Purpose : This study aims to investigate the awareness of occupational therapists among managers and directors of adult day care centers in Korea to explore the cause of the low employment rate of these therapists and offer the results as basic data that can be used to expand occupational therapy at adult day care centers. Methods : This study collected data by conducting an online questionnaire with managers and directors of adult day care centers located in Seoul, Busan, Incheon, Daejeon, Gwangju, and Ulsan. A total of 70 completed questionnaires were divided into two groups a 'group with experience in occupational therapy information (OT experience)' and a 'group with no experience in occupational therapy information (OT inexperience)' and analyzed. Results : First, the 'OT experience' was found to have higher levels of awareness of the scope of work of occupational therapists than the 'OT inexperience'. Second, significant differences were found in the degree of awareness and necessity of occupational therapists and plans to employ occupational therapists later between the 'OT experience' and the 'OT inexperience'. Third, it was shown that there were significant correlations between whether the respondents were familiar with occupational therapy and the degree of awareness and necessity of occupational therapists and plans to employ occupational therapists later. Conclusion : Based on these findings, the following measures are recommended to expand the area of occupational therapy in adult day care centers, the need for occupational therapists should be mentioned in the health and welfare-related education for workers at these adult day care centers, policies such as a medical fee system for occupational therapy at these adult day care centers should be prepared.

대상 데이터

본 연구는 2020년 02월 24일부터 03월 31일까지 서울, 부산, 인천, 대전, 광주, 울산지역에 소재하고 있는 노인 주간보호시설에 온라인 설문조사를 실시하여 자료를 수집하였다. 해당 온라인 설문조사는 ‘구글 설문’을 이용하여 지역별 노인 주간 보호시설의 대표 이메일로 발송하였고, 총 120부 중 81부의 자료를 수집하였으며, 누락 된 항목이 포함된 11부를 제외한 나머지 70부를 바탕으로 해당 자료를 분석하였다.
해당 온라인 설문조사는 ‘구글 설문’을 이용하여 지역별 노인 주간 보호시설의 대표 이메일로 발송하였고, 총 120부 중 81부의 자료를 수집하였으며, 누락 된 항목이 포함된 11부를 제외한 나머지 70부를 바탕으로 해당 자료를 분석하였다

데이터처리

수집된 자료는 작업치료 정보를 접한 경험 유무에 따라 두 집단으로 구분하였고, 대상자의 일반적 특성 및 작업치료사에 대한 인식 수준은 기술통계를 이용한 빈도 분석을 사용하였으며, 집단 간 동질성 검증을 시행하였다. 또한, 두 집단 간 작업치료사에 대한 관심도는 Mann-Whitney U 검증으로 분석하였고, 작업치료 정보를 접한 경험 유무와 작업치료사에 대한 관심도의 상관관계는 Spearman 상관분석을 통해 분석하였다. 통계프로그램은 SPSS 25.
수집된 자료는 작업치료 정보를 접한 경험 유무에 따라 두 집단으로 구분하였고, 대상자의 일반적 특성 및 작업치료사에 대한 인식 수준은 기술통계를 이용한 빈도 분석을 사용하였으며, 집단 간 동질성 검증을 시행하였다. 또한, 두 집단 간 작업치료사에 대한 관심도는 Mann-Whitney U 검증으로 분석하였고, 작업치료 정보를 접한 경험 유무와 작업치료사에 대한 관심도의 상관관계는 Spearman 상관분석을 통해 분석하였다.

