Purpose: This study aims to investigate the factors affecting fatigue of care helpers in long-term care facilities. Methods: The subjects consisted of 131 care helpers at six long-term care facilities in Chungcheongnam-do. A survey was conducted between July and September 2019 to collect data, which were analyzed using t-test, ANOVA, Pearson's correlation coefficients, and multiple regression analysis.Results: An increase in fatigue of care helpers owing to taking care of over 10 patients a day, increase in emotional labor and sleep disturbance, and a decrease in economic status was observed. Their fatigue decreased when there was an increase in their subjective health status and job satisfaction. Work-related stress (β=.37, p<.001), emotional labor (β=.21, p=.014), and sleep disturbance (β=.18, p=.026) significantly predicted fatigue. These factors explained 37% of the total variance in fatigue (F=10.48, p<.001).Conclusion: In addition to improving the work environment, interventions that alleviate emotional labor and improve sleep quality, are needed to lower the fatigue of care helpers.

05, 검정력 90%이고, 예측 영향변인은 6개로 하였을 때 116명이 산출되었다. 탈락률을 고려하여 135부를 배부하고 135부가 회수되었으나 그 중 설문지 작성이 불충분한 4부를 제외한 총 131부를 최종 분석하였다.

데이터처리

22 수준으로 왜도 절대값이 2보다 작고, 첨도 절대값이 7보다 작아 정규성 분포의 가정이 충족되었다. 대상자의 일반적 특성, 피로, 피로의 영향요인(주관적 건강상태, 업무 스트레스, 직무만족도, 감정노동, 수면장애)의 수준은 빈도와 백 분율, 평균과 표준편차로 확인하였고, 일반적 특성에 따른 피로 정도의 차이는 t-test와 ANOVA로 파악하였다. 피로와 피로 영향요인 간의 상관관계는 Pearson correlation coefficients로파악하였으며, 피로에 영향을 미치는 정도는 다중공선성 진단후 입력방식의 multiple regression으로 파악하였다.

이론/모형

피로는 Tack (1992)이 개발한 도구를 Lee, Lee와 Song (1999)이 번안한 도구로 측정하였다. 이 도구는 총 16문항으로, 지침(scoring manual)에 근거하여 점수화 하였다(Lee et al1 999; Tack, 1992).
주관적 건강상태는 시각상사척도(Visual Analogue Scale, VAS)로 측정하였다. 10 cm의 눈금이 그려지지 않은 수평자를 이용하였으며, 자의 왼쪽 끝은 주관적으로 지각하는 건강 정도가 매우 나쁜 상태(0)를, 자의 오른쪽은 매우 좋은 상태 (10)를 의미하며 자신의 건강상태에 대해 해당되는 지점을 표시하도록 하였다.
업무 스트레스는 시각상사척도(Visual Analogue Scale, VAS)로 측정하였다. 10 cm의 눈금이 그려지지 않은 수평자를 이용하였으며, 자의 왼쪽 끝은 요양보호사 업무로 인한 스트레스 정도가 매우 낮은 상태(0)를, 자의 오른쪽은 매우 높은 상태 (10)를 의미하며 자신의 스트레스 정도에 대해 해당되는 지점을 표시하도록 하였다.

감정노동은 Kim (1998)이 개발한 도구를 Kim (2014)이 요양보호사에게 맞게 수정한 도구로 측정하였다. 이 도구는 감정노동빈도 3개 문항, 바람직한 감정표현에 요구되는 주의정도 3개 문항, 그리고 감정부조화 3개 문항 등 총 9개 문항으로 구성되어 있다.
수면장애는 Buysse, Reynolds, Monk, Berman과 Kupfer (1989)가 개발한 도구(Pittsburgh Sleep Quality Index, PSQI) 를 Sohn, Kim, Lee와 Cho (2012)가 번안한 도구로 측정하였다. PSQI는 총 19문항으로, 주관적 수면의 질(subjective sleep quality), 수면잠복기(sleep latency), 수면지속시간(sleep duration), 수면효율(habitual sleep efficiency), 수면방해(sleep disturbance), 수면제 사용(sleep medication use), 주간기능 장애(daytime dysfunction)의 7개의 하위 영역으로 구성되어 있다.

성능/효과

요양보호사의 피로 영향요인을 확인한 결과, 업무 스트레스, 감정노동, 수면장애가 주요 요인임을 확인하였다. 업무 스트레스는 피로에 가장 큰 영향 요인으로 확인되었다.
본 연구는 장기요양시설 요양보호사가 경험하는 피로 정도와 그 영향요인을 파악하여 피로를 감소시키기 위한 프로그램 개발의 기초자료를 제공하고자 시도되었다. 본 연구결과, 경제 상태가 어려울수록, 하루 담당 환자수가 10명 초과인 경우 피로 정도가 높았다. 주관적 건강상태와 직무만족도가 높을수록 피로 정도가 낮았고, 업무 스트레스, 감정노동과 수면장애가 높을수록 피로 정도가 높았으며, 업무 스트레스, 감정노동, 수면장애는 피로의 유의한 예측요인으로 나타났다.
본 연구결과, 경제 상태가 어려울수록, 하루 담당 환자수가 10명 초과인 경우 피로 정도가 높았다. 주관적 건강상태와 직무만족도가 높을수록 피로 정도가 낮았고, 업무 스트레스, 감정노동과 수면장애가 높을수록 피로 정도가 높았으며, 업무 스트레스, 감정노동, 수면장애는 피로의 유의한 예측요인으로 나타났다.

후속연구

이상의 결과를 통해 요양보호사의 피로 완화를 위해서는 경제상태가 어려운 요양보호사의 피로관리방법의 다각적인 모색이 필요하며, 적정 담당 환자 수 조절 등의 업무환경 개선을 통한 업무 스트레스 감소와 직무만족도 향상을 도모하여 피로를 조절하기 위한 대책 마련이 요구된다. 또한 요양보호사의 감정노동 완화를 위한 심리적 지원과 수면의 질 증진을 돕는 실제 적인 중재방안의 개발 및 적용이 필요하다.
