Purpose: This study involeved a descriptive survey to identify clinical practice satisfaction, self-esteem, and nursing professionalism of nursing college students and determined the factors that affected their nursing professionalism. Methods: Data were collected through a survey of conducted from December 4 to December 20, 2019 (N=186). The relationship among variables was analyzed using Pearson's coefficient correlation and affecting factors for turnover intention were identified by using multiple linear regression. Results: The research results demonstrate that the mean of nursing students' clinical practice satisfaction mean was 3.64 (5 total), the mean of self-esteem was 3.84 (4 total), and the mean of nursing professionalism mean was 3.89 (5 total). Nursing professionalism was significantly different by age and satisfaction with major. Nursing professionalism had positive relationships with clinical practice satisfaction, self-esteem, and satisfaction with major, but had a negative relationships with age. Clinical practice satisfaction, self-esteem, satisfaction with major, and age were identified as influencing factors in turnover intention, and the explanatory power was 43.3%. Conclusion: A strategy to increase clinical practice satisfaction and to enhance self-esteem in nursing college students is necessary. Likewise, further analysis and programs to improve clinical practice environments and training systems should be implemeted.

대상 데이터

본 연구는 경기도, 인천시, 강원도에 소재한 3개 대학교 간호학과 3, 4학년을 대상으로 간호학 실습교과목을 이수중인 학생을 편의표집하였다. G*Power 3.
95, 예측변수 3개(임상실습만족도, 자아존중감, 간호전문직관)의 조건에서 필요한 최소표본수 119명과 더불어 일원배치 분산분석을 위해 같은 조건으로 독립 변수 11개로 계산하였을 때 최소 표본크기가 178명으로 산출되었으나 미회수, 탈락과 불충분한 자료를 감안하여 약 220명에게 배포하였고 198부를 수집하였다. 이 중 12부가 자료 불충분으로 탈락하였으며 최종적으로 186명의 자료를 분석하였다.

데이터처리

• 대상자의 일반적 특성에 따른 임상실습 만족도, 자아존중감, 간호전문직관 정도의 차이는 t-test와 one-way ANOVA, 사후 검정은 Scheffé test를 사용하였다.

이론/모형

간호전문직관은 Han, Kim과 Yun(2008)이 요인분석을 통한 구성타당도 검증으로 축소한 18개 문항을 사용하여 측정하였다. 본 도구는 전문직 자아개념(6문항), 사회적 인식(5문항), 간호의 전문성(3문항), 간호계의 역할(2문항), 간호의 독자성(2문항) 등 5개의 하위영역으로 구분되며 5점 Likert 척도(‘전혀 그렇지 않다’=1점, ‘매우 그렇다’=5점)로 부정적 문항은 역산 처리하여 총점수가 높을수록 간호전문직관이 높음을 의미한다.
임상실습 만족도는 Cho와 Kang(1984)이 개발한 임상실습 만족도 측정도구를 Lee, Kim과 Kim(2004)이 수정·보완한 31개 문항의 도구를 사용하여 측정하였다. 본 도구는 실습교과(3문항), 실습내용(6문항), 실습지도(9문항), 실습환경(7문항), 실습시간(3문항), 실습평가(3문항) 등 6개의 하위영역으로 구분되며, 5점 Likert 척도(‘전혀 그렇지 않다’=1점, ‘매우 그렇다’=5점)로 부정적 문항은 역산 처리하여 총점수가 높을수록 임상실습 만족도가 높음을 의미한다.
자아존중감 Rogenberg(1965)가 개발한 자아존중감 척도(Self-esteem scale)를 사용하였다. 이 도구는 자아존중의 정도와 자아승인의 양상을 측정하기 위해 고안된 것으로 긍정인 문항 5개, 부정인 문항 5개로 총 10문항으로 구성되었다.

성능/효과

이에 매 학기 구체적인 항목으로 구성된 실습만족도 평가를 실시하고, 분석한 내용을 토대로 실습환경 향상을 위한 전략을 도모해야 한다. 둘째, 비교과과정으로 간호대학 신입생부터 자아존중감을 증진시킬 수 있는 프로그램의 개발 및 적용이 필요하다.
이는 간호대학 인증평가 등의 각종 평가제도에 의해 실습환경은 개선되었으나, 각종 평가로 인한 교수의 업무량 증가와 학생 수의 증가로 인해 실습지도와 실습내용 등에서는 오히려 만족도가 떨어진 결과와 관련이 있을 수 있다고 볼 수 있다. 따라서 실습지도와 실습내용은 실습지도교수가 개선할 수 있는 부분이 많으므로 간호대학교수가 더 실습에 역량을 집중할 수 있도록 인증 평가제도 등의 교육 이외의 업무를 줄일 수 있는 정책의 변화가 필요하다 하겠다.
그 다음으로 영향을 주는 요인인 자아존중감과 관련 있는 전공만족도를 높이기 위해 지도교수와의 심층면담을 통해 전공만족도가 떨어지는 학생을 조기 발견하고, 학과에서는 간호전문직관 향상에 도움이 되는 비교과 과정을 체계적으로 운영함으로써 선배 간호사와의 만남, 간호전문가 초청 강연 등의 프로그램을 개발하고, 적용해보는 방안이 도움이 되리라 생각된다(Oh & Kang, 2017). 영향요인으로 나타난 연령과 관련하여 30세 이상의 간호대학생들을 위한 집중관리 프로그램을 마련하고 지속적으로 적용도 필요하다.
