Purpose : This study aimed to investigate the effects of dual-task training according to the variability of the walking environment on balance, gait, and function in patients with stroke. Methods : Twenty-five patients with stroke were randomly assigned to experimental group I (n=12) and experimental group II (n=13). Experimental group I and II performed obstacle and non-obstacle walking training, respectively, along with cognitive tasks for 21 minutes per session, 3 times a week for 6 weeks. Both groups received additional general physical therapy for 30 minutes per session. The functional reach test (FRT), gait analyzer (G-Walk), and functional independence measure (FIM) were used to evaluate balance, gait and function of pre- and post-interventions, respectively, while gait cadence, gait velocity, and stride length were evaluated using a gait analyzer. Results : In the within-group comparison of FRT, all the two groups showed significant post-intervention improvements (p<.05). In within-group comparison of gait cadence, all the two groups showed significant post-intervention improvements (p<.05). In within-group comparison of gait velocity, all the two groups showed significant post-intervention improvements (p<.05). In within-group comparison of stride length, experimental group I showed significant post-intervention improvements (p.05). In within-group comparison of FIM scores, experimental group I showed significant post-intervention improvements (p.05). There was a significant difference in the change of FIM scores pre- and post-intervention (p<.05) in the between-group comparison. Conclusion : The results of this study show that dual-task training with cognitive tasks and walking training can improve the balance, gait and function of patients with stroke, and obstacle walking training is effective for improving functions including activities of daily living compared to non-obstacle walking training.

본 연구는 뇌졸중 환자 25명을 인지 과제를 병합한 장애물 보행훈련군(실험군 1)과 인지 과제를 병합한 비장 애물 보행훈련군(실험군 2)으로 나누어 보행환경 변화에 따라 인지과제를 제공했을 때 대상자들의 균형, 보행 및 기능 변화량을 비교하기 위해 진행한 무작위 실험 연구이다. 또한 이중과제와 장애물 보행 훈련에 대한 중재 기초자료를 제공하기 위해 진행되었다.

제안 방법
실험군 1은 장애물 보행환경 내에서의 이중과제 훈련프로그램을 수행하였다. 10 m의 장애물 보행로에서 보행 훈련을 진행하며 동시에 무작위로 주어진 인지 과제를 수행하였으며, 1주 차는 장애물의 개수를 1개, 2주 차는 2개, 3주 차는 3개를 배치하여 난이도를 증진시켰고 4주 차에서 6주 차는 대상자에게 장애물의 위치를 알려주지 않고 고정된 위치의 장애물 보행환경에 추가적으로 장애물을 배치하였다. 고정된 위치의 장애물은 낮은 장애물, 스텝 박스, 경사면을 사용하였으며 낮은 장애물은 넘어가도록 지시하였고, 스텝 박스와 경사면은 마비측 다리의 안정성을 기반으로 비마비측 다리로 먼저 밟고 올라간 후 마비측 다리가 밟을 수 있도록 지시하였다.
실험군 1은 장애물 보행환경 내에서의 이중과제 훈련프로그램을 수행하였다. 10 m의 장애물 보행로에서 보행 훈련을 진행하며 동시에 무작위로 주어진 인지 과제를 수행하였으며, 1주 차는 장애물의 개수를 1개, 2주 차는 2개, 3주 차는 3개를 배치하여 난이도를 증진시켰고 4주 차에서 6주 차는 대상자에게 장애물의 위치를 알려주지 않고 고정된 위치의 장애물 보행환경에 추가적으로 장애물을 배치하였다.
추가적인 장애물은 폼롤러를 사용하여 장애물의 위치를 시각적으로 인지할 수 있는 범위 안에서 치료사가 무작위로 배치한 장애물을 닿지 않고 피해 갈 수 있도록 하였다. 실험군 2는 비장애물 보행환경 내에서의 이중과제훈련 프로그램을 수행하였으며, 장애물을 제거한 비장 애물 보행환경에서 보행 훈련과 동시에 인지과제를 수행하도록 훈련하였다(Table 1).
대상 데이터
본 연구는 C 시에 소재한 C 병원의 입원 치료를 받고있는 뇌졸중 환자 중 연구의 목적에 충족하는 조건을 갖추고 연구에 동의한 자를 연구 대상으로 선정하였다. 대상자 선정 기준은 뇌졸중으로 인해 편마비 진단을 받은 자, 한국형 간이 정신검사(K-MMSE)에서 점수가 24점 이상으로 의사소통이 가능한 자, 지팡이 보조 유·무에 상관없이 14 m 이상 독립 보행이 가능한 자, 기능적 보행 지수(Functional ambulation category; FAC)가 3점 이상으로 지시 또는 관찰 하에 신체적 접촉 없이 보행할 수 있는 자로 하였으며, 제외 기준은 편측 무시나 시공간적 장애가 있는 자, 정신질환자, 뇌졸중 이외의 다른 신경학적 질환이 있는 자, 심한 류마티스 관절염, 골관절염, 정형외과적 수술 경력이 있는 등 정형 의학적 장애가 있는 자, 청각 및 시각 장애가 있는 자로 설정하였다.

성능/효과

후속연구
장애물의 종류 및 장애물이 제시되는 형태를 다양하게 구성함으로써 장애물 보행 훈련의 임상적 효과를 증진시킬 수 있을 것이다. 뇌졸중 환자의 재활치료의 최종 목표인 지역사회 복귀를 위해 일상생활 활동에 초점을 맞춘 이중과제 훈련 프로그램은 지속적으로 개발되어야 하며, 뇌졸중 환자뿐 아니라 파킨슨병, 길리안 바레증후군과 같은 신경계 질환을 가진 대상자에게도 적용시킬 수 있는 중재가 추후에도 연구되어야 한다.
