Objectives: The purpose of this study was to identify the meanings and characteristic of the stagnation syndrome, a distinctive clinical syndrome in traditional Korean medicine (KM). Methods: The major ancient Oriental medicine literature, including Huangdi neijing (黃帝內經), Danxixinfa (丹溪心法), and Jingyuequanshu (景岳全書) were examined to identify the semantic change of the stagnation syndrome (鬱證). Also, recently published articles about the stagnation syndrome were searched from databases including MEDLINE, CENTRAL, KMBASE, KISS, NDSL, and OASIS. Results: The term of stagnation was originally used to describe not flowing and clogged situations, and the stagnation syndrome appeared as an independent syndrome in Danxixinfa. As the etiology became more sophisticated over time, emotional factors were mentioned for one of the causes of the stagnation syndrome. However, the major causes and symptoms of the stagnation syndrome were somatic factors. Various articles about stagnation were searched, and most of them used "stagnation" as the KM syndrome subtype of disease, some of them referred to the "stagnation syndrome" as an independent syndrome. The recently defined stagnation syndrome commonly shows distinctive symptoms of chest stuffiness, and an obstructing sensation in the throat. Conclusions: The semantic changes and characteristics of the stagnation syndrome were examined through searching ancient and modern literature. The meaning of the stagnation syndrome has evolved over time, and at its center, there are somatic and mental symptoms characterized by stagnation, distinguished from the depressive disorder.

본 연구에서는 울증과 주요우울장애를 명확하게 구분 하고자, 역사적으로는 울증의 개념 성립과 생리, 병리적 표현으로써 의미 분화를 살펴보고 질병 치료적 측면에서 사용된 양상을 고찰하였다. 또한 현대 한의학에서 울증을 어떻게 사용하고 있는지를 확인하고자 학술지에서 출판된 연구 논문을 검색하였다.

국내데이터베이스 검색원으로부터 총 229개의 논문이 검색되었다. 울증에 대한 진단 평가 및 치료와 관련된 논문들을 중심으로 살펴보았다. 우울증에 대한 논문 중에서 울증에 대한 정보가 전혀 없는 것은 관련이 없는 것으로 보아 제외하였다.
2006년 홍콩대학교의 Siu-man Ng는 현대의학의 우울증과 비교되는 개념으로서 중의학의 울증(Yu zhi, Stagnationsyndrome)의 개념과 특성에 대하여 기술하고 이를 평가할 수 있는 척도를 개발하였다. 이 연구자는 울증의 임상 양상에 대하여 신체적(Body) 증상으로는 굳은 표정과 부자연스러운 몸의 자세, 심리적(Mind) 증상으로는 생각하는 데 에너지를 많이 쓰고 분노, 기쁨, 슬픔 등의 감정을 억제하는것, 울증의 영적(Spirituality) 증상으로 어떤 것에 대한 집착(over-attachment to things)이 강해서 내려놓기 어려움 등을 제시하였다. 이를 바탕으로 울증 척도를 개발한 결과, 울증의 특징을 1) 집착(over-attachment), 2) 몸과 마음의 답답함(Body-Mind Obstruction), 3) 표정과 자세의 강직(Affect-Posture Inhibition)이라는 세 가지 요소로 정리할 수 있었다³⁾.
울증(鬱證)의 개념정립을 위하여 고전문헌과 현대문헌을 살펴보았다. 첫째, 시간의 흐름에 따른 울증의 의미 발전 과정을 알아보기 위하여, ≪황제내경≫ 등 고대 동아시아 의학문헌에서 울이라는 용어의 쓰임을 살펴보았다. 울(鬱)이신체 증상 표현 목적으로 사용된 경우 빽빽하거나 무성하다는 사전적 의미와 유사한 상태를 지칭하였다.

