실시하였다. 뇌파분석방법은 측정된 뇌파의 파형을 보고 잡파의 유입여부를 확인하였고 측정된 뇌파 원 자료 (raw data)의 전체 뇌파 중에서 뉴로피드백 시스템을 이용한 파형 관찰을 통해 처음과 마지막 10초를 제외한 60초를 분석에 사용하였다.
본 연구에서는 뇌파 측정을 위해 PolyG-1(Laxtha INC., Daejeon, Korea, 2011)을 사용하여 데이터를 수집하였다. 검사자는 측정에 필요한 전극 부착, 측정 장비 사용, 분석 프로그램의 적용 등에 대하여 뇌파 측정 전문가에게 교육을 받았으며 반복적으로 측정 과정을 훈련받고 실시하였다.

성능/효과

훈련 전ㆍ후 뇌활성도를 평가하여 다음과 같은 결과를 얻었다. 뇌성마비아동의 뇌활성도 변화에서 동작관찰훈련군은 C3영역에서 뮤 리듬이 훈련 전ㆍ후 유의 하게 증가하여, 뇌성마비아동의 뇌활성에 효과적이였다(p<0.05). 본 연구를 통하여 동작관찰훈련이 뇌성마비아동의 뇌활성 변화에 효과가 있음을 확인하였다.

후속연구

본 연구에서 양하 지마비 아동의 인지 수준을 통한 낮은 집중력을 고려하지 못하여 경직에 있어 대조군과 비교하여 유의한 효과를 내지 못한 것으로 사료된다. 본 연구에서 5분 시청 후 신체 훈련을 실시하였으나 차후에는 집중력을 1분부터 점진적으로 늘리는 연구가 필요하다 사료된다.
본 연구를 통하여 동작관찰훈련이 뇌성마비아동의 뇌활성 변화에 효과가 있음을 확인하였다. 앞으로 동작관찰훈련에 대한 근거를 더 확립하고, 양하지마비 뇌성 마비 아동의 뇌활성도를 향상시킬 수 있는 다양한 동작관찰훈련 프로그램에 대한 연구가 필요하다고 사료된다.
