이 연구의 목적은 머리자세와 목폄근육들의 근긴장도 및 근경직 사이에 상관관계를 검증하기 위해 실시되었다. 연구대상자는 20대 성인 80명으로 시상면에서 평소 머리자세와 목폄근육들에 근긴장도와 근경직을 측정하였다. 측정된 머리자세는 Image J를 사용하여 머리척추각, 머리회전각, 앞쪽어깨각을 분석하였다. 연구결과, 머리척추각이 감소할수록 위등세모근의 근긴장도 및 근경직이 유의하게 증가하고, 머리회전각이 증가할수록 위등세모근의 근긴장도 및 근경직과 유의하게 증가하는 상관관계가 있었다(p<.05). 그리고 연구대상자의 머리척추각을 기준으로 정상머리자세와 앞쪽머리자세로 재분류하여 비교한 결과, 앞쪽머리자세는 정상머리자세보다 위등세모근의 근긴장도 및 근경직이 유의하게 높은 특징을 확인하였다(p<.05). 이 연구결과는 임상에서 앞쪽머리자세의 평가 및 자세교육을 위한 기초자료로 활용되길 기대한다.

따라서 우리는 선행연구의 제한점을 보완하여 일차적으로 평소 머리자세에 따른 목폄근육들에 근긴장도와 근경직의 연관성을 확인하고, 이차적으로 연구대상자를 FHP와 NHP로 재분류하여 목폄근육들의 근긴장도와 근경직의 차이를 확인하여 FHP 관리 및 연구를 위한 기초자료를 제시하고자 한다.
이 연구는 전라북도 G시 소재 20대 성인 80명을 대상으로 평소 습관적인 머리자세(habitual head posture)와 목 폄근육들에 근긴장도와 근경직의 상관관계를 분석하고, NHP와 FHP의 차이를 비교·분석하기 위해 실시되었다
제안 방법
FHP와 NHP의 목폄근육들의 근긴장도 및 근경직의 차이를 확인하기 위해 총 65명의 NHP 중 무작위 추출 방법으로 15명을 새롭게 선정하여 비교·분석을 실시하였다
이 연구에서 머리자세 측정을 위한 환경 설정으로 연구대상자의 오른쪽 시상면(sagittal plane)이 측정될 수 있도록 의자를 위치시키고, 의자로부터 1m 거리에 카메라를결합시킨 삼각대를 연구대상자 얼굴 높이에 설치하였다. 그리고 CVA, CRA, FSA 측정을 위한 기준점 설정을 위해 연구대상자의 제 7 목뼈 가시돌기(spinous process of 7th cervical spine), 오른쪽 귀구슬(tragus), 오른쪽 위팔뼈의 중간지점에 표식지(land marker)를 부착하였다. 표식지 부착은 물리치료사 면허를 소지한 1인이 표식 부위를 촉진(palpation) 후 부착하였다.
따라서 우리 연구는 FHP의 선행연구들에서 많이 활용되고 있고 신뢰도가 검증된 사진촬영 방법을 사용하여 CVA, CRA, FSA를 측정요소로 설정하였다.
우리는 이차적으로 연구대상자의 CVA를 기준으로 FHP(CVA 49° 이하)와 NHP(CVA 49°~59°)으로 재분류 후 머리 자세와 몸폄근육들에 근긴장도 및 근경직을 비교·분석하였다
우리의 연구는 평소 머리자세에 따른 목폄근육들의 근 긴장도와 근경직의 상관관계를 확인하고, FHP와 NHP의목폄근육들에 근긴장도와 근경직의 차이를 검증하였다. 연구 결과, CVA가 감소하여 머리가 앞쪽으로 이동될수록 CRA, 위등세모근육의 근긴장도 및 근경직이 증가되고, CRA가 증가될수록 위등세모근육의 근긴장도 및 근경직이함께 증가되는 연관성이 있었다.

총 80명의 연구대상자 중 NHP 는 65명, FHP는 15명으로 분류되었다<표 1>.

성능/효과
FHP와 NHP의 목폄근육들의 근긴장도와 근경 직에 차이를 분석한 결과, FHP은 NHP와 비교하여 위 등세모근의 근긴장도 및 근경직이 유의하게 높은 특징을 확인할 수 있었다(p<.05).
또한, FHP는 NHP 보다 위 등세모근 육의 근긴장도 및 근경직이 높은 특징을 확인할 수 있었다. 따라서 FHP에서 나타나는 잘못된 머리 자세는 목폄근육의 근긴장도와 경직에도 부정적 영향을 미칠 수 있음을 제시한다.
연구 결과, CVA가 감소하여 머리가 앞쪽으로 이동될수록 CRA, 위등세모근육의 근긴장도 및 근경직이 증가되고, CRA가 증가될수록 위등세모근육의 근긴장도 및 근경직이함께 증가되는 연관성이 있었다. 또한, FHP는 NHP 보다 위 등세모근 육의 근긴장도 및 근경직이 높은 특징을 확인할 수 있었다. 따라서 FHP에서 나타나는 잘못된 머리 자세는 목폄근육의 근긴장도와 경직에도 부정적 영향을 미칠 수 있음을 제시한다.

후속연구
지금까지 우리 연구는 머리자세에 따른 목폄근육의 근 긴장도와 근경직의 연관성 및 FHP와 NHP의 차이를 확인하였기 때문에 운동역학적 변화와 연령별 비교·분석을 추진하지 못한 몇 가지 제한점이 있다. 그러나 잘못된 머리 자세가 목폄근육의 긴장도 및 근경직에 미치는 영향 및 연관성을 정량화하여 FHP 연구의 새로운 근생리학적 기초자료를 제시하였다. 우리의 연구결과는 FHP의 이해, 재활 평가, 헬스케어 관리에 활용할 수 있을 것으로 기대한다.
그러나 잘못된 머리 자세가 목폄근육의 긴장도 및 근경직에 미치는 영향 및 연관성을 정량화하여 FHP 연구의 새로운 근생리학적 기초자료를 제시하였다. 우리의 연구결과는 FHP의 이해, 재활 평가, 헬스케어 관리에 활용할 수 있을 것으로 기대한다.