제안 방법

세부 변증으로 울(“stagnation”)의 용어가 사용된 논문들을 해당 질환에 따라 나누어 정리하였다(Table 2).
셋째, 국내에서 보고된 울증(鬱證) 관련 논문을 살펴보면,여드름, 주요우울장애, 갱년기 증상, 구강작열감증후군 등 다양한 질환 환자를 울증의 범주에서 진단하고 치료하였다. 울증(鬱證)은 협의(狹義)와 광의(廣義)로 나누어 파악할 수 있는데, 협의의 울증은 정신적 소인으로 기기(氣機)가 울체된 것을 말하고, 광의의 울증은 외사(外邪), 정지(情志) 등의 원인으로 발생한 울증을 포함하고 있다.
울증(鬱證)의 개념정립을 위하여 고전문헌과 현대문헌을 살펴보았다. 첫째, 시간의 흐름에 따른 울증의 의미 발전 과정을 알아보기 위하여, ≪황제내경≫ 등 고대 동아시아 의학문헌에서 울이라는 용어의 쓰임을 살펴보았다.
1). 질환의 하위 변증 분류로 울이라는 단어가 사용된 논문들은해당 질환의 분류에 따라 나누어 정리하였으며, 울증이 단독병명으로 사용된 논문들을 바탕으로 해당 울증의 특징에 대하여 표로 정리하였다.
2). 포함된 논문 중 울증 환자에 대한 증례 보고 및 단면 연구를 중심으로 각 논문에서의 울증 판단/변증 기준 및 질병명, 치료 방법을 표로 정리하였다.

대상 데이터

OR "stagnation syndrome"[All Fields]) OR ("stagnation"[All Fields] AND "Chinese medicine"[All Fields])국내데이터베이스로는 KMBASE (의과학연구정보센터,http://kmbase.medric.or.kr), KISS (한국학술정보, http://kiss.kstudy.com), NDSL (과학기술정보연구원, http://www.ndsl.kr), OASIS (한국한의학연구원, http://oasis.kiom.re.kr)에서 울증(鬱證)을 제목에 포함하는 논문들을 검색하였다(검색일: 2020년 4월 9일).
국내데이터베이스 검색원으로부터 총 229개의 논문이 검색되었다. 울증에 대한 진단 평가 및 치료와 관련된 논문들을 중심으로 살펴보았다.
국외데이터베이스 검색원으로부터 총 1,049개의 논문이 검색되었다. 검색된 논문 중 동물실험 논문이나 鬱의 의미로“stagnation”이 사용되지 않은 논문을 관련이 없는 것으로 보아 제외하였다.
국외데이터베이스인 MEDLINE(미국 국립의학도서관, http://www.pubmed.com), Cochrane CENTRAL(코크 란, http://www.cochranelibrary.com)을 검색원으로 하여 “stagnation syndrome”을 포함하는 논문들을 검색하였다.
울증에 대한 선행 연구7) 및 교과서에서 참고한 의서를 비롯하여 시대별 주요 의서들 중 ‘울(鬱)’이라는 표현이 등장하거나 독립적으로 다룬 내용들을 수집하였다.