성능/효과

넷째, 작업치료 정보의 경험 유무가 작업치료사의 인식도, 필요성, 향후 고용계획 사이에서 유의한 상관관계가 있는 것으로 나타났다.
다섯째, 작업치료사의 고용계획에 대한 부정적인 응답의 원인으로는 ‘이미 시설 운영에 대한 인력기준이 적합하다’가 가장 많은 것으로 나타났다.
둘째, 작업치료 정보 유경험 집단이 무경험 집단보다 작업치료사의 업무 범위에 대한 인식 수준이 높은 것으로 나타났다.
조사 결과, 작업치료사의 업무 범위에 대한 인식 수준은 모든 항목에서 작업치료정보 유경험 집단의 정답률이 무경험 집단보다 더 높게 나타났고, 작업치료사에 대한 인식도, 필요성, 향후 작업치료사 고용계획의 항목에서도 작업치료 정보 유경험 집단이 무경험 집단보다 통계학적으로 유의한 차이가 있는 것으로 나타났다. 또한, 작업치료에 대한 정보 경험 유무와 작업치료사의 인식도, 필요성, 향후 고용계획 사이에서 모두 유의한 상관관계가 있는 것으로 나타났다. 이는 노인 주간 보호시설의 종사자들이 작업치료에 대한정보를 직·간접적으로 경험한 것만으로도 작업치료사의 필요성을 인식하게 되고, 결국 작업치료사의 고용증가로까지 연결될 수 있는 밀접한 관련이 있는 결과라고 볼 수 있다.
본 연구에 참여한 전체 대상자 중 54.3 %가 작업치료에 대한 정보를 접한 경험이 있다고 응답하였고 그 중, 작업치료에 대한 정보를 접하게 된 매체는 보건복지 관련 교육이 가장 많은 것으로 나타났다. 이는 설문조사의 대상자 대부분이 사회복지사 또는 작업치료사이었고, 특히 치매 전문교육 또는 인지 재활 관련 교육을 이수한 사회복지사의 경우에는 작업치료에 대한 정보를 경험하였을 것으로 생각한다.
셋째, 작업치료 정보 유경험 집단이 무경험 집단보다 작업치료사에 대한 인식도, 필요성, 향후 고용에 대한 계획에서 유의한 차이가 있는 것으로 나타났다.
작업치료 정보를 접한 경험 유무와 작업치료사의 인식도, 필요성, 고용계획과의 상관관계는 모두 유의한 상관관계가 있는 것으로 나타났다(p<.05)(Table 5).
이에 따라 본 연구에서는 작업치료에 대한 정보를 접한 경험 유무에 따라 두 집단으로 구분하여 자료를 분석하였다. 조사 결과, 작업치료사의 업무 범위에 대한 인식 수준은 모든 항목에서 작업치료정보 유경험 집단의 정답률이 무경험 집단보다 더 높게 나타났고, 작업치료사에 대한 인식도, 필요성, 향후 작업치료사 고용계획의 항목에서도 작업치료 정보 유경험 집단이 무경험 집단보다 통계학적으로 유의한 차이가 있는 것으로 나타났다. 또한, 작업치료에 대한 정보 경험 유무와 작업치료사의 인식도, 필요성, 향후 고용계획 사이에서 모두 유의한 상관관계가 있는 것으로 나타났다.
첫째, 작업치료 정보 유경험 집단의 주된 작업치료사 정보 매체는 보건복지 관련 교육이 가장 많은 것으로 나타났다.

후속연구

따라서 노인 주간보호시설에 작업치료 영역이 확대되기 위해서는 노인 주간보호시설 종사자를 대상으로 보건복지 관련 교육에 작업치료사의 필요성에 관한 내용이 언급되어야 하고, 노인 주간보호시설에서 작업치료프로그램에 대한 수가체계와 같은 제도적 마련과 함께 작업치료사를 대상으로 커뮤니티 케어에 대한 체계적이고 전문적인 교육이 필요할 것으로 생각한다.
본 연구의 제한점은 수집된 자료의 수가 적어 본 연구에서 나타난 결과를 일반화시키는 것에 어려움이 있고, ‘작업치료사 고용계획이 없는 이유’에 관한 문항만으로는 작업치료사의 낮은 고용률의 전반적인 원인을 파악하는 것에 어려움이 있다고 생각한다. 따라서 추후 연구에서는 노인 주간 보호시설에서의 작업치료사 고용의 다양한 요인 분석이 필요하며, 다양한 지역 및 직종의 종사자를 대상으로 작업치료에 대한 정보를 전달한 후 인식도와 필요성의 변화 연구도 필요할 것으로 생각한다.
본 연구의 제한점은 수집된 자료의 수가 적어 본 연구에서 나타난 결과를 일반화시키는 것에 어려움이 있고, ‘작업치료사 고용계획이 없는 이유’에 관한 문항만으로는 작업치료사의 낮은 고용률의 전반적인 원인을 파악하는 것에 어려움이 있다고 생각한다