성능/효과

1. 시간의 흐름에 따른 울증의 의미 변화를 알아보기 위해 주요 고대 문헌을 중심으로 살펴본 결과, 황제내경에서 울이라는 단어는 빽빽하게 막히거나 답답한 자연 현상, 병리현상, 증상을 표현하기 위해 사용되다가 ≪단계심법≫에서부터 울증이 독립적인 병증으로 제시되었다. ≪경악전서≫에서는 울증의 원인 중 하나로 정서적인 것을 추가하였으나, 울증의 주된 원인 및 치법에 대한 논의는 기(氣), 혈(血), 식(食), 담(痰), 풍(風), 습(濕), 한(寒), 열(熱) 등 물리적인 요소들의 울체에 집중되었다.
2. 울증 관련 현대 문헌을 살펴본 결과, 울이라는 용어는 간기울결과 같이 질병의 하위 변증 분류로 사용된 문헌이 대다수였고, 그 질병의 범주는 소화기질환, 비뇨생식기질환, 정신질환 등으로 다양했다. 울증을 독립적으로 다룬 소수의 문헌으로부터 울증의 특성을 정리해보면, 공통적으로 가슴이 답답하고 목이 막히는 것 같은 신체 증상이 있었다.
3. 고전 문헌 및 현대 문헌에서 울증의 용례를 살펴보았을 때 정서적인 요인은 울증의 다양한 원인 중 하나로 제시되었다. 울증은 신체 증상이 위주라는 점에서 심리 증상이 위주인 우울 장애와 구별되며, 울증의 심리 증상 또한 불안, 집착 등으로 나타나 슬픔, 공허감 등이 위주인 우울 장애와 차이가 있다.
한편 울증의 심리적, 영적증상으로 생각하는 데 에너지를 많이 쓰는 것, 어떤 것에 대한 집착이 강해서 내려놓기 어려운 것 등을 제시한 연구³⁾도 있어 주목할 필요가 있다. 결과적으로, 울증은 신체 증상이 위주인 신체화 질환이라는 점에서 심리 증상이 위주인 우울장애와 다르게 정의할 수 있으며⁵⁰⁾, 울증의 주요 심리 증상 또한 불안, 또는 집착 등으로 슬픔, 공허감, 절망감 등이 주요 심리 증상인 우울 장애와 다른 부분이 있다(Table 5).
동의신경정신과 학회지에 발표된 논문들에서는 한의신경정신과학 교과서에 기재되어 있는 울증의 변증 별증상을 확인하고 그에 따른 처방을 사용한 증례가 많았으며, 주요우울장애 진단을 만족하는 환자들이 많았다^38-41). 기울화화(氣鬱化火)형 울증은 억울한 기분(精神抑鬱), 마음이 편안하지 않음(情志不寧), 답답하고 화가 잘 남(煩躁易怒) 등의 증상을 보이는 것, 심비양허(心脾兩虛)형 울증은 잘 잊어버리고(健忘), 우울, 기억장애, 주의력 저하 등의 증상을 보이는 것, 담기울결(痰氣鬱結)형 울증은 우울하고 불안하며 가슴이 두근거리는 증상을 보여서 해당 변증으로 판단되었다. 한편 1996년 출판된 동의신경정신과학에서는 기울의 증상으로 기분이 우울함, 명치 아래가 답답함, 식욕부진, 팔다리가 무거움, 배가 더부룩함, 대변이 굳어짐 등의 7가지를 제시하였는데, 이러한 기준을 따른 기울증 환자 대상 연구도 있었다⁴²⁾.
둘째, 국외 데이터베이스에서 울증 또는 울(“stagnation”)을 검색하였을 때, 주로 간기울결을 하위 변증 유형으로 포함하는 병명들이 다수 검색되었다.
이상에서 울증이라는 목차 및 병명이 지칭하는 범주가 시대에 따라 점차 확장되었음을 알 수 있다. 그러나 시대에 따라 변하더라도 그 핵심에는 주단계의 육울(六鬱)이나 그에 준하는 기기(氣機)의 울결이 있다.
종합해보면 울증의 의미는 시간에 따라 변화 및 발전해왔으며, ≪황제내경≫에서 빽빽하고 무성한 자연 현상에 대한 표현, 막혀서 정상적인 흐름을 방해한다는 병리적 상황에 대한 표현, 답답하다는 증상에 대한 표현으로 사용된 것으로부터 ≪단계심법≫의 육울(六鬱)에서부터 울체을 중심에 둔 논의가 발전하였다. 이 때 울증은 기(氣), 혈(血), 열(熱), 식(食), 습(濕), 담(痰)이라는 주어와 연결되어 이들이 뭉쳤을 때 나타나는 여러 증상에 대한 설명 및 치법이 기술되어 있다.

후속연구

이를 바탕으로 협의의 울증은 서양의학의 신경증적 장애와 유사하고 신경쇠약 등에서 울증의 증상을 보이는 경우가 많으며, 이외에도 갱년기 장애, 우울증, 정신병적 장애에서도 울증의 형태를 보일수 있다고 하였다⁵¹⁾. 그러나 울증의 증상을 보일 수 있는 질환의 범주는 신경증적 장애 뿐 아니라 피부 질환, 소화기 질환, 비뇨생식기 질환 등으로 매우 다양하므로, 울증의 주요 특징은 막힌 듯 답답함을 위주로 한 신체 증상으로 파악하는 것이 바람직할 것이다.
울증은 신체 증상이 위주라는 점에서 심리 증상이 위주인 우울 장애와 구별되며, 울증의 심리 증상 또한 불안, 집착 등으로 나타나 슬픔, 공허감 등이 위주인 우울 장애와 차이가 있다. 따라서 울증을 우울증과 동일시하거나 상호포함관계로 해석하지 않도록 구별하여 사용해야 할 것이다.
이는 향후 중국및 일본의 데이터베이스를 포함하여 울증(鬱證) 관련 문헌 검색을 수행해볼 수 있다. 또한 문헌 연구 결과를 임상으로 확장하기 위하여, 실제 환자를 대상으로 우울증과 구별되는 울증(鬱證) 환자의 특성을 탐색하는 임상 연구가 이루어질 필요가 있다.
